Abstract:

Precocious puberty refers to the appearance of physical and hormonal signs of pubertal development at an earlier age than is considered normal. The onset of puberty before 7 or 8 years for white girls and 9 years for boys is considered precocious. Early onset of puberty can cause several problems. The early growth spurt initially can cause tall stature, but rapid bone maturation can cause linear growth to cease too early and can result in short adult stature. The early appearance of breasts or menses in girls and increased libido in boys can cause emotional distress for some children. Premature pubarche and premature thelarche are 2 common, benign, normal variant conditions that can resemble precocious puberty but are nonprogressive or very slowly progressive. A thorough history, physical examination, and growth curve review can help distinguish these normal variants from true sexual precocity. If the history, physical examination, and laboratory data suggest that a child exhibits early and sustained evidence of pubertal maturation, the clinician must differentiate central precocious puberty (CPP) from precocious pseudo puberty. Central precocious puberty, which is gonadotropin-dependent, is the early maturation of the entire hypothalamic-pituitary-gonadal (HPG) axis, with the full spectrum of physical and hormonal changes of puberty. Precocious pseudo puberty is much less common and refers to conditions in which increased production of sex steroids is gonadotropin-independent (Precocious pseudopuberty). Correct diagnosis of the etiology of sexual precocity is critical, because evaluation and diagnosis of patients with precocious pseudopuberty is quite different than that for patients with central precocious puberty.

Abstract:

Following the use of GnRH analogues for treatment of precocious puberty and their effect on preserving final height, physicians are more encouraged to treat the idiopathic type of this entity. In 1999, a study reported about 20 girls with slowly progressive and unsustained precocious puberty with mean age of 4.1±0.2 years who achieved normal final height without any treatment. Appearance of first signs of puberty in 95% of the population or 2 standard deviations below the mean age of puberty in normal girls is the definition of age limit of normal puberty. In 1997 a study conducted in Pediatric Research Network, based on pubertal staging of over 17000 girls between 3-12 years of age indicated that breast development occurred at 9.96±1.82 years of age in white girls and at 8.87±1.93 years in African American girls, about 1 year earlier in white girls and 2 years earlier in African American girls than what previous studies had shown. On the other hand, treatment of precocious puberty is based on suppression of hypothalamo-pituitary-gonadal axis with GnRH analogue, so this axis needs to be activated first. The interpretation of LH and FSH levels depends on laboratory methods of assessment, the latest being ICMA (immunochemiluminometric assay) which is more sensitive than older methods like radioimmunoassay. With the new ICMA method, basal LH>0.3 IU/L and with post GnRH test, peak LH>5 IU/L are diagnostic. The younger the age at onset of puberty, the longer the duration of puberty that results in a more prolonged pubertal growth spurt. The mean increase in stature after menarche is about 10 cm for girls with early menarche and about 5 cm for those with delayed menarche. Hence in this article, clinical and laboratory criteria and the age limit for defining precocious puberty are reviewed to distinguish those requiring treatment.

Data was collected by semi-structured interview and analyzed with a content analysis method. Results: Two themes and 6 subthemes were extracted from data analysis. Theme “ puberty challenges found by adolescents” consists of three subthemes including confidante chosen to talk about puberty, puberty confusion and information source about puberty. Second theme “ mother’ s view” consists of three subthemes including mother’ s concerns about puberty, mother’ s point of view about their daughter’ s information source about puberty and mother’ s awareness of their daughter’ s puberty challenges. Conclusion: According to mothers’ role as their daughter’ s confidante for educating puberty and on the other side, partial incompatibility of Challenges between mothers and adolescents, it seems necessary to empower mothers in effective communication with teenagers for better education of pubertal issues.

Abstract:

What is always referred to as puberty as the beginning of the responsibilities and limitations of the obligated human being is the attainment of a girl and a boy at a certain age,But regardless of its religious basis, puberty is a natural phenomenon with the actualization of sexual talent and reproductive powers. The criterion and rule specified in verse 6 of Surah An-Nisa '. However, many narrations and news have mentioned various signs to interpret this stage of sexual ability and have stated different ages for it. The studies of this research show that the jurisprudential arguments presented to determine age as a criterion of maturity for individuals can be criticized and damaged. At the same time, these arguments contradict the truth of maturity mentioned in the verse. Because attention to maturity in terms of ability and fertility is affected by many factors such as climate and climatic characteristics. That is why in the narrations, various ages have been expressed that have dealt with the subject of the need of the addressee of the word. However, this issue has not been much considered in the fields of jurisprudence and law, and dynamic jurisprudence is expected to have a fundamental review of the issue.

The main question that may arise is that what is the view of jurisprudence and law about the usual transactions that today are being carried out in our society by immature people? This article is an attempt to make a critical study or the views of jurisprudents and lawyers and their proofs in this regard, and finally with reference to the principles and rules of jurisprudence and law and considering the place of custom in transaction the author tried to work out a logical and justifiable solution for such transactions.
