Abstract: 

Introduction: PROLIFERATIVE VITREORETINOPATHY (PVR) is distinguished by creation of cellular membrane on two sides of retina and vitreous body. The main aim of this study was evaluating the effect of encircling band in patients with retinal detachment complicated by PROLIFERATIVE VITREORETINOPATHY.Method & Material: This clinical trial study was conducted on 53 eyes with retinal detachment complicated by posterior PROLIFERATIVE VITREORETINOPATHY. vitrectomy combined with encircling band in a group of 25 eyes and vitrectomy without band in another group of 28 eyes was performed. Both patients were followed for a period of 4 to 18 months. Results: In 92% (23 eyes) of the eyes from first group, the retina was successfully reattached. This rate for the second group was 71%. The rate of successful anatomic retina was not significant in two groups. 16 eyes (64%) from the first group had a post operative visual acuity of 5/200 or better. In the second group 20 eyes (71%) out of 28 eyes had post operative visual acuity of 5/200 or better. In aphakic eyes there was no difference in two groups, but in phakic retinal detachment complicated by PROLIFERATIVE VITREORETINOPATHY (PVR) there were more cases of redetachment in the second group (without band) (p<0.05). Conclusion: We recommend vitrectomy combined with encircling band for phakic retinal detachment complicated by posterior PVR, but we don’t advise encircling band for aphakic patients with posterior PVR.

Abstract: 

Purpose: To report the anatomical and functional outcomes of retinotomy and/or retinectomy for the management of rhegmatogenous retinal detachment (RRD) complicated by advanced PROLIFERATIVE VITREORETINOPATHY (PVR). Methods: In this retrospective study, the charts of patients who underwent pars plana vitrectomy with retinotomy and/or retinectomy for the management of RRD complicated by PVR were reviewed. Primary outcome measures were final best-corrected visual acuity (BCVA) and anatomical reattachment rate. Results: Sixty-one eyes of 61 patients with a mean age of 48. 56 ±,15. 92 were studied. The mean follow-up time was 21. 38 ±,23. 08 months. The mean angle of the retinotomy was 171. 31°,±,79. 15°, . Thirty-two (52. 5%) of them needed extensive (≥, 180°, ) retinotomy. In addition, simultaneous retinectomy was performed in 36. 2% of the cases. The BCVA was 2. 18 ±,0. 63 and 1. 85 ±,0. 71 logMAR before the surgery and at the last visit, respectively (P = 0. 001). The initial anatomical success was achieved in 45 eyes (73. 8%) after retinotomy surgery. Sixteen eyes (26. 2%) had recurrent RD and needed reoperation, which was performed 5. 60 ±,4. 01 months after the initial retinotomy surgery. At the last examination, the retina was attached in all patients. Conclusion: Retinotomy with/without retinectomy is an effective procedure in the majority of patients with RRD associated with advanced PVR,however, additional surgeries are needed in a significant number of eyes to achieve final anatomical success.

Abstract: 

Background: PROLIFERATIVE VITREORETINOPATHY (PVR) is the leading cause of recurrent retinal detachment after surgical repair of rhegmatogenous retinal detachment (RRD). Our study aimed to assess the efficacy and safety of intravitreal methotrexate infusion (IMI) for the prevention of PVR after pars plana vitrectomy (PPV) in eyes with RRD. Methods: This prospective comparative interventional study was conducted from September 2020 to November 2021 at Ain Shams University Hospitals, Egypt. We recruited a consecutive, non-randomized sample of 47 eyes of 47 patients with RRD undergoing PPV. Participants were allocated to a control group or an intervention group that received IMI during surgery. Each group was subdivided into subgroups of eyes at high-risk of developing PVR and eyes with established preoperative PVR grade C. Outcome measures at the 3-month postoperative follow-up were the rate of retinal attachment, incidence of PVR, reoperation rate to flatten the retina, and changes in the retina and/or optic nerve function as assessed by full-field electroretinogram and flash visual evoked potential. Results: Data from 47 eyes (23 and 24 eyes in the intervention and control groups, respectively) were evaluated. Subgroups IA, IB, and IIB each included 12 eyes, subgroup IIA included 11 eyes, and all subgroups had comparable sex ratios and age distributions. Postoperative PVR at 1 month and between 1 and 3 months was present in 13% and 4% of eyes in the intervention group, respectively. Reoperation to flatten the retina was required in 2 (9%) eyes in the intervention group, while 22 eyes (96%) had complete flattening of the retina at 3 months. No significant differences were found between the study groups and the corresponding subgroups regarding the outcome measures (all P > 0. 05). No adverse events attributable to IMI were detected up to 3 months postoperatively. Conclusions: Although IMI was safe for intraocular use in eyes with RRD and PVR grade C or a high risk of developing PVR, it did not affect the anatomical success rate or development of PVR up to 3 months after PPV. Further multicenter randomized clinical trials with longer follow-up periods and larger sample sizes are needed to verify these preliminary outcomes.

Abstract: 

Purpose: To evaluate the effect of intravitreal bevacizumab (IVB) as a surgical adjunct in prevention of PROLIFERATIVE VITREORETINOPATHY (PVR) after retinal detachment surgery.Methods: In this controlled, randomized pilot study, 27 patients with primary retinal detachment undergoing pars plana deep vitrectomy were included. Of these, 12 received IVB at the end of procedure. The anatomic success and best corrected visual acuity (BCVA) were compared to the control group at months 3 and 6 postoperatively.Results: At three month follow‑up, 3 of 11 eyes (27.3%) had detached retinas in the IVB group versus 6 of 12 (50.0%) in the control group (P=0.40). At six‑month follow‑up, 3 of 10 eyes (30%) had detached retinas in the IVB group versus 3 in 8 (37.5%) in the control group (P>0.99). Mean logMAR BCVA improved significantly in both groups relative to baseline, but did not show a significant difference at three‑and six‑month follow‑ups between the two groups.Conclusion: Our preliminary results show neither a benefit nor any harm from intervention in both anatomic and visual outcomes. Our results support conducting additional studies to evaluate the effect of intravitreal bevacizumab on postoperative PVR.

Abstract: 

Purpose: To report a relatively rare case of familial exudative VITREORETINOPATHY (FEVR).Patient and Findings: A 13-year-oldboy referred with decreased visual acuity of both eyes, specially right eye from 6 years ago. He had several ophthalmologist and optometrist visits and spectacle prescription without any improvement of vision. Best corrected visual acuity (BCVA) was 25 cm of count finger in right eye and 10/10in left eye. Right eye had +2 APD without positive finding on slit-lamp examination. Blurred optic disk margin with large and diffuse exudates involing macular region and avascular zone, especially on temporal side with fibrovascular tissue between avascular zone were detected on funduscopy in the right eye. Following several laboratory examination, diagnosis of FEVR was determined and patient underwent full panretinal photocoagulation.Conclusion: Early detection of FEVR plays an important role on preventing the progression of the diseases and improving the vision of patient and his/her family.
