Abstract:

Background: Maximum exercise capacity is the best predictor factor of future cardiac events, so the present study was designed with the aim of determining the effect of PTCA (Percutaneous Transluminal Coronary ANGIOPLASTY) on exercise capacity in different special subgroups, candidated for PTCA, in Shahid Modarres hospital during the academic year 2000. Materials and methods: The before and after clinical trial strategy of this study was performed on 70 patients using conventional anti-ischemic drugs. Patients had undergone symptom limited exercise test (Bruce protocol) using Marquette Electronics Treadmill Max-I model, before, one-month, and three-month after the PTCA. Exercise time, workload and rate pressure product were all measured and statistical analysis was managed using paired Hest and t-student tests. Results: 70 patients (69% male and 31 % female) with mean age of 53.9±10.1 yrs had entered this study. In a one-month follow up 70 and in a 3-month follow up 28 patients had undergone exercise test. Exercise time was increased from 7.8±2.6 minutes to 9±2 and 9.1±2.2 minutes in the one-month and 3-month follow up, respectively (p<0.01). However, workload was increased from 9.4±2.7 METS to 10.6±2.2 and 10.7±2.3 METS in the one month and 3-month follow up, respectively (p<0.01). Rate pressure product (RPP) has diminished from 24348±5354 units during the later months (N.S). Type of revascularization and PTCA on LAD had no significant effect on exercise capacity. Conclusion: PTCA is accompanied by increased exercise test capacity, where, type of revascularization and PTCA on LAD had no significant effect. A true experimental study seems helpful.

Abstract:

Background: Anxiety and depression are the most important factors on the quality of life in patients after coronary ANGIOPLASTY. These patients have less capability for adjusting themselves with changes in life style, diet, medication and physical activity adherence. This study aimed to determine predictive factors of anxiety and depression in patients undergoing coronary ANGIOPLASTY.Methods: In this descriptive cross-sectional study, 148 patients were selected by convenience sampling method. Data were collected by a 2-section questionnaire, including sociodemographic factors and the hospital anxiety and depression scale. Multivariate logistic regression was used to determine predictors of anxiety and depression. Using a convenience sampling method, the patients were selected among those patients undergoing PCI in Heshmat Heart Medical Hospital in Rasht, Iran between March 2015 and June 2015. Analyses were performed using SPSS 19. Results: The majority of samples were male (61.5%), married (93.2%), retired (31.1%), and illiterate (48%). Their mean (SD) age was 60.02(10.5) years. According to the findings, 62.2% suffered from clinical anxiety and 20.3% suffered from clinical depression. The results of multivariate adjusted model showed that education level is significantly associated with mild depression. Also sex and age are significant predictors for severe depression. Male patients were less likely to have severe depression compared to female ones. Middle age patients (45-64 years) compared to older adults were more likely to be diagnosed with severe depression. Conclusion: Considering the high prevalence of anxiety and depression symptoms in these patients, they need counseling, social and psychological support before, during and after procedures such as ANGIOPLASTY.

Abstract:

Percutaneous transluninal coronary ANGIOPLASTY (P.T.CA) is one of the efective treatment of coronary artery disease. This method was described in 1979 and since then has improved rapidly. In this study the results, and the complications of 162 cases of PTCA is described. The procedure was done between the years of 1373 to 1375 in Chamran Hospital in Isfahan. The patients were followed up untill the end of 1380. Primary successful was 80%. The most but rare complications were acute closure disection and also hematoma at inquinal area.

Abstract:

Objective- The aim of this study was to evaluate the use of endovascular intervention in managing native and recurrent aortic coarctation in adult patients. Methods-Balloon ANGIOPLASTY was undertaken transfemorally in 16 patients (mean age 18.3 ± 4 years old) with arterial hypertension in 12 native and 4 recurrent aortic coarctations. Systolic pressure gradient measurements were taken peri-interventionally and in the course of follow-up during conventional angiography and transesophageal echocardiography. The average diameters of the balloon catheters used ranged from 15 to 20 mm.Result-lmmiediately after aortic ANGIOPLASTY, the peak systolic pressure gradient decreased from 61 mmHg ± 5.4 to 7.6 mmHg ± 2.2 across the coarctation (P < 0.001). One patient With recoarctation had an unsuccessful immediate result. Restenosis occurred in two patients, who subsequently underWent repeat dilation with successful results. Transesophageal echocardiography was repeated post-procedure and at 6 months post-intervention. One patient developed a small aneurysm at the site of dilation. In the course of follow-up, no other major or minor complications were observed.Conclusion- Endovascular intervention appears to be a safe and effective alternative to surgery for the treatment of native and recurrent aortic coarctation in most adult patients with a low rate of complications

Abstract:

Background: The patients undergoing coronary artery ANGIOPLASTY should be motivated to adhere to lifestyle modifications. One of the factors affecting this issue is cardiac self-efficacy. Cardiac self-efficacy motivates individuals to select a lifestyle related to their cardiovascular diseases through creating the desire to adjust with such behaviors. Objectives: The current research aimed to determine the cardiac self-efficacy predictors after coronary artery ANGIOPLASTY.Patients and Methods: In this cross-sectional study, 193 patients undergoing coronary artery ANGIOPLASTY in Dr. Heshmat educational therapeutic center were selected through convenience sampling. The data were collected by a questionnaire consisted of two sections, including socio-demographic factors and cardiac self-efficacy after oral consent of the subjects. Then data were analyzed via descriptive (frequency distribution, mean and standard deviation) and inferential statistics (Pearson, ANOVA and independent t-test). To define cardiac self-efficacy predictors, multi-linear regression model was applied. Results: Based on the obtained results, the mean age of subjects was 58.35 ± 9.97 years, the majority were male (60.6%) and the underlying diseases history (57.5%) and the mean time interval between repeated ANGIOPLASTY was 17.48± 25.2 months. Also the mean score of cardiac self-efficacy among these patients was 8.43±4.5. Also, based on the results of the statistical tests, there was a significant relationship between age (P<0.001), gender (P=0.024), education (P<0.0001), hypertension history (P=0.006), living conditions (P=0.03), job (P=0.001) and monthly income (P=0.003) with cardiac self-efficacy. As the multi-linear regression test results suggested, age was the only cardiac self-efficacy predictor. Conclusions: The current study results implied low cardiac self-efficacy score in the study units; then it is recommendedto consider care plans to improve the cardiac self-efficacy.
