Background: There is no standard therapy for the lesions of chronic cutaneous leishmaniasis (CCL). Treatment failures are quite common with the currently recommended therapeutic regimens. In addition the prognostic implication of the presence or absence of amastigotes in the direct smear and/or biopsy specimens of these lesions is yet to be clarified. Objective: To find an effective treatment for lesions caused by CCL. Methods: Forty two consecutive patients with various lesion-types of CCL were divided into 2 groups, namely amastigote-positive and amastigote-negative according to the results of their direct smear or biopsy specimen. All cases were subsequently treated with a paste of a herbal extract: "Z-HE" for 5 consecutive days. The treatment was repeated every 2 weeks, as needed, for a maximum period of 3 months. In addition patients were divided into 3 subgroups according to their clinical response to therapy: completely cured, partially cured, and the failure group. Treatment results were correlated with both the "specific lesion-types" and the presence or absence of amastigote. Results: There were 16 males and 26 females, with a mean age of 23 years. The duration of disease ranged from 1-34 years with a mean of 4.95 years. Overall 29 (69%) had complete cure, 5 (12%) had partial cure and 8 (19%) failed to respond to therapy. The complete cure rate, however, was 86% for amastigote-positive cases and only 25% for the amastigote-negative group (P=<0.001). This was true for all the CCL lesion-types. The mean period to complete recovery was similar in both groups. No drug-induced hematologic, hepatic or renal derangements were noted. Conclusion: "Z-HE" appears to be effective in the treatment of lesions of CCL, especially in the amastigote-positive group

Leishmaniasis represents a serious threat to the health as one of the most important neglectedtropical diseases as designated by the World Health Organization. The disease is endemic in82 countries, among them Tunisia is an indigenous area for cutaneous Leishmaniasis. In aprevious work, two tritepenic acids namely oleanolic and maslinic acids have been isolatedfrom olive leaf extract. In the present paper, the in vitro activity against amastigotes stage ofLeishmania (L. ) infantum and Leishmania (L. ) amazonensis was investigated. Maslinic acidshowed the highest activity, against L. amazonensis, with an IC50 of 1. 417 ± 0. 401 μ g/mL anda selectivity index of 9. 405. Although, the oleanolic acid exhibit a better activity against L. infantum with an IC50 of 0. 999 ± 0. 089 μ g/mL and selectivity index of 8. 111.

Recent scientific evidence indicates that distinct patterns of susceptibility in BALB/c mice to Leishmania major infection are attributable to the differential expansion of distinct CD4+ T-cell subsets and their cytokines production. Production of the Th1 cytokine IFN-g is associated with resistance, whereas production of the Th2 cytokines IL-4 and IL-10 are associated with extreme susceptibility. The major host immune defense mechanism against Leishmania is activation of macrophages by INF-? derived from T cells. The inability of susceptible hosts to mount the immune response necessary to activate macrophage and destroy the parasites may be due to the parasite-specific proteins that are able to suppress the immune system. In the present study, we have semi-purified the excreted antigens of Leishmania major promastigote and amastigote by column chromatography. The isolated fraction showed a potent immunosuppressive activity on normal BALB/c mice lymphocytes stimulated with mitogens. Fifteen microgram of the isolated fraction caused 81% suppression of lymphocyte proliferation. These data may suggest that the parasite by secreting immunosuppressive factor down regulate the immune system and as a result survive in the body.

Background: Today, most patients with cutaneous leishmaniasis are treated with drugs such as glucantime; however, severe drug resistance to these drugs is found, in addition to recurrence, extensive drug complications, and secondary infections in many patients. Objectives: Therefore, due to the lack of sufficient information on this subject, this study was undertaken for the first time to investigate the antimicrobial activity of multifunctional lactoferrin as one of the most important bioactive compounds on amastigotes and promastigotes forms of Leishmania major in vitro. Methods: In this study, six concentrations of lactoferrin (2. 5, 5, 10, 20, 40, and 80  g/mL) were added to parasite cultures at specified times. The MTT was done. Leishmania major promastigotes were incorporated to J774 cells and then incubated for 72 hours. The results were compared with the results of glucantime, as a standard. In the end, flow cytometry was performed to investigate apoptosis. Results: After 24, 48, and 72 hours of incubation in the presence of various concentrations of lactoferrin, the number of amastigotes and promastigote parasites increased over time without any significant difference compared to the control group (P > 0. 05), but it was significant compared to glucantime at the 80  g/mL concentration (P < 0. 05). Besides, lactoferrin showed very low toxicity effects on macrophages and stimulated the growth of both forms of L. major parasite. Conclusions: Our findings indicated that lactoferrin could not induce early and late apoptosis in both forms of L. major.

amastigotes are main pathogenic stage of Leishmania parasite in mammalian host. During the course of leishmania infection the amastigote surface would be covered by host proteins like immunoglobulins and complement components. These proteins produce false positive reaction and low signal/noise ratio in evaluation of antibody raising against lesion-derived amastigote by indirect ELISA. To overcome this problem we were treated lesion-derived amastigotes with several known antigen-antibody dissociating agents like glycine, potassium thiosufate, chloroquine, guanidine HCI, urea, etc. Our results indicated that these agents were ineffective in ELISA results and could not produce a clear discrimination between positive and negative controls. We were also studied the effect of pepsin digestion on amastigotes and coated these pepsin-treated amastigotes to check the ELISA results. Our results revealed combination of tests including pepsin treated amastigotes coated in alkaline pH and protein A-peroxidase conjugates as secondary antibody in an indirect ELISA design, possess high signal/noise ratio i.e. low background color.

Background: Promastigotes of Leishmania spp. have been readily cultured, but the axenic culture of amastigotes has been successful in L. donovani, L .infantum L. mexicana and L. pifanoi. However, some species such as L. major, is much less amenable to axenic cultivation. In present study, we describe an in vitro culture system for the generation and propagation of axenic amastigotes form of L. major.Methods: Promastigotes of L. major were cultivated in a biphasic NNN medium. The liquid phase was Schneider's medium, pH 3.5, supplemented by 25% FCS (fetal calf serum). The cultures were maintained at 33-34°C for 120 hours.Results: Fine structure analysis of these in vitro-grown amastigotes by electron microscopy, demonstrated that they have a pear-shaped body with abortive short terminal flagellum. The in vitro-grown cells are agglutinated by peanut lectin. SDSPAGE pattern of these axenic amastigotes showed a 66-kDa band, which was not present in promastigotes. The axenic grown amastigotes were able to infect peritoneum macrophages of BALB/c mice. In supernatant of culture, biochemical, analysis showed decreased protein and acid phosphate activity.Conclusion: These amastigotes like cells might serve as a suitable strain for the study of amastigote biochemistry, in vitro drug testing, and immunology of L. major.

It was recently demonstrated that 5- (5-nitrofuran-2-yl) -1, 3, 4-thiadiazoles with piperazinyl-linked benzamidine substituents are effective in vitro against Leishmania major.Following on this evidence, we used colorimetric assay of acid phosphatase activity in the promastigotes as an indicator for cell viability. Also we studied the effect of these compounds on induction of nitric oxide (NO) in macrophage and production of reactive oxygen species (ROS) in lymphocyte that have important role in activation of immune response against Leishmania and elimination of parasite.Results showed that these compounds decrease the viability of the parasite and increase ROS and NO production in lymphocyte and macrophage respectively.These compounds can induce parasite killing, directly by decreasing the parasite viability and indirectly by exhibiting a significant increase on immune system.
