Abstract: 

Objective: Increasing cost of hospitalization, family separation and superimposed due to home care attention is considered in schizophrenic patients. Recovery of chronic disease in schizophrenic not predictable for the time in future care Nursing services, purpose of this study is determination of home care service effectiveness for care of for schizophrenic patients.Materials & Methods: In this experimental study 48 schizophrenic patients were selected and divided two groups as a random allocation (24 patients in intervention group and 24 patients in control group). For intervention group, Psychiatric home care services after discharge were done every 15 days for 3 months. For control group routine predetermined date for next visit was done. Both groups assessed and compared for their Psychiatric situation by using Brief Psychiatric Rating Scale (BPRS).Results: Findings showed, rate of BPRS in the discharge time, and after 3 month follow up in interventional group was better than control group. This different was found to be significant (P = 0.007).Conclusion: A home care service is effective for schizophrenic’s Psychiatric situation. Nursing care may be continued after their hospital discharge at the home.

Abstract: 

Background & Aim: Communicating with a Psychiatric client due to the impairment in their cognitive, emotional and behavioral processes needs essential skills. Regarding the importance of acquiring knowledge of real experiences of Nursing students in practice setting and improving the quality of education and considering the capabilities of qualitative approaches, this study was done with the aim of the recognition of Nursing students’ experiences of communication with Psychiatric clients.Material & Methods: It was a qualitative phenomenological study. Fifteen female Nursing students of Islamic Azad University were recruited by purposeful sampling. Data was collected by in-depth unstructured interviews until saturation and analyzed using Colaizzi method.Result: Ninety codes were emerged from the interview manuscripts that were classified into four major themes including: learning communication skills, task-based communication, Psycho-social adaptation, and Anxiety.Conclusion: Knowledge of students’ lived experiences helps to better recognition of their communicative problems with Psychiatric clients.

Abstract: 

Introduction: Clinical competency in Psychiatric Nursing education provides the foundation for the development of competency in Nursing practice. There are different definitions of clinical competency, but there is no certain definition for it in Psychiatric Nursing. The purpose of this study was to explore the definition of and method of achieving clinical competency in Psychiatric Nursing students.Method: In this qualitative study semi-structured interviews were conducted with 16 participants (5 undergraduate senior students in their last term, 2 MSc students in the Psychiatric field, 4 faculty members of the Department of Psychiatric Nursing, 3 head nurses, and 2 nurses working in Psychiatric wards). In addition, we used two focused group interviews with Nursing students. All interviews were recorded and transcribed. The data were analyzed using conventional qualitative content analysis method.Results: During data analysis, four main themes emerged; preparation and familiarization, confrontation, involvement, and competency.Conclusion: Undergraduate Nursing students need to pass specific stages before attaining clinical competency as a mental health nurse. Each stage has specific factors that may help the students on their path.

Abstract: 

Introduction: As to the significance of clinical competency, the competency concept should be up-to-date periodically until clinical competency is evaluated based on it. This study aimed to develop and evaluate psychometric properties of a new tool to measure the postgraduate Psychiatric Nursing students’,competencies. Methods: The current study was conducted with a sequential exploratory mixed-method, in Iran, in 2019-2022. The qualitative part was conventional content analysis, and the quantitative part was a methodology study. The questionnaire was developed by the item generation via individual semi-structured interviews with 21 participants sampled purposefully, and a literature systematic review. In the quantitative phase, psychometric analysis was performed based on consensus-based standards for the selection of health status measurement instruments (COSMIN) criteria, and using face, content, and construct (i. e., convergent, known group, and exploratory factor analysis done on 199 postgraduate Psychiatric Nursing students by available sampling) validity. t-test analysis was used to compare the clinical competence ratings of two groups of experienced and inexperienced postgraduate Psychiatric Nursing students. Additionally, dependability was examined for internal consistency, stability over a one-month period, and measurement error. The sampling technique used for content validity was deliberate. Then, the responsiveness (through minimally detectable changes), and interpretability (through minimal important changes) were calculated. Results: The questionnaire consisted of 43 items. Construct validity assessment via exploratory factor analysis (EFA) showed that 67. 53% of the cumulative variance was explained by two factors: “, Education and Nursing care”,(23 items) and “, Evidence based Psychiatric Nursing interventions”,(20 items). The convergent validity with one golden standard instrument was 0. 49. The difference of the clinical competency scores of the two groups of experienced and novice was significant (P<0. 001). The internal consistency of the entire instrument, and the first and second factors analyzed using alpha Cronbach (α, ) were respectively 0. 947, 0. 897, and 0. 891. Stability was confirmed by the ICC agreement 0. 956 for interrater (CI 0. 907-0. 980). Standard error of measurement was 3. 14. The competency score of students based on their demographic information was not significant (P>0. 05). Conclusion: The 43-item postgraduate Psychiatric Nursing students’,clinical competency questionnaire is a valid and reliable newly developed instrument. Further studies are recommended to be conducted to assess competency with the largest sample size to promote instrument.

Abstract: 

Introduction: Psychiatric home care programs are changing rapidly in response to the increasing numbers of acut Psychiatric patients in community and the newly created competitive health care market. Some of this growth is a result of the proven efficacy of Psychiatric patient in a cost-effective manner. The aim of this study is to assess the effects of home Psychiatric Nursing care in patients with schizophrenic disorders after being dischaged.Materials & Methods: The study was a quasi-experimental research design. The participants included 60 patients diagnosed with schizophrenic disorder; case and control groups. The samples were selected by access sampling .The case group were provided a 3-month home care after being discharged and evaluated 6 months later.Findings: The results showed the following in the experimental group after home Psychiatric Nursing care: an improvement in the quality of life, stopping unnecessary hospitalization, facilitating more rapid discharges, prevention of symptomatic recurrences and better psychotherapeutic drug orders than the control group.Conclusion: Work with families to reduce relapses was always seen as an adjunct to maintain the medication and not as a substitute for it. Indeed, family therapy, when added to antipsychotic medication, has been shown to be more efficacious than medication alone in preventing relapses in schizophrenia.

Abstract: 

BACKGROUND: Technical skills result in the patient’, s safety. Psychiatric nurses need to learn and apply them to provide effective and secure cares. This study explains the technical competency required for postgraduate Psychiatric Nursing students. MATERIALS AND METHODS: This conventional qualitative content analysis study is part of a larger study on the clinical competency of postgraduate Psychiatric Nursing students performed in Iran in 2020. A qualitative study was conducted at universities holding master’, s degree courses in Psychiatric Nursing. Participants were instructors and postgraduate Psychiatric Nursing students, Psychiatric nurses, psychologists, and psychiatrists selected by purposeful sampling method. Sampling continued until data saturation was achieved. Data were collected through a semi‑, structured individual interview and were analyzed by conventional qualitative content analysis. RESULTS: The “, technical competency”,has two subcategories of “, specialized knowledge”,(including general Nursing knowledge, Psychiatric Nursing knowledge, and basic knowledge of psychology) and “, application of knowledge in practice”,(including clinical experience, skills of Psychiatric Nursing interventions, educational skills, and research and evidence‑, based practice skills). CONCLUSION: The Psychiatric Nursing student learns and operates knowledge of Psychiatric Nursing, psychology, education, and research, besides improving their knowledge of general Nursing.
