Abstract: 

Introduction: This study was performed to investigate the effect of vitrification procedure using ethyleneglycol as cryoprotectant on the follicular morphology of neonatal ovarian tissue of mouse after thawing andautotransplantation.   Material and Methods: Ovaries from 3 weeks neonate NMRI mice were excised then vitrified in asolution of DMEM medium containing ficol 70, sucrose, acetanied and ethylene glycol (EGFS 40%) andstored in liquid nitrogen. After thawing with 1 M sucrose solution and equilibration with DMEM medium, some vitrified-warmed andfresh ovarian tissues were studied morphologically and the others were AUTOGRAFTed intraperitoneally. Fourweeks after transplantation, animals were sacrificed and the fresh and grafted samples were fixed and studiedunder light microscopy.   Results: Our results showed that there was no statistically significant difference between the normalfollicles in vitrified, toxicity tested and fresh control ovaries. After transplantation statistically significant differences was observed between normal follicles in intactcontrol, transplanted-control and vitrified-transplanted groups (P<0.014) and these rates were 98.18% ,89.33% and 91.54% respectively. In the transplanted group the number of follicles had a relative decrease andthe ovarian tissue was invaded by fibrous and fat tissue. However follicles in different developmental stageswere located in the periphery of ovarian tissue.   Conclusion: Our observations showed that this procedure is useful and efficient for preservation ofneonatal mouse ovarian tissues and can be used for preserving the reproductive potential of infertile patientsin the future. However it is necessary to improve maturation conditions such as changing the site oftransplantation.

Abstract: 

Early wound excision and grafting of burn wounds is regarded not only as the method of choice in speeding up the healing process in deep burns, but also limits contracture and infection. On the other hand, smoking has generally been implicated in the delay of the healing process in a variety of surgical procedures. In this study we report the outcome of a cross-sectional study of all the 395 medical records of adult patients who underwent split-thickness autographing during a six month period (Sep. 1999 to March 2000) in Taleghani Burn Center, Ahwaz. The subjects, both male and female, who underwent auto grafting for deep burns, were allocated into smoker and nonsmoker groups. The results showed that rejection of the auto grafts was significantly more widespread and more common among cigarette smokers. The extent of rejection was 6.7±0.5% of the graft area as compared to 2.9+0.3% in the control group (p<0.01). The incidence of rejection and successful take was 66.6 and 28.7% respectively among the cigarette smoking group (O.R. 4.95). The reason for the increase in rejection may be due to the toxic constituents in cigarette smoke. We recommend that smokers be encouraged to abstain from smoking prior to and post-burn grafting surgical procedures, which may be a useful preventive measure. Further research in this field is undergoing in order to assess the effectiveness of this recommendation in reducing the incidence of graft rejection.

Abstract: 

Background: Microtia is a congenital abnormality with low incidence but considerable morbidities. Reconstruction of the microtia deformity is a complex and difficult process that requires a proper planning. The primary technique of reconstruction employs patient’s own rib cartilage. Irradiated homograft cartilages previously have been used in facial reconstruction but its application in microtia surgery has not been reported yet. This study is designed to compare the results of AUTOGRAFT versus homograft auriculoplasty.Methods: Between 1992 – 2002, 23 patients underwent auricular reconstructive surgery by the senior author in our department. AUTOGRAFT implantation was performed in one stage but homograft auriculoplasty was done in two stages.Results: Auricular deformity was right-sided in 13, left-sided in 8, and bilateral in 2 cases. Implanted graft was AUTOGRAFT in 9 patients and homograft in 14 patients. During mean follow-up of 4 years, cartilage graft resorption was detected in two cases, one in AUTOGRAFT and one in homograft group (P>0.05). No postoperative infection was observed. Status of postauricular sulcus was optimal in 85.7% of homograft and 77.8% of AUTOGRAFT groups (P>0.05). The satisfaction score of the patients and/or parents was excellent in 66.7% of AUTOGRAFT and 92.9% of homograft groups (P<0.01).Conclusion: Based on better satisfaction score, equivalent aesthetic appearance, and absence of complications such as scaring and pain on the chest wall, homograft auriculoplasty is an appropriate option for reconstructive surgery in patients with microtia.

Abstract: 

Purpose: To report the result of limbal AUTOGRAFTing in primary and recurrent pterygium.Methods: In this case series, all patients who were referred to our clinic at Farabi hospital, Isfahan, Iran for treatment of pterygium were included. Indications of surgery included chronic ocular irritation, impaired or threatened vision, ocular motility problem and cosmetic reasons. Under subconjunctional anesthesia, pterygium was removed from the cornea and conjunctiva and a crescent shape limbal tissue from supratemporal region of the same or the sound eye was then prepared and sutured in the area of pterygium removal. In cases with conjunctival problem due to previous surgery or severity of the disease, some amount of conjunctival tissue was also included in the transplanted limbal tissue. Postoperatively, topical betamethasone for 4 weeks and antibiotic for 2 weeks were prescribed.Results: Two-hundred and three eyes were studied of which, 125 eyes (61.6%) had primary and 78 eyes (38.4%) had recurrent pterygium. After at least one year of follow-up, only 9 eyes (4.5%) had recurrence larger than 1 mm. Visual acuity and refraction improved in 60% of the eyes. No significant complication occurred except appearance of a fibrous tissue in the donor site which was not progressive and was covered by the upper lid in all cases.Conclusions: Surgical removal together with limbal AUTOGRAFT is a safe and effective method for treatment of primary and recurrent pterygium.

Abstract: 

Background: Peroneus longus tendon can be a suitable alternative AUTOGRAFT for anterior cruciate ligament reconstruction (ACLR). The present study aimed to compare the clinical outcome and donor site morbidity in ACLR using peroneus longus tendon AUTOGRAFT versus hamstring tendon AUTOGRAFT. Methods: In a comparative cross-sectional study, ACLR was performed with quadrupled hamstring tendon AUTOGRAFT in 65 patients between 2017 and 2018, and in another group, peroneus longus tendon AUTOGRAFT was used for ACLR in 65 patients between 2018 and 2019. The same surgical technique, fixation method, and postoperative protocol were used in both groups. The knee functional outcomes were assessed according to the Lysholm score and the International Knee Documentation Committee scale at preoperative and at the end of at least 2 years after the procedure. Moreover, graft diameter was measured intraoperatively. Thigh circumference, the American Orthopedic Foot and Ankle Score (AOFAS), The Foot & Ankle Disability Index (FADI), and ankle range of motion (ROM) were used to evaluate ankle donor site morbidities. Results: A total of 130 patients (65 patients in each group) were evaluated with a minimum follow-up of 2 years (range 24–, 31 months). Both groups showed no significant differences in clinical outcomes and knee stability. The peroneus longus graft diameter was significantly larger than the hamstring diameter (P<0. 001). Assessment of AOFAS, FADI, and ankle ROM showed no apparent ankle joint dysfunction in the peroneus longus tendon group. Conclusion: Faster knee extensions, less anteromedial knee pain, and thigh hypotrophy were observed in peroneus longus tendon patients. Peroneus longus tendon AUTOGRAFT can be an appropriate AUTOGRAFT for ACLR due to its strength, larger graft diameter, and avoiding potential complications of hamstring AUTOGRAFT obtained from the knee region.
