Abstract: 

To determine and compare the prophylactic effects of i.v. dexamethasone 8mg with metoclopramide in preventing nausea and vomiting during intrathecal injection of meperidine for postoperative analgesia.In a randomized, double-blinded, placebo-controlled study, 105 ASA physical status I or II men ranging 18-40 years underwent elective lower limb orthopedic surgery receiving spinal anesthesia with 75mg lidocaine 5% and 15mg meperidine for postoperative analgesia, were enrolled. Patients were randomly allocated to normal saline (2ml saline, iv), methoc1opramide (2ml equal to 10mg, iv) and dexamethasone (2ml equal to 8mg dexamethasone, iv) groups and received their medications just before surgery. The incidence of PONV and side effects were evaluated during surgery, recovery period, 6 and 24hr after spinal anesthesia.The differences between demographic and hemodynamic variables, infused ringer duration of surgery, highest level of anesthesia, and number of patients requiring rescue antiemetics and analgesics and postoperative pain intensity were not significant. Patients who received dexmethasone and METOC1OPRAMIDE, have significantly lower incidence of nausea and vomiting compared who received saline in recovery period, 6 and 24hr after spinal anesthesia (p<0.05). The incidence of nausea in 6 and 24 hr and the incidence of vomiting in 24hr after spinal anesthesia were significantly lower in dexamethasone compared with METOC1OPRAMIDE (p<0.05).Dexamethasone, 8mg i.v., is suggested in preventing nausea and vomiting associated with intrathecal injection of meperidine for postoperative analgesia.

Abstract: 

One of the most common post - op complications in PACU is nausea and vomiting. this is more common in special surgical procedures like OBGYN surgeries or laparoscopic surgeries, Where complications are mostly encountered. Different therepeutic regimens have been used for this condition; among which Metoclopramide is the first choice. Cases of this study were subdivided into 3 groups, all of which were pregnant women below 30 years, undergoing cesarean section. None of the cases had confounding conditions like vertigo, pre -op nausea, etc. Samples were distributed in the 3 group randomly. The first group received 10 mg metoclopramide, stat, post - operatively; the second and the third groups received promethazine 25mg and dihidrobenzpoeridole 2.5 mg, along with a 10 mg dose of metoclopramide, respectively. All patients were checked both in the PACU and 8 hours after surgery, regarding nausea and vomiting. No significant difference was noted among the 3 groups regarding the symptoms.

Introduction:Postoperative nausea and vomiting (PONV) occurs in 20-30 percents of patients but is related to patient's condition, method of anesthesia and surgery. This study was done to compare the efficacy of ephedrine, METOC1OPRAMIDE and promethazine for prevention of PONV with placebo.Study design:The study was done as a randomized doublebline clinical trial on 80 patients with ophthalmologic surgery. patients received 10mg metoclopramide or 25 mg promethazine or 25 mg ephedrine or sterile water, intravenous and prophylactically at the end of surgery. Results:Incidence rate of nausea was 20 percents in control group and 10 percent in the other groups. The incidence of vomiting was 15 percent in control group and 10, 5 and zero percent in ephedrine, metoclopramide and promethazine receivers respectively, but the differences were hot statistically significant.Discussion:Prevention of PONV is recommended in high risk cases; According the results of this-study and literature, the use of serotonin receptor antaginists or metoclopramide is recommended.

Abstract: 

Introduction: Nausea and vomiting are a common manifestation in anatomic and functional disorders and these are current complication after general anesthesia and are unpleasent and with secondary complication. Nausea and vomiting are the most common complications after general anesthesia.Materials and Methods: In this study we compared three methods which one of them is droperidol with other methods consisting of IV METOC1OPRAMIDE and placebo. This controlled study was done in teaching Vali Asr hospital. Total number of patient was 300. Type of sampling was random. Type of operation was laparatomy and all of them had general anesthesia. We divided them in three equal groups (droperidol group, metoclopramide group and placebo group). In this study we used from history and examination and we finished puzzles and we use from statistication.Results: We concluded that there is valuable difference between metoclopramide and droperidol group (with considering P=0.000). The results were shown that: droperidol is more effective than metoclopramide in the prevention of postoperative vomiting after abdominal surgery.Conclusion: With comparison of these data we fined that droperidol is a suggestive and valuable method also effective method in preventing postoperative vomiting. Also finding were shown that there isn't a meaning relation between age the kind of abdominal surgery and postoperative vomiting.
