Today, with the advent of new medications, treatment of Ulcerative colitis (UC) has been markedly improved. Immunosuppressive drugs used in therapy predispose patients to opportunistic infections. A 22-year-old woman was admitted to emergency department due to acute exacerbation of UC and decreased level of consciousness. She was under treatment with 5-aminosalicylate, prednisolone and azathioprine. In neurological evaluation, patient had cerebral herniation. Non-contrast CT scan revealed multiple hemorrhagic areas in both frontal lobes. Right frontal craniotomy was performed emergently. Histopathologic evaluation of brain tissue was reported as "Herpes simplex encephalitis". Polymerase chain reaction (PCR) assay was also positive for HSV DNA. Immunosuppressive drugs such as azathioprine have a pivotal role in the treatment of resistant and/or severe cases of UC. Prevalence of infectious complications was reported to be 7.4%, 1.8% of which were severe (including herpes zoster encephalitis) .Our review of literature indicates that no case of herpes simplex encephalitis following  immunosuppressive therapy for UC has been reported.

چکیده: 

During a period of 7 years (1997-2003) we were received pairs of serum and CSF of 27 suspicious clinically cases from neurology department of several hospitals in Tehran. Among them 7 were serologically positive for progressive rubella panencephalitis (PRPE) and 2 were positive for sub acute sclerosing panencephalitis (SSPE). Rubella virus was isolated from 4 of 7 serologically positive cases of PRPE using three different cell culture systems.

Background: encephalitis is associated with high rates of mortality and morbidity; however, its pathophysiology is yet to be determined. Objectives: A prospective cohort study of patients with clinically diagnosed encephalitis was performed to investigate herpes simplex virus (HSV) or tuberculosis infection in encephalitis patients. Methods: Overall, 114 patients were enrolled according to clinical profile and were tested for the presence of HSV-1 and 2 or tuberculosis using TaqMan real-time PCR method on cerebrospinal fluid (CSF). Results: Thirty patients were clinically diagnosed with HSV infection, whose CSF samples were sent for HSV qRT-PCR test. The results showed that 10 (33. 3%) subjects were HSV-1 or 2-positive. Out of 84 patients who were tested for tuberculosis encephalitis, and tuberculosis qRT-PCR was carried out on their CSF, 3 (3. 6%) cases were positive. No significant differences were found in gender and age between HSV or tuberculosis-negative and positive groups. The main symptoms in HSV positive subjects were fever and depressed level of consciousness. No cases of tuberculosis infection were diagnosed in direct CSF smear test. Conclusions: Since infectious encephalitis is a life-threatening problem and tuberculosis and HSV are progressing in the developing countries, health authorities, physicians and researchers should address this problem more seriously.

Background: The media plays as an important role in delivering information about emergent issues, such as the Eastern equine encephalitis virus (EEEV) outbreak of 2019. As such, there has been an increase in news coverage of vector-borne disease coverage due to a rise in emerging or re-emerging arboviruses. Methods: The purpose of this study was to describe the content of news clips related to the recent (2019) EEEV outbreak in the United States. Results: Only 3 of the important topics identified were mentioned in a majority of the 110 videos analyzed. These topics were, mosquito mentioned as transmitter, prevention by repellent/ pesticide, and geography. Thus, many aspects of EEEV were lacking in coverage. Conclusion: A priority for public health professionals should be to engage in discourse with news media to assure that information disseminated via news channels is not vague or misleading.

Background: encephalitis may arise from infectious or non-infectious etiologies. Infections, predominantly viruses, are the most common causes of encephalitis. Herpes simplex virus (HSV) is the most prevalent cause of viral encephalitis in the developed countries. HSV encephalitis occurs equally in both genders and most cases are caused by HSV type 1 (HSV-1). Methods: The current descriptive, cross sectional study was conducted on patients admitted with HSV-1 encephalitis from 2011 to 2016 to Imam-Khomeini hospital complex in Tehran, Iran. The patients with clinical diagnosis of encephalitis whose cerebrospinal fluid (CSF) HSV-1 polymerase chain reaction (PCR) was positive were included. All clinical, laboratory, and imaging characteristics of patients along with their outcomes were evaluated and analyzed. Results: In the current study, of 12 cases with encephalitis manifestations and positive CSF HSV-1 PCR results, 5 were male and 7 were female with mean age of 42 years. Fever and headache were the most common complaints. Headache was more common in the cases admitted earlier (P value = 0. 001). The patients were studied by brain magnetic resonance imaging (MRI) and the findings confirmed encephalitis in all the cases. Five cases had unilateral temporal lobe lesions and 6 cases had bilateral lesions. There was a clear relationship between bilateral temporal lobe lesions and higher protein level in CSF (P value = 0. 001); 25% of patients (3 cases) needed stay in the intensive care unit (ICU). Three patients died and 5 patients developed disabilities. Conclusions: HSV encephalitis is a devastating disease. Clinical findings along CSF analysis and neuroimaging may result in the appropriate diagnosis. Despite appropriate treatment, undesired outcomes are common.
