Abstract: 

Background and objectives: The orbital complication of paranasal sinusitis is rare, but any delay in diagnosis may leads to visual loss and is life threatening. Periorbital cellulitis primarily affects the young children. This study was conducted to evaluate the treatment outcomes.Materials and Methods: In this case series, 42 consecutive patients with orbital cellulitis secondary to sinusitis in two University-Hospitals of Tabriz University of Medical Sciences managed from June 1998 to Feb 2001were reviewed. Visual acuity with correction, clinical signs, methods of paraclinical and treatment were through a questionnaire. All Patients had similar findings: periorbital edema, erythema, proptosis and pain.Results: Fifty seven point one percent of patients were male and 42.9 percent were female. The patients’age ranged from 7 months to 68 years (50.1% were under ten years). Thirty three cases received medical and surgical treatments. The most common involved sinuses were maxillary and ethmoid (18 patients). After treatment, 78.6 percent of patients acquired complete recovery in the first week with penicillin G and metronidazole.Conclusion: Orbital cellulitis treated with antibiotics, but subperiosteal abscess is usually treated by incision and drainage and parentral antibiotics. CT and MRI commonly used in the diagnosis of orbital infections. Orbital cellulitis is a relatively common and potentially serious disease in children. Early surgical intervention is indicated if there is a risk of visual loss, or if no improvement is observed within 48 hours of starting medical therapy.

Abstract: 

The analysis of metalworking art in the Safavid era is a multi-layered and multi-faceted process that can be achieved by studying and separating the different layers, society, religion, politics and economics to the infrastructure of tradition and beliefs and rituals and components of religion among Iranians. With the aim of recognizing and examining metal bowls and cups of the Safavid period, this study tries to analyze and study the difference between the form and function of these vessels. In this regard, samples of metal bowls of the Metropolitan Museum have been examined and the themes used in them in the form of role, calligraphy and composition have been analyzed. The research method is descriptive-analytical and the results of the research show that the themes that were considered by the craftsmen and artists of that period in these bowls are sacred symbols for Shiites including the names of God, the fourteen infallibles (AS) and the symbols of Islamic mysticism. Including the composition of motifs in a circular shape and the repetition of a name, prayer or bit of Persian language poets, as well as national and Iranian symbols such as metal as a sacred and revered material, and finally a combination of national, Shiite rituals and mystical aesthetics. And it is sensory.

Abstract: 

Background: Brain abscess is one of the most important diseases among the neurosurgical infectious diseases which is accompanied by considerable mortality and morbidity. The aim of this research is contemplation of brain abscess in children (5-12 years) to ascertain the incidence, effectual underlying factors, clinical and laboratory findings and mortality and morbidity rates in Ghaem hospital in Mashad since 10 years ago. Methods: This is a descriptive and analytic study in children with brain abscess in Ghaem hospital achieved retrospectively. We considered age, sex, underlying factors, clinical and laboratory findings, location of abscess, methods of treatment (medical or surgical), the duration of staying in hospital, and mortality and morbidity of the disease. Statistical analysis was achieved with Odds Ratio and Chi2 test.Findings: The age of children with brain abscess was 5-7 years. Male to female ratio was 1.6 to 1.The most common underlying factors for brain abscesses were acute and chronic diseases of ear, mastoiditis, and chronic cyanotic heart disease (CCHD). The patients had fever (55.5 %), headache (46.6%), focal neurological findings (64.6%) and altered consciousness in 46.7%. The most common sites of involvement were: temporo-parietal (20%) and parietal lobe (18.2%). Pus culturing was positive in 34% of cases. In 73.3% of cases one organism and in 26.6% several organisms were found in the culture. We achieved aspirated type operation in 93.3 % of cases. The incidence of mortality and morbidity was 17.7% totally. The mortality and morbidity rates in patients with CCHD and otogenic brain abscess were 11.1% and 22.2% respectively. There was no meaningful correlation between mortality and morbidity with the number of abscesses or patients age.Conclusion: Attention to hygiene of mouth and teeth in pediatric patients with cyanotic heart disease is very important in prevention of infectious brain diseases. We must also consider attention to ear-nose-throat diseases of children. It is very important that the patients are examined again after treatment because of finding and treatment of unknown otitis and chronic sinusitis.

Abstract: 

Background & Aims: The orbital cavity is an important anatomic structure in the face. It is in close connection with the skull, paranasal sinuses, and the nose. Precise knowledge of the orbital cavity dimensions can be helpful in the diagnosis and treatment of diseases associated with the orbital cavity. Due to the lack of adequate research on orbital size in Iran and the city of Kerman, the aim of this study was to evaluate the proportions of the orbital fossa of women and men in Kerman.Methods: In the present study, a sliding caliper and a compassed caliper were used for the measurement of variables. The variables included orbital width, orbital height, and inter-orbital distance. The variables were measured in 2000 subjects in the age range of 20-30 years.Results: The mean orbital height and width in men was 35.8 and 41.6 mm, respectively. The mean orbital height and width in women were 23 mm and 33.1 mm, respectively. In addition, the mean inter-orbital distance in men and women was 31.9 mm and 24.1 mm, respectively.Conclusion: In this study, the statistical analysis of the variables showed no significant difference in orbital height, width and inter-orbital distance between men and women.

Abstract: 

From ancient time, use of magic and sorcery was prevalent among people of different countries. Ancient man lived among a host of frightening, negative demonic forces which he attempted to control or coerce in order to ensure a harmonious and benevolent word-order. They believed on effect of magic on banishing demons and evils and had different ways for making use of them. A vast array of Magic-religious mechanisms evolved to cope with this problem in Sasanian period, including incantation bowls which were produced in vast quantities in Mesopotamia and Khuzistan more specifically in Susa during the sixth and seventh century CE., or may suppose the practice began somewhat earlier, in the fifth or even fourth century C. E. Also some Sasanian seals with special pictures and texts are supposed to be made for purpose of protection against the disease, the evil eyes. The peculiar nature of the incantation bowls raises many questions to which no satisfactory answer has yet been given. The reason for their sudden emergence in the fifth or sixth century CE remains obscure; moreover, the practice seems not to have spread outside a limited geographical region. An incantation bowl, also known as a demon bowl or devil trap bowl, is a form of early protective magic. The bowls were usually inscribed in a spiral, concentric circles beginning from the rim and moving toward the center and may be read in some cases by turning the bowl clockwise and in others counterclockwise. There are also deference’ s in language and script: the texts on extant bowls are in Aramaic, Syriac, Mandaic or Pahlavi (Middle Persian). The language of the bowls is some kind of strange magical jargon incomprehensible to all but demons. It contains formulaic expressions and oral compositions which show a certain play on words normally not found in written texts. They are varying in size, shape, and type of clay. In shape, they are either hemispherical with round or flat bottoms or truncated cones with flat bases. The bowls were buried usually face down but also occasionally face to face with another bowl to form an enclosure and were meant to capture demons. According to a tradition a little salt was placed between two such bowls before they were buried. They were commonly placed under the threshold, or a gate or courtyards, within a household at the four corners of a room or bed and also in graveyards. The study of Incantation bowls is a matter of great interest and importance. They are an important source of knowledge which shed lights on old cultures, practices, beliefs. They constitute the only direct epigraphical documents of Mesopotamia, written mostly by some of the minority groups like Mandaean, Christians, Jews and others. Names that use in texts, may tell us about a person’ s religion. We find clients with typically Persian (presumably Zoroastrian), names in bowls that imply despite the magical powers commonly attributed to the Zoroastrian clergy themselves, some people sought help from sorcerers outside of their community. Since the bowls are original manuscripts, they can teach us a great deal about popular religious practices and certain aspects of social life, family, structure and dwellings and provide precious evidence of how these ancient religious traditions survived or were transformed in one corner of late antique world. This article aims to introduce these two ways of averting evil forces from life in Sasanian period and answer these questions: what was public opinion especially in the Sasanid era about magic and also what kind of information the incantation bowls provide. In introduction of this research paper, first explain about magic and sorcery and also give a short explain about bowls and seals. Afterward, illustrate about bowls and seals separately, showing belief of people and ways of using magic and sorcery in ancient time.

Abstract: 

We describe a case of orbital apex syndrome due to Perineural Spread (PNS) of forehead Squamous Cell Carcinoma (SCC). This type of tumor spread occurs rarely and has a highly aggressive nature. Therefore, the high level of suspicion for perineural invasion is required and early diagnosis increases the likelihood of success in treatment.

Abstract: 

In two recent decades, nucleus independent chemical shift (NICS) criterion was used to gauge the amount of aromaticity of organic and inorganic compounds in a lot of publications. In this study, aromatic, nonaromatic and ant iaromatic character in different rings of coronene (C24H12), corannulene (C20H10), sumanene (C21H12) and circumtrindene (C36H12) bowl-shaped nanostructures, which are fullerene (C60) substructures was evaluated by this criterion. In the first, structures were optimized at B3LYP/6-311+g (d, p) level of theory and then analytical frequency calculations were done at the same level to ensure achieving the correct structures. NMR calculations were also done at the same level of theory to determine the magnetic shielding tensors in order to calculating NICS and NICSzz at the polygon surface and 1 Å above and below it for different rings of each molecule. These parameters were used to discuss local aromaticity, nonaromaticity and antiaromaticity character and were also used to determining overall aromaticity character of aforesaid four molecules. Results showed that NICS or NICSzz values are more negative for concave face than convex face for each molecule. Average of the NICSzz for coronene, corannulene, sumanene and circumtrindene molecules are equal to -21.57, -7.46, -16.13 and -0.52, respectively, therefore coronene and sumanene are very aromatic, corannulene is slightly aromatic and circumtrindene is nonaromatic.
