Abstract:

Introduction: Sarcomas account for 1% of all tumors originated in the head and neck regions in adults. They constitute a heterogeneous group of tumors of mesenchymal origin with multiple histological variants. The undifferentiated pleomorphic sarcoma (UPS) subtype is the most frequent variant with an incidence range of 2. 7-38%. Materials and Methods: This retrospective case series was conducted on 11 patients who were diagnosed with UPS and treated in our institution in the last 11 years. Results: According to the results, 26. 8% of the sarcomas corresponded to UPS variant. Out of 11 patients, 5 cases were female. The median age of the participants was 58 years (range: 18-74 years). Seven and four patients were T1b and T2b, respectively. Ten patients had no metastasis at the moment of diagnosis. Six patients were categorized as intermediate-grade, and the remaining (n=5) were classified as high-grade. The most frequent location was the scalp (n=3). In the patients undergoing surgery, the mean follow-up duration was 52 months. Disease-free survival (DFS) rates were obtained at 70%, 50%, and 50% in the 1st, 3rd, and 5th years, respectively, with the mean DFS of 36 months. In addition, the overall survival rates in the 1st, 3rd, and 5th years were 100%, 100%, and 83%, respectively. Conclusion: The UPS represented the most common histological subtype in our series. They tend to be intermediate or high-grade tumors. An acceptable global survival rate justifies surgical treatment as the main therapeutic tool.

Abstract:

Background: Bone sarcomas are the most common malignancies of bone tissues in children, and are classified into two groups as osteosarcoma and Ewing’s Sarcoma. Treatment and prognosis depend on the subtype and grade of the tumor. The goal of this study was to evaluate the features bone sarcoma in patients referred to MAHAK children hospital since 2007 to 2009.Materials and Methods: This was a retrospective study of patients with bone sarcoma referred to MAHAK children hospital for continuing their treatment or as a new case of cancer. All of the patients had local pain, local swelling, and decreased range of motion and pathologic fractures as the presenting symptom. All patients underwent bone scan, chest computed tomography (CT) scan, chest X- ray, echocardiography, hearing tests, and bone marrow aspiration/biopsy. All of the data were analyzed by SAS software. Results: In this study, 36 patients with bone sarcoma were included with the age range of 3 to 20 years old (55% male and 45% female). Analysis showed that local pain and local swelling were the most frequent local manifestations in patients. The most common primary tumor site was distal femur (30%). Treatment for the patients at MAHAK children hospital is performed according to German protocol. Out of 36 patients, 75% were followed until 2008 and 25% until 2009.Conclusion: According to the analysis we can conclude that these data are similar to other studies. To achieve the best results, oncologists have to modify the treatment of patients completely.

Abstract:

Background: Soft tissue sarcoma (STS) is a wide group of solid tumors with specific features originating from mesenchymal tissue. Radiotherapy (RT) and chemotherapy have been widely applied in the treatment of these tumors to enhance surgical outcomes. This study was performed aiming to compare the results of postoperative brachytherapy (BRT) versus RT in patients who underwent surgical resection of extremity STSs. Methods: In a prospective study from 2011 to 2015, 166 patients with extremity STS who underwent surgical resection were included. All visible tumors, scars, and drain sites, if present during the surgery, were resected. A number of 79 patients received adjuvant RT, and 87 patients underwent BRT after surgical resection. Results: In patients who were treated through RT, the two-year local control was 90% in comparison with 87% for those treated through BRT (P > 0. 050). The total radiation dose was 3869 ±,370 and 3048 ±,465 in the patients who underwent RT and in the BRT group (P < 0. 001), respectively. The number of radiation sessions in the RT group and BRT group was respectively 27. 3 ±,4. 5 and 5. 8 ±,2. 0 (P = 0. 001). Conclusion: Not only BRT can lead to similar local control and survival in comparison with the conventional adjuvant RT, but it can also decrease the total dose and number of radiation sessions in patients with STS of the extremity, which is a high grade, in individuals who underwent surgical resection.
