Abstract: 

Sexual impotent is very common entity and define as inability to induce or maintaining penile erection during Sexual intercourse. Studies reveals about 10% of men aged 40-70 years have complete, 17% have mild and 25% have moderate amount of erectile dysfunction.The Sexual Impotence has different etiology including psychogenic, neurogenic, arteriogenic and venogenic causes.Current studies reveal that organic cause of impotent is a bout 50-90% cases.Among them vascular reasons including arterial insufficiency or venous incompetence contributed 50-70% cases (arteriogenic Impotence 30%, venogenic Impotence 15% and 25% mixed Venus and arterial).Diagnosis of an organic cause of Impotence is very important because is could be curable.The cavernosal arteries are the main feeders to penis and where the erection is performed. Selective angiography with selective internal pudendal is an invasive but the gold standard in evaluation of penile arteries in evaluating arteriogenic Impotence.This technique is invasive and is not suitable as a first step or screening examination.Color doppler imaging could be a suitable technique inassessement of vasculogenic Impotence during injection of an intra cavernosal vasodilating pharmacological agent and observing the response.By color doppler imaging blood velocity mof cavernosal arteries could be measured before and after intracavernosal injection of vasodilating agents.Also increase in size of the vessel diameter (0.75%) is an indication of normal arterial flow. A suitable vasodilator could be intrcavernosal injection of 60 mg of Papavarine in a 2 ml solution into right or left corpus cavernosum.Alternative drugs are cocktail of triple agent consisting of Papavarine 4.4 mg, phentolamine 0.15 mg and PG-E1.Drug induced priapism may occur in 2-3% of the patients.The patient can be divided into normal, with an average PSV of 47 cm/second, B. Mild to moderate with average peak systolic velocity (PSV) of 35 cm/second and C. Severe arterial insufficiency with an average PSV of 7 cm/second.Generally peak systolic velocity of 40 cm/second was normal.The parameters that indicate the presence of arterial disease are a subnormal clinical response to vasoactive agents, a less than 60% increase in the diameter of the cavernosal artery, and a peak systolic velocity of the cavernosal arteries less than 25 cm/second.In the presence of normal arterial function, doppler findings suggestive of an abnormal venous leak are persistent end diastolic velocity of the cavernosal artery greater than 5 cm/second and demonstration of flow in the deep dorsal vein.The development of diastolic flow reversal after an injection has been found to be a reliable indicator of venous competence.In conclusion color duplex doppler ultrasoung with intracavernosla injection of vasodilator. Sonography is an excellent and accurate means of assessing patients with erectile dysfunction. It is a minimal invasive test with low complication rate and can produce good results and should replace angiography and cavernosometry as primary or screening test.

Abstract: 

Introduction: Vasectomy is a safe and effective method to prevent fertility. This method is practical in most regions because it doesn’t require complex technology. In spite of this in some regions of Iran it hasn’t been approved because of general believes that vasectomy cause Impotence. Purpose of this study is to evaluated 2 items libido and Impotence between vasectomies.Materials and Methods: In this cross-sectional study 110 vasectomied persons who had referred to Khoram abad health centers during 1999-2002 were participated. Demographic information, libido and Impotence variation evaluated.Results: After necessary evaluations of the cases it was found that 60% of people hadn’t any libido variation and even 30% had increased libido, also 85.5% of them were seen without Impotence.Conclusion: In this study vasectomy as a safe and successful method for contraception was prone. In most cases it hasn’t side effect such as libido and Impotence reduction, therefore vasectomy in families with optimal number of children to prevent fertility is recommended.

Abstract: 

The phenomenon of Devotism under the title of rape of the disabled has not been researched and studied much. To verify whether devotees have characteristics suggestive of a paraphilia behavior, an ad hoc internet questionnaire was developed to study a population of 209 subjects enrolled from online devotee communities. In the first observations, we found that the vast majority of these people committed this act because of Sexual activity. The other group was led to this act by encouraging the stimulation of the disabled person, this group of people, unlike the previous group, will have a healthy Sexual intercourse without any discomfort due to Sexual preferences in having sex with disabled people. Future research is necessary to confirm the information obtained from this research. This article has been written in the form of translation and analytically and statistically, and it aims to answer the question of how the rape of the disabled is and at what level the statistics of this rape will be in the language of statistics.

Abstract: 

Introduction & Objective: Sexual dysfunction is called to inability to get or keep a successful Sexual intercourse. The term “Sexual dysfunction” may also be used to describe other problems that interfere with Sexual intercourse, such as inability to achieve erection and problems with ejaculation or orgasm. Damage to nerves, arteries, smooth muscles and fibrous tissues, often as a result of a disease, are the most common cause of erectile dysfunction. The aim of the present study was to evaluate the frequency of Sexual dysfunction in patients referred to Fatemieh infertility clinic, Hamadan.Materials & Methods: In this cross-sectional study all males referred to Fatemieh infertility center in 2005, were entered the study. All patients were clinically examined and their medical histories were obtained. Data such as age, accusation, education, history of smoking and opioid abusing, Sexual dysfunction, inability to achieve orgasm, premature ejaculation and the frequency of intercourse were entered in the questionnaires. Data was analyzed using SPSS 13 and c2 statistical test.Results: The mean age of patients studied was 32.3±7.4 years (20-65 years). From 245 cases, 135 (55.1%) had Sexual dysfunction. The most common Sexual dysfunction was premature ejaculation(38.8%). Other Sexual dysfunctions were Impotence with 16.3% , lack of Sexual desire with 12.2% and orgasm disorder with 10.6% respectively. 65.9% patients were uneducated or had elementary education , 27.4% had diploma , and 6.7% had academic education. 23.7% of patients had addiction and 34.1% were smoker.Conclusion : This study showed that most common etiology of Sexual dysfunction was premature ejaculation. Other Sexual dysfunctions were: Impotence, Sexual desire disorder and orgasm disorder respectively.

Abstract: 

As a result of permanent marriage, a marital relationship is established between husband and wife, and duties and rights are created for each of them in relation to the other. Some of these rights belong to the wife that the husband is obliged to do them. The right of having Sexual intercourse or the right of Movaqe'ah is one of them. The jurists believe that the husband should not leave Sexual intercourse with his spouse for more than four months and according to this perspective, in this article the question arises that if Sexual intercourse is considered as a woman's right, what an executive guarantee is provided for it? If the man does not observe this right of the wife, does the woman have the right to sue and complain, or does she have to wait and be patient with this cruelty and oppression until the man pass away. In this article, by referring to valid jurisprudential reasons, we have proved that if the husband violates this right or if he is unable to do so due to his disability and Impotence, such as the case where the husband suffers from defects such as impotency that cannot use the right of termination, the judge can oblige the husband to divorce the wife according to the wife's divorce request and if not possible, the judge will divorce her, proving this issue is the main purpose of this study.

Abstract: 

Introduction: Erectile dysfunction (ED) is a common Sexual dysfunction, which affects the quality of life in couples. The use of acupressure for treating ED has been reported in various studies. This systematic review was performed to evaluate the current evidence for the efficacy and safety of acupuncture in treating ED.Methods: In this systematic review, electronic databases of SID, IranMedex, Magiran, IranDoc, Scopus, PubMed, Cochrane, and Google Scholar were searched up to November, 2017 to identify all the studies reporting the effect of acupuncture for treating ED. The search was performed using the keywords of “acupuncture”, “electro-acupuncture”, “intradermal needling”, “erectile dysfunction”, “Impotence”, and “male Sexual dysfunction”. The quality of the included studies was assessed using the Jadad scale. The results were reported qualitatively.Results: Among 1274 articles retrieved in the initial search, five clinical trials (228 participants) were selected for this systematic review. The studies were conducted in Austria, China, the Netherlands, and Turkey. Most studies showed the beneficial effects of acupuncture for treating ED. Some studies investigated the possible side effects of acupressure and reported no serious adverse effects in this regard.Conclusion: Acupuncture for treating ED can be an effective method, but due to the poor methodological quality in the majority of trials, more high-quality studies with larger sample sizes are needed to confirm these findings.

Abstract: 

Objective(s): To validate Persian version of the FSFI and to determine its diagnostic cut off score. Methods: This was a descriptive analytic study. To fill in the Persian version of the FSFI. In addition clinical interviews were carried out by a psychiatrist on DSM-4 criteria. Women divided in two age and educational matched groups: the control group (n=28) who have not Sexual dysfunction and the case group (n=53) who have Sexual dysfunction. Data were analyzed by Cronbach’s alpha and validity was assessed by comparing mean scores of two groups. Optimal cut off score determined by Receiver Operating Characteristic (ROC) curve. Results: The findings showed satisfactory results for full scale and each subscale with Cronbach’s alpha ranging from 0.66 or above for Sexually dysfunctional sample and 0.70 or above for Sexually active sample. Discriminant validity confirmed the ability of both total and domain scores to differentiate between active and inactive women. On the basis of sensitivity and specificity analyses we found that the optimal cut off score was found to be 28 for differentiating women with and without Sexual dysfunction. Conclusion: The findings showed that the Iranian version of Female Sexual Function Index (FSFI) is a reliable and valid instrument for measuring Sexual function in women.

Abstract: 

Background: Successful rehabilitation of cardiac surgery patients should include consideration of their Sexual activity, but there is paucity of data regarding this matter.Objective: This study determined the incidence and type of Sexual dysfunction in our patients.Materials and Methods: Two hundred-seventy nine men with age under 70 years old who had coronary artery bypass graft (CABG), valvular, or other types of cardiac surgery from Dec.2006 until Dec.2007 were enrolled in this descriptive-analytical study. They were interviewed before and 12 weeks after the operation in regard to the impact of surgery on their Sexuality. The statistical methods used included analysis of variance, Kappa test, and chi-square analysis.Results: The mean age of the patients was 55.7±10.66 (25-69) years. The incidence of Sexual dysfunction was 20.1% before, and76.4%, 12 weeks after the operation. P-valueas tested by Kappa test was 0.0001, which means that cardiac surgery had adverse effect on Sexual activity of the patients. Types of Sexual dysfunction were Impotence, premature ejaculation, and decreased or loss of libido in 6.5%, 4.3% and 9.3%, respectively before operation, and 34.8%, 21.5% and 20.1%, respectively 12 weeks after the operation. Concurrence of more than one dysfunction was not reported.Conclusion: Sexual dysfunction is common after cardiac surgery, and Sexual counseling is still not being addressed adequately. The role and responsibility of the physician and the rehabilitation nurse becomes evident, together with the need for the patient’s partner to participate in counseling.
