ALZHEIMER’S DISEASE THERAPEUTIC APPROACHES; CURRENT ADVANCES AND FUTURE DIRECTIONS

Abstract

 INTRODUCTION: OVER THE LAST DECADES, ALZHEIMER’S DISEASE (AD) RESEARCH HAVE WITNESSED A PROFOUND ADVANCES. INTERDISCIPLINARY INVESTIGATIVE APPROACHES HAVE REVEALED THE MULTIFACTORIALITY OF AD PATHOLOGY AND DRUG DEVELOPMENT EFFORTS HAS LED TO THOUSANDS OF CLINICAL TRIALS IN AD TREATMENT. USING ANTI-INFLAMMATORY AGENTS, ANTIOXIDANTS, NEUROTROPHIC FACTORS; TARGETING NEUROTRANSMITTERS, BETA-AMYLOID PEPTIDE, TAU PROTEIN, APO LIPOPROTEIN E4, METAL HOMEOSTASIS AND MITOCHONDRIAL FUNCTION; APPLYING REGENERATIVE STRATEGIES AND FINALLY EMPLOYING ELECTRICAL OR OPTICAL-STIMULATORY APPROACHES ARE SUMMARIZED SUBJECTS OF CURRENT AD TREATMENT RESEARCHES. AD PATIENTS STILL SUFFER FROM THE LACK OF EFFECTIVE TREATMENTS AND EFFORTS ARE MADE TO TRANSLATE EXPERIMENTAL DATA INTO NOVEL THERAPEUTIC APPROACHES TO PREVENT, HALT, OR REVERSE THE DISEASE PROGRESS. OBJECTIVES OF THIS STUDY ARE ESTABLISHING AND EMPLOYING COST AND TIME-EFFECTIVE ANIMAL MODELS OF AD, INTRODUCING NOVEL THERAPEUTIC TARGETS AND ALSO ETHNO PHARMACOLOGICAL STUDY OF ANTI-DEMENTIA MEDICINAL PLANTS DESCRIBED IN IRANIAN TRADITIONAL MEDICINE (ITM) WERE OUR MAIN OBJECTIVES.RESULTS: LENTIVIRAL-MEDIATED DELIVERY OF APPSW/IND IN ADULT ANIMALS AND COLD WATER-INDUCED CHRONIC HYPOTHERMIA COULD SUCCESSFULLY ESTABLISH TWO AD RAT MODELS. SPHINGOSINE-1- PHOSPHATE RECEPTOR-1 SIGNALING PATHWAY WAS INTRODUCED AS A POTENTIAL THERAPEUTIC TARGET IN AD. THE EFFECTIVENESS OF MEDICAL “HOT” HERBS AGAINST AD, RESEMBLING DEMENTIA WITH COLD IN TEMPERAMENT OF BRAIN WAS ALSO PROVED.CONCLUSIONS: CONSIDERING THAT NONE OF THE CURRENT AD ANIMAL MODELS RECAPITULATES ALL ASPECTS OF DISEASE MECHANISM, ESTABLISHING NEW RELEVANT MODELS IS OF GREAT IMPORTANCE IN PRECLINICAL TESTING OF MECHANISM-BASED THERAPIES. FURTHERMORE THOUGH MODERN RESEARCH FOCUSES ON SPECIFIED MOLECULES AND SIGNALING PATHWAYS AS THERAPEUTIC TARGETS, THE ETHNO MEDICAL DATA AND PHYTOCHEMICAL/PHARMACOLOGICAL STUDIES SHOULD NOT BE NEGLECTED IN THE DEVELOPMENT OF NEW THERAPIES.
