Abstract:

Background: Developing standards for Health Promotion HOSPITALS (HPH) plays a crucial role in hospital accreditation. This study aimed to compare the development of HPH standards in public and private HOSPITALS in Tabriz, Iran in 2021. Methods: This descriptive-analytical and cross-sectional study was conducted to evaluate all public and private HOSPITALS in the city. Data were collected using observation and interviews through a valid and reliable HPH checklist. Data were analyzed using t test. Results: The study found that the standards for patients’ data and interventions (77. 38%), continuity in treatment and cooperation (69. 38%), management policy (61. 92%), patient evaluation (60. 76%), and providing a healthy workplace (42. 7%) had the highest scores, respectively. Private HOSPITALS scored higher than public HOSPITALS in all dimensions. There was a significant difference between public and private HOSPITALS regarding the total score (P=0. 001), management policy(P=0. 01), patient information and interventions(P=0. 003), and continuity in treatment and cooperation (P=0. 002). Conclusion: The competitive atmosphere among private HOSPITALS appears to have improved their status in HPP, while public HOSPITALS were less concerned with competition and improving their performance in this field. Additionally, ignoring human resource management policies has made the managers of all HOSPITALS feel the need for further upgrading in terms of standards for promoting a healthy work setting.

AIMS. Military HOSPITALS should be evaluated and accredited based on their specific criteria and characteristics, as they are specific and have specific objectives and structures. The present study was conducted to identify the indicators and standards of accreditation programs of military health care centers. MATERIALS & METHODS. In this narrative review study, all Persian and English studies conducted from 2000 to 2019 in PubMed, Scopus, and Google Scholar databases using a variety of keywords accreditation, quality assessment, hospital, health care center, military medicine, military and the armed forces were available, examined. In addition to manual search, websites of some HOSPITALS and military centers were also searched. After initial retrieval of 354 papers and documents, the three researchers independently evaluated their quality according to the study objectives, and unrelated studies were excluded from the final review and the necessary information was extracted from the remaining 16 sources. Data collection was done using a data extraction form designed based on the study purposes. FINDINGS. JCAHO was the most important organization that accredited the military health care services in the world. The main criteria, which are considered by hospital and other military health care centers, were in two categories of specific criteria and general criteria in 20 rows including structural, processes, outcomes standards, and treatment, services, management and leadership and quality improvement and security indicators. CONCLUSION. With regard to the results of this study, paying more attention to structural, processes and outcome standards related to mission and structure of military health care providing centers and HOSPITALS is suggested. Also these centers should be accredited based on modified standards as well as by other common accreditation organization.

Abstract:

Introduction and purpose: The quality of healthcare services holds a significant position in the healthcare system. Understanding patients' expectations and perceptions is essential for improving the quality of health services. The present study aimed to assess the quality of healthcare services in public (governmental) teaching HOSPITALS of Ferdows using the HEALTHQUAL model in 2019. Methods: This descriptive-analytical (cross-sectional) study was conducted in two public (governmental) teaching HOSPITALS in Ferdows. The study population consisted of 350 hospitalized patients who were selected via convenience sampling in 2019. The patients’ perceptions and expectations regarding the quality of hospital services were evaluated using the HEALTHQUAL questionnaire, which assesses four dimensions: environment, human interactions, effectiveness, and efficiency. Data were analyzed in SPSS software (version 24) using independent t-tests, one-way ANOVA, and mean comparison tests. Results: The study reported the overall mean quality score of hospital services as moderate to high. A significant difference was observed between the mean perception score (3.83±0.49) and the mean expectation score (4.54±0.56) across all dimensions of healthcare service quality (P<0.001). Conclusion: As evidenced by the obtained results, patients' mean scores of perception of service quality were notably lower than their mean expectation scores, indicating a significant gap between the two domains. These results highlight considerable opportunities for improving the quality of healthcare services. Policymakers and hospital administrators should identify and address this gap to implement effective measures for enhancing healthcare service quality.

Abstract:

BACKGROUND: The public hospital preparedness is essential for epidemic disaster like COVID-19 pandemic. This study was conducted to present a hospital management preparedness model of Iran's public HOSPITALS for the epidemic. MATERIALS AND METHODS: The method of this study was an exploratory sequential mix method study (qualitative–quantitative). In this study, qualitative and quantitative methods were used in four stages. (1) Interviews with hospital managers to identify hospital management experiences during the COVID-19 epidemic and categorize the results in themes and subthemes, (2) assessing the performance of public HOSPITALS in managing the coronavirus epidemic in a quantitative method, (3) present the initial hospital management model for a public hospital in epidemic conditions using an expert panel, and (4) validation of the model using the Delphi method. RESULTS: Experiences of hospital managers and specialists were categorized into eight themes: information gathering and environmental analysis, general and operational planning, provision of equipment and physical and financial resources, training and empowerment of human resources, a compilation of instructions and job descriptions, review and ensuring maximum readiness, monitoring and follow-up of service provision and existing problems, evaluation and feedback of performance problems and level of preparation and 51 sub-themes. The quantitative study indicated that all the investigated indicators had a significant decrease in the first month and an increase in the epidemic's continuation. The results were categorized in nine themes and 59 sub-themes, and finally, the model was validated in one round by the Delphi method. CONCLUSION: In Iran, managers have valuable experiences in COVID-19 epidemy management, but these experiences are scattered and not organized. In this study, by a qualitative–quantitative, a model was presented that contains essential points obtained from the experience of hospital managers and experts in actual disaster conditions and is appropriate and fits hospital structure and infrastructure of the health system in Iran.

Abstract:

Background: Hospital management has been faced with several challenges in the recent decades. Identification and elimination of these challenges lead to improvement of effectiveness and efficiency of hospital activities. This study aimed to determine hospital management challenges.Materials and Methods: This qualitative study using content analysis approach was conducted in Shiraz University of Medical Sciences in 2014-2015. The study participants included educational HOSPITALS’ managers, senior managers, faculty members, and post-graduate students of the University. In this regard, 24 subjects were selected by purposeful sampling. The data were collected using interviews and focus group discussions and were analyzed through content analysis. Results: The hospital management challenges were categorized into five themes, namely structure of the University, rules and guidelines, resources and facilities, weakness of educational courses, and macro polices. On the other hand, changing the culture and beliefs, restructuring, education, and empowerment were mentioned as strategies for eliminating the challenges. Conclusion: This study showed that the hospital managers were faced with structural challenges. Yet, these challenges could be decreased by increasing the relationship between HOSPITALS and other centers, senior managers supporting hospital managers, empowerment of managerial culture, and clarification of hospital indices.
