BACKGROUND: Dairy products intolerance is defined by existing of gastrointestinal symptoms following dairy product consumption. Its prevalence varies among different countries. This study is conducted to determine the frequencies and severities of intolerance symptoms in the consumption of different dairy products in Iranian students of Medical Sciences.METHODS: In this cross-sectional study, 1041 students from Isfahan University of Medical Sciences who apparently were healthy and had not used any drug before, participated. The questionnaire included information about dairy product consumption and avoidance, severity of dairy intolerance symptoms included gas passing, flatulence, diarrhea and abdominal pain and coexistence of irritable bowel syndrome (IBS).RESULTS: The frequencies of dairy intolerance syndromes in milk, yoghurt, cheese and ice cream consumption were 51.1% (532), 16.6% (173), 11.7% (122) and 13.4% (140), respectively. Most severe symptoms for milk, yoghurt, cheese and ice cream intolerance were diarrhea (2.11±0.08), gas passing (1.56±0.09) and flatulence (1.49±0.09), respectively. Fifty (9.4%) of symptomatic respondents never drank milk. Correlation coefficient between symptoms severity and the avoidance of consumption in milk intolerance was 0.38.CONCLUSIONS: Students tolerate other dairy products better in comparison with milk. In the present study, the phenomenon as irritable bowel syndrome was not strongly associated with the severity of all symptoms in dairy intolerant persons. Also, there were weak association between the severity of symptoms with dairy consumption and avoidance, but more studies are needed to evaluate calcium absorption and osteoporosis in symptomatic cases.

Abstract:

Background: Stroke in YOUNG ADULTs causes morbidity in this socioeconomically-active age group. Etiologic frequency of ischemic stroke in YOUNG ADULTs is different around the world. This study was conducted to determine the causes of stroke in Iranian YOUNG ADULTs. Methods: The study population consisted of 314,000 YOUNG ADULT residents in the Southern Khorasan Province, East of Iran. All the patients with stroke, admitted to Vali-e-Asr Tertiary Care Hospital, entered this study. Demographic data, clinical presentation, and investigations of consecutive patients aged 15 – 45 years, presented with ischemic stroke, were registered in Southern Khorasan Stroke Database between 2000 and 2005. All the patients underwent a standard battery of diagnostic investigations by a stroke neurologist. Etiologic classification of stroke in the patients was made based on the Practical Iranian Criteria. Results: One hundred and twenty-four patients (60 females and 64 males) were prospectively investigated during a 5-year period. The incidence of ischemic stroke in YOUNG ADULTs was 8/100,000 per year. Cardioembolic mechanism constituted 54% of all stroke etiologies in YOUNG ADULTs. Rheumatic valvular heart disease was present in 32% of the patients and caused 2.5 preventable stroke cases per 100,000 YOUNG ADULTs per year. Conclusion: Rheumatic valvular heart disease is the most common cause and a preventable etiology of stroke in Iranian YOUNG ADULTs.

Abstract:

A 21-year-old man was referred to dermatology clinic for skin biopsy. He presented to neurology center with behavioral changes with cognition and memory disorders. He had two episodes of generalized tonic clonic seizure and series of myoclonc jerks in his limbs and face especially after awakening since several months ago. In neurological examination, he had mini mental score as 13; with some degree of bradykinesia, limb rigidity and dysartheria. He had leaved school because of inability to learning. His sister had died 5 months ago from the same disease. We performed biopsy from his right axillary skin. Histopathological examination revealed multiple intracytoplasmic homogenous pale basophilic globi in apocrine glands acini which were showing PAS positive reactivity in special staining. Reminder of epidermis and mid reticular dermis was unremarkable.

Abstract:

Background & Aim: Drug addiction, especially addiction to Ecstasy has ruinous effect on lives. Lack of knowledge is the most important cause of Ecstasy use among YOUNG ADULTs. Research studies showed that students use these pills in order to get comfort and to overcome grieves. Addiction to ecstasy has been increased in Iran in recent years. This study was carried out to assess the factors associated to Ecstasy use in students of Qazvin University of Medical Sciences. Methods & Materials: In this cross-sectional study, we recruited 200 students using clustered, random sampling. Data collection tool was a structured valid and reliable questionnaire containing 28 questions in two sections. Data were analyzed using Student t-test, ANOVA and chi-square tests. Results: Results indicated that the prevalence of Ecstasy use was 1.5%. Prevalence of alcohol and cigarette use was 1.5% too. Mean score of knowledge on Ecstasy was 9.8% (SD=3.8). Knowledge of the female students was higher than the knowledge of male ones. About 86.7% of the students took Ecstasy in order to end grieve due to family problems. Most important associated factors to Ecstasy use were: positive attitude toward Ecstasy use, lack of knowledge (96.5%), peer pressure (96%), and lck of parents' attention to their YOUNG children (94.5%).Conclusion: Lack of knowledge about complications of addiction to Ecstasy is the most important factor leading to addiction among students. Living in places such as dormitories and rented houses, and lack of parents' control over their children can be important factors related to addiction of students to Ecstasy, as well.

Abstract:

Diffuse normolipemic plane xanthoma is a rare disorder often seen as a para- neoplastic manifestation accompanied with gammopathics and hematologic disorders. In this report, a 44-year-old man IS presented who was referred to us due to developing erythematous papules on the hands and yellow patches on different parts of the body. Based on cell blood count, peripheral blood smear and positive anti HTL VI, the diagnosis of ADULT T cell lymphoma/leukemia was suggested which confirmed by the biopsy of the hand lesions. The histopathologic evaluation of yellow lesions also confirmed the diagnosis of diffuse xanthoma.
