Abstract: 

Stem cells’,secretome contains biomolecules that are ready to give therapeutic activities. However, the biomolecules should not be administered directly because of their in vivo instability. They can be degraded by enzymes or seep into other tissues. There have been some advancements in localized and stabilized secretome delivery systems, which have increased their effectiveness. Fibrous, in situ, or viscoelastic hydrogel, sponge-scaffold, bead powder/ suspension, and bio-mimetic coating can maintain secretome retention in the target tissue and prolong the therapy by sustained release. Porosity, young’, s modulus, surface charge, interfacial interaction, particle size, adhesiveness, water absorption ability, in situ gel/film, and viscoelasticity of the preparation significantly affect the quality, quantity, and efficacy of the secretome. Therefore, the dosage forms, base materials, and characteristics of each system need to be examined to develop a more optimal secretome delivery system. This article discusses the clinical obstacles and potential solutions for secretome delivery, characterization of delivery systems, and devices used or potentially used in secretome delivery for therapeutic applications. This article concludes that secretome delivery for various organ therapies necessitates the use of different delivery systems and bases. Coating, muco-, and cell-adhesive systems are required for systemic delivery and to prevent metabolism. The lyophilized form is required for inhalational delivery, and the lipophilic system can deliver secretomes across the blood-brain barrier. Nano-sized encapsulation and surface-modified systems can deliver secretome to the liver and kidney. These dosage forms can be administered using devices such as a sprayer, eye drop, inhaler, syringe, and implant to improve their efficacy through dosing, direct delivery to target tissues, preserving stability and sterility, and reducing the immune response.

Abstract: 

Aim: The aim of the present study was to investigate the effect of human bone marrow-derived mesenchymal stem cells conditioned medium on fibroblast to myofibroblast differentiation. Background: Mesenchymal stem cells have a long-term clinical application and widely have used in autoimmune disease and regenerative medicine. However, some MSCs derived cytokines such as TGF-β could have a dual role in suppression or progression of disease. Fibroblast activation and extracellular matrix production are two key features of wound healing which mostly are controlled with multifunctional cytokine TGF-β 1. Methods: Bone marrow MSCs were isolated, cultured and used for conditioned medium preparation. The flow cytometry analysis was done for MSCs cell surface markers. MRC-5 subconfluent cells were starved with the medium containing 0. 5 % FBS for 24h, then treated with exogenous TGF-β 1 (10ng/ml as positive control) and MSCs-conditioned medium for 48h. Finally, the mRNA expression of three target genes: collagen I, collagen III and α-SMA were evaluated by RT-PCR technique. Results: Our findings demonstrated that bone marrow-derived mesenchymal stem cells-conditioned medium (secretome) significantly upregulated type I and III collagen expression but non-significantly α-SMA gene expression. Conclusion: Totally, Real Time PCR results suggest that MSCs conditioned medium activates differentiation of fibroblast to myofibroblast phenotype as confirmed through the presence of α-SMA, collagen I and collagen III expression compared to control in MRC 5 cells.

Abstract: 

The age-related degenerative joint disease known as osteoarthritis (OA) is typified by a gradual deterioration of cartilage. Once damaged, it is hard for joint cartilage to recover. The secretome is a factor secreted by cells, tissues, or organisms into the extracellular cavity under certain conditions and at a certain time. Studies have shown that injecting secretome into an OA knee can accelerate and stimulate cartilage repair. However, the evidence supporting this claim is limited. This study aims to analyze the effect of secretome injection on the microscopic characteristics of joint cartilage tissue, synovial membrane tissue, and subchondral bone tissue in experimental animals with osteoarthritis. An in vivo experimental study on the Wistar Rat was carried out. To create osteoarthritic conditions, type VII collagenase from Sigma-Aldrich was injected. A total of twenty Wistar rats were divided into two groups: control group and secretome-injected group (cartilage-derived secretome). Each group consisted of 10 rats. After 21 days, the experimental animals were terminated for microscopic analysis of cartilage, synovial, and subchondral bone tissues. The two groups were then statistically compared. It was found that cartilage erosion, subchondral bone erosion, and synovial membrane damage were significantly worse in the control group (p<0.05). It has been discovered that injecting secretome has a positive and protective effect on joints affected by osteoarthritis (OA). The administration of secretome on the osteoarthritic knee was found to be significantly protective, resulting in less damage to cartilage, synovial membrane, and subchondral bone tissue.
