Abstract:

Introduction: The use of lasers for treatment has become a common phenomenon in the medical field. Currently, numerous laser systems are available for dental use. The use of lasers for periodontal treatment becomes more complicated because the periodontium consists of both hard and soft tissues. Methods: Related articles were gathered and selected carefully and reviewed. Among the many lasers available, high power lasers such as Carbon Dioxide Laser (CO2), Neodymium-Doped Yttrium Aluminium Garnet (Nd: YAG) and diode lasers can be used in PERIODONTICS. The use of these lasers is limited to gingivectomy, frenectomy and similar soft tissue procedures including the removal of melanin pigmentation of gingiva. Recently, Erbium: Yttrium Aluminium Garnet (Er: YAG) and Erbium, Chromium doped Yttrium Scandium Gallium Garnet (Er, Cr: YSGG) lasers are used for scaling, root debridement, cutting, shaving, contouring and resection of oral osseous tissues. Results: In addition to their surgical applications, low-level lasers such as Er: YAG laser irradiation promotes osteoblast proliferation showing higher and favorable bone tissue regeneration. These findings suggest faster bone tissue healing following periodontal and peri-implant low level laser therapy. Conclusion: Advantages of laser treatment in PERIODONTICS are effective and efficient soft and hard tissue ablation with a greater hemostasis, bactericidal effect, minimal wound contraction, faster bone tissue healing, minimal collateral damages along with reduced use of local analgesia.

Abstract:

Removal and elimination of the bacterial biofilm from the root surface is an essential part of periodontal treatment. Nd: YAG-lasers, diode-lasers, and Erbium-lasers show a high antibacterial effect in the periodontal pocket. Additionally the Er: YAG and the Er, Cr: YSGG lasers can be used for the debridement of the root surface. The presentation gives an overview of the recent literature and how lasers could be a part of modern periodontal treatment.

Abstract:

Removal and elimination of bacterial biofilm from the root surface is an essential part of periodontal treatment. Nd: YAG lasers, diode-lasers, and Erbium-lasers show a high antibacterial effect in the periodontal pocket. Additionally, these lasers can be used as minimally invasive surgical tools to remove the pocket epithelium. Debridement of the root surface from hard calculus deposits is only possible with the Er: YAG and the Er, Cr: YSGG lasers. In periodontal regenerative procedures, the laser light of the Erbium-lasers might be used for root surface conditioning. Recent reviews concluded that the diode lasers and Nd: YAG lasers provide a benefit as an adjunct to conventional periodontal treatment. The presentation gives an overview of the literature, the techniques and how lasers could be a part of modern periodontal treatment.

Abstract:

Laser has emerged as an adjunct in several treatment modalities in dentistry in the past few decades. This less invasive bladeless technique is bringing revolutionary outcomes in a plethora of periodontal treatment procedures as well. A unique ameliorative approach termed LANAP, described as laser-assisted new attachment procedure was developed by Gregg and McCarthy. In 1990 they introduced an innovative treatment for diseases of gums incorporating pulsed neodymium yttrium aluminum garnet (Nd: YAG) 1064 nm wavelength laser (PerioLase MVP-7). The LANAP concept was endorsed by Yukna et al who conducted a study according to the protocol reinforced at the1996 world workshop in PERIODONTICS, which established specific histologic criteria to prove regeneration. Yukna’s histological study found that regeneration of the periodontally compromised root could be achieved by Nd: YAG laser. LANAP facilitates refurbishing of new tissues from supporting structures of the periodontium wherein the unhealthy surface of the roots exhibit pristine attachments in human beings. This paper is a review providing a detailed report of LANAP from its inception to recent advances.

Abstract:

Background and Aim: Lasers can serve as a new powerful tool in dentistry. High intensity laser therapy is becoming the treatment of choice compared with conventional periodontal therapy due to accurate incision and soft and hard tissue ablation, hemostasis, reduction of postoperative pain and infection, optimal debridement, and enhanced healing. Although high intensity laser therapy assumes to provide better results compared with conventional treatments, a significant variation exists in the applied laser parameters such as different wavelengths and energy densities of laser. The objective of this study was to collect preliminary information about the type and parameters of surgical lasers and how to use them technically based on the available literature on this topic. Materials and Methods: After initial screening of 152 potentially relevant articles identified through an electronic search, 44 articles were selected based on the eligibility criteria by three independent reviewers. The inclusion criteria included studies on the outcomes of periodontal high intensity laser therapy in humans published in English between 2013 and 2021 in journals indexed in PubMed Central, Science Direct, Wiley Online Library, Springer or Google Scholar. Results: The results showed that diode, Nd: YAG, Er: YAG, Er, Cr: YSGG and CO2 lasers had the highest efficacy for frenectomy, gingivectomy, and osteotomy with specific wavelength, power density, frequency, and pulse mode. Various lasers have been suggested for the abovementioned purposes, and the method of choice depends on the efficacy and availability of laser. Conclusion: This study confirmed higher efficacy of different laser types compared with conventional treatments.

Abstract:

Based on Albert Einstein’s theory of spontaneous and stimulated emission of radiation, Maiman developed the first laser protype in 1960. Since then, lasers have been widely used in medicine and surgery. Lasers designed for surgery deliver concentrated and controllable energy to tissue. Typically, lasers are named according to the active element(s) that is induced to undergo the stimulated quantum transitions which, in turn, create the energy beam. Thus, lasers commonly used in dentistry consist of a variety of wavelengths delivered as either a continuous, pulsed (gated), or running pulse waverform. Wavelength affects both the clinical application and design of the laser. The wavelength of lasers used in medicine and dentistry generally range form 193 n.m to 10600 n.m, representing a broad spectrum. The lasers most commonly used in dentistry are Co2, Nd: YAG, HO: YAG, Er: YAG, Er, Cr: YSGG, Nd: YAP, GaAs (diode), and argon. Clinical application of lasers includes: intra-oral soft tissue procedures, such as frenectomy, gingivectomy and gingivoplasty, removal of granulation tissue, second stage exposure of dental implants, lesion ablation, incisional and excisional biopsies of both benign and malignant lesions, irradiation of aphtuous ulcers, coagulation of free gingival graft donor sites, and gingival depigmentation, and also hard tissue procedures such as scaling of root surfaces, crown lengthening, osteoplasty and ostectomy. The purpose of this article is to provide information about the current and potentinal application of laser technology to periodontal practice.
