EVALUATION OF ANTIBIOTIC RESISTANCE AND ESBL PRODUCING ENTEROBACTERISOLATES FROM IMAM REZA HOSPITAL IN KERMANSHAH

Abstract

 BACKGROUND AND AIM:THE APPEARANCE OF ORGANISMS THOSE ARE ABLE TO PRODUCING ESBLS IS CRITICAL IN THE TREATMENT OF BACTERIAL INFECTIONS. ENTEROBACTER IS A GRAM NEGATIVE BACILLI THAT CAN PRODUCE THIS TYPE OF LACTAMASES. BECAUSE OF NONE DETECTING OF THESE TYPES OF THE BACTERIA IS NOTROUTINE IN LABORATORIES DIAGNOSIS OF THE FREQUENCY OF ESBL PRODUCING BACTERIA, IT APPEARS TO BENECESSARY. METHODS: OUT OF 150 SAMPLES WERE COLLECTED FROM PATIENT IN IMAM REZA HOSPITAL, IN KERMANSHAH, IRAN. ALL ISOLATES WERE CULTURED ON BLOOD AGAR AND MACCONKY AGAR AND COLONIES WEREDIAGNOSED BY BIOCHEMICAL AND MICROBIOLOGICAL METHODS, AND THEIR SENSITIVITY TO ANTIBIOTICS USING AGARDISK DIFFUSION METHOD AND ESBLS PRODUCTION WAS DONE USING COMBINATION  DISK DIFFUSION METHOD. THERESULTS WERE INTERPRETED ACCORDING TO CLSI GUIDELINES. RESULTS:OUT OF 150 SAMPLES,35 ISOLATEWERE DIAGNOSED AS ENTEROBACTER. ALL ISOLATES (100%) WERE RESISTANT TO GENTAMICIN, IMIPENEM, CEFOTAXIME, CEFTAZIDIME, AND 24 ISOLATES (71.25%) WERE RESISTANT TO CIPROFLOXACIN, AND 6 ISOLATES(17.5%) TO NALIDIXIC ACID, AND 3 ISOLATES (85.2%) TO CEFTRIAXONE AND 3 ISOLATES (85.2%) TOCHLORAMPHENICOL AND 3 ISOLATES (85.2%) TO MEROPENEM SHOWED MODERATE SENSITIVITY. OF 35 ISOLATED, 21 ISOLATES (60%) WERE ESBL PRODUCING GENE. CONCLUSION:OUR DATA SHOWED THAT THE RATE OFRESISTANT TO DIFFERENT KINDS OF ANTIBIOTICS WAS RELATIVELY HIGH AND ALSO THE RATE OF ESBLPRODUCING BACTERIA WAS VERY HIGH. FOR PREVENTING OF TREATMENT FAILURE, TESTS FOR THE DETECTION OFTHESE ENZYMES IS NECESSARY
