Abstract:

Background and Aim: Meningioma is one of the most common brain tumors and its prevalence wasl-4% in children. This study was done to survey the tumor cases that were done by surgery. Materials and Methods: In this retrospective- descriptive survey the the patients under 17 years old and were operated for Meningioma in Ghaem and Mehr hospitals of Mashhad in 1995-2003 were studied. Information was collected by examining case notes and pathology records; complete medical records are available for 9 cases and long term follow up was achieved for 8 patients. All the cases were diagnosed and treated after CT scan and MRI scanning. Results: The samples 9 patients consisted of 5 boys and 4 girls, the age rate at the time of diagnosis varied between 3 months and 17 years. 6 cases were between 7-17 years old, 1 case was 3 years old and one 20 months and 1 case was 3 months. The most common presenting symptoms in declining order of frequency were raised intracranial pressure (ICP) focal neurological deficits, skull deformity and epilepsy. There have been no surgical death and gross total excision was achieved in 7 cases. Tumor recurrence was observed during the follow up period in 1 case, the follow up period varied between 1 and 9 years. Conclusion: Based on this research data Meningioma in children and young adults have a good prognosis if the tumor can be removed radically however relapse can occur many years after the primary operation.

Abstract:

Purpose: To report a rare case of isolated intraconal Meningioma. Case Report: A 24-year-old woman presented with painless proptosis in her left eye which started and progressed during her pregnancy about 10 months ago. Hertel exophthalomometry revealed anterior displacement of the globe with 4 mm of proptosis which was remarkable. Magnetic resonance imaging (MRI) demonstrated an intraconal circumscribed oval-shaped mass with hypointense signals on T1-weighted images and hyperintense signals on T2-weighted images, mimicking cavernous hemangioma. This mass, however, was free of any connections to optic nerve or bones. Due to the imaging characteristics, more prevalent diagnoses like cavernous hemangioma were placed on the top of the differential diagnoses list. However, during the surgical excision, the tumor’ s consistency and gross features were not compatible with cavernous hemangioma. The pathologic findings instead determined meningotheliomatous Meningioma, a very rare condition, which was far from our expectations prior to the surgery. Conclusion: Ectopic orbital Meningiomas are rare tumors that are not easily diagnosed without postoperative histopathology. Despite its low prevalence, they should be considered in the differential diagnosis list of intraconal masses with hypointense signals on T1-weighted images and hyperintense signals on T2-weighted images.

Abstract:

Although Meningiomas are common tumors of the central nervous system, primary extracranial Meningiomas are extremely rare. These tumors constitute approximately1% of all Meningiomas. This study has reported a very rare case of Meningioma on the left carotid sheath that presented as a left neck mass. We performed a core biopsy of the mass. Subsequently, the tumor was excised and histopathologic examination confirmed the diagnosis of primary extra cranial Meningioma.

Abstract:

Introduction: Extracranial Meningioma is a rare tumor which is located in middle ear, temporal bone, sinonasal cavity, oral cavity, parotid gland and soft tissue of head and neck.Case Report: The case of the present study was a 67-year-old male patient who referred with pain complaint during flexion of neck from 5 years ago and palpable mass in posterior neck triangle from 2 years ago. Conclusion: Fine needle aspiration was performed and epithelial lesion was reported. Finally excision of the mass was performed and the diagnosis of Meningioma was made. This diagnosis was confirmed by immunohistochemical staining which were vimentin, EMA positive and GFAP negative.

Abstract:

Histologically confirmed Meningiomas in first year of life are quite rare. However, review of the sporadic cases in the literature reported Meningioma in infants. Those have unique features in common.Case presentation An 11 month-old Iranian male infant presented with a several weeks history of decrease in movement of his right sided upper and lower extremities. Brain MRI revealed a large well-defined extra axial, Dural-based mass lesion, which is contained solid and cystic components. It is originated from falx, and showed intense contrast enhancement. The patient underwent a central craniotomy. The mass was removed sub totally. Microscopic examination of the specimen revealed a Meningioma. Radiotherapy was deferred because of the young age. Patient follow up at 4 months later showed that the child was well. Conclusion Pediatric Meningiomas show a higher frequency of cyst formation in compared with adult Meningiomas. In addition, they show more frequent sarcomatous changes, which is male predominance. Absence of dural based attachment and higher incidence of intraventricular location were seen.

Abstract:

Background & Aim: The purpose of this presentation to present our data in evaluating whether venous indocyanine green (ICG) videoangiography has any potential for predicting the presence of a safe collateral circulation for veins that are at risk for intentional or unintentional damage during surgery. Methods & Materials/Patients: We performed venous ICG videoangiography during 153 consecutive neurosurgical procedures. On those occasions in which a venous sacrifice occurred during surgery, whether that sacrifice was preplanned (intended) or unintended, venous ICG videoangiography was repeated so as to allow us to study the effect of venous Sacrifice. A specific test (clipping test) to predict the presence of venous collateral circulation was also applied in 21 of these cases. Results: Venous ICG videoangiography allowed for an intraoperative real-time flow assessment of the exposed veins with excellent image quality and resolution in all cases. The veins observed in this study were found to be extremely different with respect to flow dynamics and could be divided in 3 groups: 1) arterialized veins; 2) fast-draining veins with uniform filling and clear flow direction; and 3) slow-draining veins with nonuniform filling. Temporary clipping was found to be a simple and reversible way to test for the presence of potential anastomotic circulation. Conclusion: Venous ICG videoangiography is able to reveal substantial variability in the venous flow dynamics. "Slow veins, " when they are tributaries of bridging veins, might hide a potential for anastomotic circulation that deserve further investigation.
