Abstract: 

Transient myopia is an unreported toxicity of docetaxel. We report an unexpected development of myopia following systemic chemotherapy with docetaxel. A 38-year-old woman undergoing chemotherapy for breast cancer received docetaxel every two weeks. Prior to chemotherapy, she had no visual impairments, ocular disorders, or other comorbidities. After seven cycles of systemic chemotherapy, she began experiencing visual impairment. Her spherical equivalent refractive errors were -2.00 D in the right eye and -1.63 D in the left eye. Uncorrected visual acuity was 0.42 logMAR in the right eye and 0.3 logMAR in the left eye, both correctable to 0 logMAR. At a 4-week follow-up after cessation of chemotherapy, her vision gradually recovered. To our knowledge this is the first reported case of docetaxel induced myopia in a patient undergoing cancer treatment in the English literature, underscoring the need for clinicians to be aware of this potential ocular toxicity.

Abstract: 

This is a case report of acute myopia following topiramate administration. There are several reports of this side effect recently and fortunately all of them were reversible. In our case we tried again topiramate with very low dose and increase it very slowly, the patient was followed and she tolerated it well and didn't find ocular problem anymore. So it could be suggested that starting with low dose topiramate may prevent recurrence of myopia.

Abstract: 

Acute myopia following use of some drugs is a relatively rare condition. Topiramate prescribed for seizure prophylaxis is one of these drugs. In this paper a case of acute transient myopia after taking topiramate is reported.The reported patient is a 28-year old woman who had no history of any eye disease. Following consumption of topiramate pill for one week, she had found blurred vision since one day prior to the admission. A complete ocular examination revealed acute bilateral myopia. This disease was resolved about 48 hours after discontinuation of topiromate. Due to the wide spectrum of indications for the administration of Topiramate, neurologists and psychiatrists should be aware of its ocular side effects. In conclusion, ocular complications following this drug should be taken seriously and be subjected to ophthalmic counsaltation.

Abstract: 

Purpose: To report a case of steroid induced ocular hypertension that caused keratitis and transient myopia after laser in situ keratomileusis (LASIK). Methods: Uncomplicated bilateral LASIK was performed in a healthy 29 year old woman with myopic astigmatism. Her preoperative refractive error was -11.50 -1.25×45° and - 5.25 - 2.5×18° in the right and left eye respectively. The patient had no history of ocular disease. After the procedure, she used topical steroids frequently for 2 weeks when she reported worsening of visual acuity specially in the right eye. Her UCVA was 1/10 in the right eye and 8/10 in the left. IOP by applanation tonometry was 37 mmHg in the right eye and 27 mmHg in the left. Slit lamp examination revealed corneal haze at the interface. Retinoscopy was -7.50 - 2.25×10° in the right eye and -0.50 - 2.00×15° in the left. Results: The topical steroid was discontinued. The patient was started on combined topical timolol and systemic Acetazolamide to lower IOP. After 2 weeks corneal haze disappeared. Retinoscopy showed +0.50-1.00×178° and +1.00-1.25× 119° in the right and left eye respectively. IOP measured by applanation tonometry was 15mmHg in both eye, UCVA was 9/10 in both eye. Conclusion: Pressure induced stromal keratitis (PISK) has not been well recognized. Early detection of this complication of LASIK is very important because the condition may be misdiagnosed as DLK but unlike DLK it would be caused or worsened by frequent topical steroids. Treatment consists of normalizing IOP and discontinuation of topical steroid. Elevated IOP after LASIK may be associated with myopia, prompt diagnosis and treatment would prevent glaucomatous optic neuropathy.

Abstract: 

The correlation between myopia and intraocular inflammation has rarely been explored. The aim of this article is to review myopic changes induced by inflammatory diseases and inflammatory diseases related to myopia, followed by a discussion on inflammatory choroidal neovascularization. Clinical cases are used to illustrate these conditions. The review does not include inflammatory conditions caused by surgical interventions employed for treatment of myopia. Uveitic conditions that can induce a myopic shift include sclero-choroidal inflammation, lens induced myopia due to steroid cataracts, juvenile idiopathic arthritis (JIA) induced myopia, and transient drug induced myopia due to sulfonamides and acetazolamide used for treatment of ocular toxoplasmosis and inflammatory cystoid macular edema, respectively. Most inflammatory conditions related to myopia are conditions involving the choriocapillaris.These include multifocal choroiditis and/or punctate inner choroiditis, multiple evanescent white dot syndrome and acute idiopathic blind spot enlargement. It can be hypothesized that fragility of the choriocapillaris due to particular anatomic changes due to myopia, together with unknown immunogenetic factors predispose myopic eyes to primary inflammatory choriocapillaropathies.

Abstract: 

Purpose: The possible effects of refractive errors on vestibulo-ocular reflex (VOR) has been a conflicting issue. The aim of this study was to evaluate the effects of induced myopia on VOR using the ocular Vestibular Evoked Myogenic Potential (oVEMP). Methods: In this cross-sectional quasi-experimental study, 35 emmetropic and normal subjects with the mean age of 23.89 ± 3.93 (range, 20–40 years) without any ocular, nervous system, and vestibular disorders, underwent the oVEMP test in the comprehensive rehabilitation center of Mashhad University of Medical Sciences. The oVEMP was performed under five different conditions of testing binocularly, monocularly, and when myopia was induced with the use of spherical lenses of +1.00, +3.00, and +5.00 diopters, respectively. There were 2 to 5 min of rest with closed eyes after each condition to avoid adaptation, fatigue, and any other sources of bias. Mean latencies of oVEMP waves (N1 and P1) and amplitudes of N1–P1 complex were measured. Results: There was no significant difference between the right and left sides (P > 0.05). The induced myopia significantly increased the N1 and P1 latencies using lenses of +1.00, +3.00, and +5.00 diopters but the amplitudes of N1–P1 complex were not influenced by the different amounts of induced myopia. There was no significant difference among the different conditions of induced myopia either (P > 0.05). Conclusion: Induced myopia could affect the VOR due to prolonging the latencies of oVEMP waves. However, the amplitudes were not affected and the effects of multiple degrees of induced myopia were not significantly different.
