Abstract: 

Objectives: This study was designed to investigate the possibility of dopaminergic involvement in obsessive-compulsive patients (OCD). Method: This double-blind study compared the efficacy of fluoxetine-haloperidol with that of fluoxetine-placebo in two groups, each comprised of 12 subjects. These subjects with obsessive-compulsive disorders were selected on the basis of DSM-IV diagnostic criteria; they were assessed by a demographic questionnaire and Yale-Brown Scale for evaluating obsessive-compulsive disorders. Findings: The results showed that the response to treatment in haloperidol-fluoxetine group was somewhat better than the fluoxetine-placebo group from the first week of the study. The difference was significant at the third visit (end of fourth week). Results: In addition to serotonin neurotransmitter system, dopaminergic system may play a role in pathophysiology of obsessive-compulsive disorder.

Abstract: 

Objective: To investigate the effect of fluoxetine as an SSRI on menstrual disorders.Materials and methods: In this cross-sectional study 200 women referring to the Islamic Azad University affiliated hospitals during March 2011-March 2010 were divided into 4 groups. All included patients in this study had the chief complaint of mood disorders in peri-menstrual period. All patients received daily vitamin B6 tablets as primitive treatment. In groups A, B and C patients also received daily doses of 10 mg, 20 mg and 40 mg of fluoxetine respectively for 6 months and in group D as the control group fluoxetine was not given. Data were analyzed by spss18 statistical software. Significant difference was set at p<0.05.Results: In the groups receiving fluoxetine (groups A-C) 34 patients and in control group 5 patients showed irregular menstruation started during fluoxetine administration and following 3 months. Significant difference with p<0.05 was found regarding the incidence of menstrual irregularity between groups with and without fluoxetine prescription.Conclusion: fluoxetine consumption leads to increased incidence of menstrual irregularity compared to the control group. The incidence increases with increasing dosage of the drug.

Abstract: 

fluoxetine is a Selective Serotonin Reuptake Inhibitor (SSRI) that exerts its anti-depressive effect by blocking the presynaptic reuptake of the neurotransmitter serotonin, 5-hydroxytryptamine (5-HT). Although fluoxetine is usually considered safe for most patients, in the present case report, we describe a young patient with Mixed Anxiety and Depression Disorder (MADD) treated with fluoxetine 10 mg/day, who developed hypotension when the dosage was titrated up to 20 mg/day. After discontinuing the use of fluoxetine, the symptoms of hypotension improved. A temporal association and dose-dependent relationship between the hypotension and the use of fluoxetine was observed. To the best of our knowledge, this is the first case report that precisely associates regular doses of fluoxetine with the presence of hypotension. Because boosting central serotonergic function lowers blood pressure, it is suggested that a significant effect of fluoxetine on the vasomotor center may be responsible for the reduction of blood pressure. Thus, physicians should be aware of the possible risk of hypotension induced by fluoxetine and recommend patients discontinue the drug immediately if complications have occurred.

Abstract: 

Objective(s): Due to cross-clamping of the aorta during aortic aneurysm surgeries, ischemia-reperfusion (IR) develops, and it may cause damage to the aorta itself or even to remote organs by oxidative stress or inflammation. fluoxetine (FLX) which might be used in the preoperative period for its tranquilizing effect also has antioxidant effects in short-term use. The purpose of our study is to examine whether FLX protects aorta tissue, against the damage caused by IR. Materials and Methods: Three groups of Wistar rats were formed randomly. 1) Control group (sham-operated), 2) IR group (60 min ischemia+120 min perfusion), and 3) FLX+IR group (FLX dose was 20 mg/kg for 3 days IP before IR). At the end of each procedure, aorta samples were collected, and oxidant-antioxidant, anti-inflammatory, and anti-apoptotic status of the aorta were evaluated. Histological examinations of the samples were provided. Results: Levels of LOOH, MDA, ROS, TOS, MPO, TNFα, , IL-1β, , IL-6, NF-kB, MMP-9, caspase-9, 8-OHdG, NO, and HA were found to be significantly increased in the IR group compared with control (P<0. 05) and SOD, GSH, TAS, and IL-10 levels were significantly lower (P<0. 05). FLX significantly decreased LOOH, MDA, ROS, TOS, MPO, TNFα, , IL-1β, , IL-6, NF-kB, MMP-9, caspase-9, 8-OHdG, NO, and HA levels in the FLX+IR group compared with IR group (P<0. 05) and increased IL-10, SOD, GSH, and TAS (P<0. 05). FLX administration prevented the deterioration of aortic tissue damage. Conclusion: Our study is the first study that demonstrates FLX-mediated suppression of IR injury in the infrarenal abdominal aorta by antioxidant, anti-inflammatory, and anti-apoptotic properties.

Abstract: 

Introduction: According to the high prevalence of PMS, and severe function and distress it causes, it’s important to compare the various treatments which are available for the disorder.Method and materials: We chose 450 18- year– old single female students from the Mashhad Medical University, and asked them to fill the inventories which were prepared based on the DSM–IV criteria for PMS. They were settled at some dormitories around the city. 409 of them answered the questions.PMS was diagnosed in 84 cases, using the inventories; and clinical and structurized interviews were performed with them. We divided them into two groups randomly. Some received fluoxetine (20 mg/ day) and others took multivitamin plus mineral capsules as placebo (one capsule a day for 3 months). This survey was a randomized and controlled clinical trial.Results: The improvement rate in the fluoxetine treated group was statistically significant higher than the placebo treated group (P <0.01). The symptoms, which best responded to drugs were depression, emotional. lability, anger, appetite changes, self-control disability, disinterest in usual activities, concentration difficulties, and sleep disturbances.Conclusion: Considering the cost benefits and availability of fluoxatine and its good effect on PMS (which has been proved in different researches), this drug with dose of 20mg/day is believed to be one of the best medications for PMS management.

Abstract: 

In this single-blind clinical trial, 68 patients with criteria of International Society of Headache suffered from at least two headache attacks per month were divided into two groups who were similar in sex, age, severity and number of headahe attacks per month. fluoxetine (20 mg per day) for experimental group and imipramine (50 mg per day), for the other group were prescribed for 12 weeks. During study, 8 patients in control group and 10 patients in fluoxetine group discontinued the trial due to adverse drug effects (P>0.05). There was no significant headache attacks between fluoxetine group [6.94±2.85headache attacks per month before treatment which changed to 3.48±3.11 attacks per month; p<0.05] and imipramine group [Z30±3.11 attacks per month to 3.08±2.84 attacks per month; p<0.05] (p>0.05). The severity of headache attacks were decreased in both groups, however, there was no difference between groups. fluoxetine has no contraindication in pregnancy, it could be prescribed in single dose. Therefore, it is recommended as a prophylactic choice measure for migraine headache.

Transvestic fetishism is having a sexual or erotic interest in cross-dressing. This case report describes a 17-year-old male with transvestic fetishism who suffered from obsessive thoughts and subsequent masturbation compulsion. He was managed successfully with fluoxetine.
