Abstract: 

چشم قرمز یکی از یافته های شایع در چشم پزشکی است. هرچند ضروری است تا بیشتر بیماران مبتلا به موارد شدید و جدی چشم قرمز توسط چشم پزشک ویزیت شوند؛ ولی بسیاری از موارد خفیف بیماری در مراحل ابتدایی توسط پزشکان عمومی ویزیت و درمان می شوند. اگر این گروه از همکاران قادر به تشخیص و افتراق علل چشم قرمز باشند؛ درمان بیماران سریع تر و با عوارض کمتری همراه خواهد بود. از جمله علل شایع چشم قرمز می توان به مواردی نظیر بلفاریت، خراش قرنیه، خونریزی زیر ملتحمه، گلوکوم، اسکلریت و اپی اسکلریت اشاره نمود.

Abstract: 

Low back pain (LBP) is a common and complicated disorder which is influenced by a number of factors, among them is the lack of spinal stability provided by muscle contraction. One of the most important muscles which has a role in spinal stability is musculus transversus abdominis. A literature search for the period of 2000-11 was performed in PubMed, ProQuest, Science Direct, Thomson, EMBASE, OVID, CINAHL and MEDLINE databases using musculus Transversus Abdominis, ultrasonography, chronic nonspecific low back pain as keywords. Nineteen articles were selected according to the inclusion criteria of the study. Evidences demonstrated that thickness of transversus abdominis reduces in patients with chronic nonspecific low back pain (LBP) and ultrasonography seemed to be a valid and highly reliable instrument for measuring thickness of transversus abdominis in patients with LBP and healthy subjects on different positions and states. Studies demonstrated that there are adequate evidences to confirm the merit of ultrasonography in the assessment of musculus transversus abdominis in patients with chronic LBP and healthy subjects.

Abstract: 

Background and Objective: Disability of upper extrimity from stroke are often permanent. Despite numerous functional problems, there is less attention to upper exterimity disabilitis than lower limbs. Some new methods of treatment focuses on using the magnetic stimulation as a means brain currents to produce therapeutic effects. This study was done to evalute the effect of low frequency repeatitive transcranial magnetic stimulation to improve motor function and grip force of upper limb in hemiplegic patients.Materials and Methods: This clinical trial study was done on 12 stroke hemiplegic patients in Firoozgar hospital in Tehran, Iran during 2009-10. Patients in group I, recieved rehabilitation program with placebo magnetic stimulation, and patients in group II, received magnetic stimulation with routine rehabilitation program for 10 session, 3 times in week. Pre and post were evaluated by Barthel and Fugl-Meyer indeces and dynamometer. Data were analyzed using SPSS-15, Kolmogorov-Smirnov, paired t-test, independent t-test and Wilcoxon signed tests.Results: According to Barthel and Fugl-Meyer indeces both groups I, II showed significant improvement (P<0.05). Using dynamometer, it was demenstrated that grip force of upper limb in group I was not significant but this index in group II was significant after intervention (P<0.05).Conclusion: This study showed that low frequency repeatitive truscrianial magnetic stimulation has therapuetic effect on grip force of upper limb.

Abstract: 

Background and Objective: Myofascial pain syndrome is one of the painful conditions of the musculoskeletal system. There is controversy about the effectiveness of treatment strategy. This study was done to compare the effects of diclofenac phonophoresis with ergonomic instructions on neck and shoulder pain and disability in women with myofascial trigger points in trapezius muscle.Materials and Methods: This clinical trial study was conducted on thirty female students with trigger points in trapezius muscle in Razmejo-Moghadam Physiotherapy Clinic, Zahedan University of Medical Sciences, Iran during 2009. Patients were randomly assigned to one of three equal groups: diclofenac phonophoresis, ergonomic instructions, and control groups. In phonophoresis group, after applying diclofenac gel, ultrasound with frequency of 1 MHz, continuous mode, intensity 1.5 W/cm2, and duration 4.5 min was used. Ergonomic group received instructions in order to maintain appropriate posture during activity of daily life. Control group received ultrasound without output. A 12 session treatment program, during 4 weeks, 3 sessions per week was performed. Neck pain was assessed before and after intervention with Northwick Park Neck Pain Questionnaire and shoulder pain and disability with Shoulder Pain and Disability Index. Data were analyzed using SPSS-17, Kolmogorov-Smirnov, paired t-test, One-way ANOVA and Tukey tests.Results: Neck pain score decreased from 18±3.5 to 7.6±4.4 in phonophoresis group and from 17.8±3.5 to 10.5±3.4 in ergonomic group (P<0.05). Also, shoulder pain and disability score decreased from 106.2±28.1 to 36.76±30.7 in phonophoresis group and from 103.3±22.9 to 26.2±12.3 in ergonomic group (P<0.05). There was no significant difference between post and pretreatment results in control group. After treatment, there was no significant difference between two treatment groups regarding neck, shoulder pain and disability. However, after treatment there was significant difference between two treatment groups and control group regarding pain (P<0.05).Conclusion: This study showed that diclofenac phonophoresis and ergonomic instructions are effective in decreasing neck and shoulder pain and disability in patients with myofascial trigger points in trapezius muscle. None of both treatment strategy was superior to other.

Abstract: 

Background and Objective: Ecstasy [3, 4 Methylendioxy Meth Amphetamine (MDMA)] exerts destructive effects on body organs particularly on the nervous system. The current study was carried out to measure the adverse effects of MDMA on hepatocyte and liver-specific enzymes.Materials and Methods: In this experimental study, 50 male Wistar rats were randomly divided in equal numbers into 5 groups: control, sham, experimental 1, 2, and 3. Animals in the experimental groups were received, intraperitoneally 2, 4 and 8 mg/kg of MDMA, respectively. The sham group were received normal saline but the control group was not subjected to any injection. Serum samples were collected and levels of three enzymes under study: alanine aminotransferase (ALT), aspartate aminotransferase (AST) and alkaline phosphatase (ALP) were measured. The sections from the liver tissue were prepared counting the hepatocytes. Data were analyzed using SPSS-16 and One-way ANOVA and Tukey's HSD tests.Results: Data indicated the levels of all three enzymes had been elevated in the animal groups that received MDMA and that the increase was statistically significant compared to sham and control groups (P<0.05). Also, in experimental groups the number of hepatocyte were reduced in comparison with controls (P<0.05).Conclusion: This study showed that the MDMA significantly increased ALT, AST, ALP and reduced the number of hepatocytes and these effects are dose depended.

Abstract: 

Background and Objective: Epidermis is the outer layer of skin, regenerating continuously. Epidermal stem cells play important roles in tissue regeneration, scar regeneration and neoplasm formation. This study was displayed for the isolation and culture of interfollicular epidermal stem cells from newborn mouse skin without feeder layer.Materials and Methods: This experimental study was displayed on 0-3 old-day newborn NMRI mouse skin 60-70 gr weight. The epidermal keratinocytes were separated mechanically and enzymatically from 0-3 old day newborn mice skin (NMRI strain) and seeded on fibronectin-collagen culture substrates. Putative epidermal stem cells were selected by rapid adherence for 10 minutes on this composite matrix of type 1 collagen and fibronectin and the unattached cells were discarded and attached cells were cultured in essential minimal eagle medium (EMEM) (ca+2-free culture medium containing 0.05 mM Ca+2, 9% FBS, 50% conditioned medium, EGF (epidermal growth factor) and Cholera Toxin. The immunocytochemistry of b1-integrin analysis used to indicate their stemness nature.Results: The results indicated that rapid adherence yields 50% purity. By using this method, the stem cells have been subcultured continuously without any change in the cell properties. The isolated interfollicular epidermal stem cells, expressed epidermal stem cells special marker (b1-integrin) in high levels, which indicates stem cell nature.Conclusion: This new method yields pure viable epidermal stem cells that can be used in regenerative medicine and cell therapy.

Abstract: 

Background and Objective: After axotomy or the compersion the nerve, the death of spinal cord nerves cell body occur. Compersion is one of the factors causing the degeneration of the spinal cord cell body. This degeneration is due to the reversed factors of the damaged area that have reached to cell body. Prosopis farcta is a member of leguminosae family and mimosaceae subfamily. The purpose of this study was to investigate the effect of ethanolic extract of pod prosopis farcta plant, on neuronal density of anterior horn following sciatic nerve compression in rat.Materials and Methods: This experimental study was performed on thirty male wistar rats with the age of about three months years and 300-350 gr weight. The animals were divided into five groups.A) control, B) compression, C) compression + treatment with 25 mg/kg ethanolic extract, D) compression + treatment with 50 mg/kg ethanolic extract and E: compression + treatment with 75 mg/kg ethanolic extract. After anesthetizing the rats, the muscle of thigh was splited and the sciatic nerve was kept under compersion, the muscle and skin were stitched subsequently. In the experimental groups the alchoholic extract of the prosopis farcta was injected to the rats with 25mg/kg, 50mg/kg, 75mg/kg dosage by the intrapritoneal way weekly. After 28 days of compresion, the rat, were put under the perfusion method and some samples were taken of their lumbar spinal cord and after tissue processes, 7 micron slide were provided of the samples serially. Slides were stained by toluidin blue, and some photos were taken and neuronal density of the alpha motoneurons alpha anterior horn of the spinal cord was calculated by the disector method. Data were analyzed using Minitab software, ANOVA and t- tests.Results: The neuronal density in the compression group (628±29.7) was decreased significantly in compare to the control group (1562±35.3) (P<0.05). The neuronal density in group C (1070±91), increased significantly in compare to the compression group (p<0.05). The neuronal density in group D (1117±62.8) and group E (1669±86.5) significantly increased in compare to the compression group (P<0.05).Conclusion: This study showed that alchoholic extract of the prosopis farcta has a neuroprotective effect following sciatic nerve compression in rats.

Abstract: 

Background and Objective: Sphingosine-1-phosphate (S1P) is involved in regulation of proliferation, differentiation, hypertrophy and anti-apoptosis and activation of satellite cells. This study was done to evaluated the effect of 8 weeks resistance training on sphingosine-1-phosphate level and gene expression of SK1 enzyme, isoforms of MHCs in skeletal muscles of male Wistar rats.Materials and Methods: This experimental study was done on Twenty four 8-week-old 190-250 gr male Wistar rats. The rats were allocated randomly into control (N=12) and training (N=12) groups. Resistance training was done using a 1 meter height ladder with 2 cm grid with an 85 degree incline, and weights attached to rat's tails. The content of S1P present in the chloroform layer was determined by means of high performance liquid chromatography (HPLC). Determination of relative mRNA expression was performed by Real-time PCR. Data were analyzed using SPSS-17, Kolmogorov-Smirnov and independent t-test.Results: Resistance exercise training increased the total content of S1P in FHL (fast-twitch) and soleus (slow-twitch) muscles in comparison with control group (P<0.05). Resistance exercise training changed the gene expression of FHL SK1, SOL SK1, FHL MHC I, Sol MHC I, FHL MHC IIa, Sol MHC IIa, FHL MHC IIb, Sol MHC IIb, FHL MHC IIx, Sol MHC IIx in comparison with control group (P<0.05).Conclusion: This study showed that S1P level and gene expression of SK1, MHCs increased at skeletal muscles after training.

Abstract: 

Background and Objective: Brain ischemia is one of the most important factor of morbidity and mortality and leaving many people with mental and physical disabilities. Until now there are no appropriate medications to prevent and cure ischemic injury. This study was done to evaluate the protective effect of Adenosine A1 receptor and ascorbic acid on hippocampal neuronal density and memory disorder in ischemia reperfusion induced Rats.Materials and Methods: This experimental study was performed on the hippocampus pyramidal neurons on 56 male BALB/c mice. Animals randmly allocated into 8 groups (N=7) including: 1) intact,2) ischemic control group, 3) ischemic, plus agonist and adenosine of A1 receptor, 4) ascorbic acid(100 mg/daily), 5) ischemic plus agonist adenosine receptor (1 mg/1 kg) one week after ischemia,6) ischemia, ascorbic acid befor and after ischemia and A1 receptor (1 mg/1 kg) agonist after ischemia,7) A1 receptor, antagonist (2.25 / 1 kg), one weed after ischemia, 8) Ascorbic acid (100 mg/1kg) before and after ischemia plus A1 receptor antagonist (2.25 / 1 kg) after ischemia. Ischemia induced by clamping of common carotid artery and the drugs was injected subsequently into peritoneum after reduction of inflammation of ischemic zone. The Y-maze memory test performed after completing the treatment period, afterward brains fixed and prepared for microscopic nissl staining method. The counting of pyramidal cells were performed at 53500 square micrometer of CA1. Data were analyzed using SPSS-15 and ANOVA test.Results: The Y-maze test showed extensive deficit in short-term memory in ischemic group (PA=200) but in treatment groups this deficit significantly reduced (PA=243, 248 and 265). The normal neuronal cell in ischemic group was significantly lowered (n=87) than treatment groups (n=111, 105 and 125) including ascorbic acid group (125), adenosine receptor agonist (105) and ascorbic acid plus agonist adenosine receptor (111). The number of normal neuronal cell in ischemic groups significantly is reduced compared to treatment group (P<0.05).Conclusion: This study showed that concurrent treatment of ascorbic acid and Adenosine A1 receptor agonist can significantly reduce the complications caused by brain ischemia in CA1 area of hippocampus.

Abstract: 

Background and Objective: Improving the ability to walk is often a key target for the treatment of abnormal gait in children with spastic diplegic cerebral palsy (CP). One of the goals of orthotic rehabilitation is to improve walking in this field. The aims of this study was to design and manufacture the dynamic neoprene orthoses and evaluate its impact on the gait parameters in children with cerebral palsy.Materials and Methods: This quasi-experimental study was done on 12 children with spastic CP at University of Social Welfare and Rehabilitation, in Tehran, Iran during 2010-11. Initially neoprene dynamic orthosis is designed specifithy for each subject, this neoprene dynamic orthosis was used for six weeks and 6-8 hrs daily. For evaluating the walking speed and the gait variation, 10 meter walking test and visual analogue scale have been used. Modified Ashworth’s Scale and electro-goniometre were used to assess muscle spasticity and the flexion degrees of knee joint. Data were analyzed using SPSS-16, Kolmogorov-Smirnov and Paired t-tests.Results: The alteration of knee flexion angle, walking speed and walking distance following dynamic orthosis were -18.31±4.61 (degree), -0.50±1.82 (meter) and 4.18±1.51, respectively. The improvement in knee joint angle and walking following dynamic orthosis was significant (P<0.05), but the walking speed was not significant.Conclusion: This study showed that neoprene dynamic orthosis can improve knee flexion angle and walking distance among children with spastic diplegic cerebral palsy.

Abstract: 

Background and Objective: Headache is one the most common compliant of patients and has different causes and the migraine and tension headaches are common. Personality is characteristics forming an individual distinctive manner. This study aimed to determine the frequency and association of different types of personality characteristics in patients with migraine and tension headaches.Materials and Methods: This descriptive-analytic study was done on 160 (12 male and 148 female) subjects with chief complaint of headache whome referred to 5th Azar clinics of Gorgan, Iran during 2007-08. Subjects were selected by simple random sampling and were examined by either a neurologist or psychiatrist patient with diagnosis migraine either of or tension headache were included. Selected patients evaluated by short form of Minnesota Multiphasic Personality Inventory (MMPI). Data were analyzed using SPSS-13 and Chi-Square test.Results: The mean age was 11.18±33.29 in women and 34.33±13.7 in men. 96 patients (60%) had tension headache and 64 (41.2%) had migraine. In tension headache, depressed personality (43.8%) and hysterical personality (16.4%) were more common but in migraine headache, depressed personality (48.9%) and paranoid personality (20%) were common but this difference was not significant.Conclusion: This study showed that there is no relation between personality characteristics with tension headach and migraine.

Abstract: 

Background and Objective: Since accurate and quick clinical and paraclinical diagnostic methods are not available, in some cases diagnosis of active pulmonary tuberculosis occurs after considerable time from the onset of disease. This study was designed to determine the diagnostic value of High Resolution Computed Tomographic (HRCT) scan in active pulmonary tuberculosis, in Gorgan, Golestan province, North of Iran.Materials and Methods: This diagnostic screening study was carried out on 135 (79 male and 56 female) hospitalized patients suspected with active pulmonary tuberculosis, and HRCT was used in their course of treatment as recommendation of their clinician. The patients were chosen from 5th Azar hosptial during 2009-10. Also it should be mentioned that patients were selected on avaliabity bases, and they were examined by smear, and sputum culture. The patients with negative smear and culture were set up as true healthy group (64 subjects). The lung or small nuddles in HRCT was considered as proper position of lung involvument in active lung pulmonary. The HRCT findings between the case group (71 subjects) and healthy group were compared. According to HRCT findings, the sensitivity and specifity were determined for each patient. Data were analyzed using SPSS-16 and Chi-Square test.Results: In this study, sensitivity, specificity, positive predictive value and negative predictive value of HRCT in active pulmonary tuberculosis were equal to 97.2%, 71.9%, 79.3% and 95.8% respectively. Involvement of upper and middle lobe of the right lung and upper lobe of the left lung were significantly higher than the control group (P<0.05).Conclusion: This study showed that HRCT has high sensitivity and specificity in diagnosis of active pulmonary tuberculosis and can be used as a quick diagnostic way in active pulmonary tuberculosis, especially in patients with strong clinical suspicion and negative smear.

Abstract: 

Background and Objective: Although multiple sclerosis is a disease affecting white matter of brain and spinal cord, but involvement of basal ganglions in some studies demonstrated a decrease in T2 sequence signals. This study was done to assess signal intensity of basal ganglions in T2 magnetic resonance imaging (MRI) in multiple sclerosisMaterials and Methods: This case – control study was done on 30 multiple sclerosis patients and 30 controls in Imam Reza hospital, Kermanshah, Iran, during 2010. MRI images of multiple sclerosis of patients and control group were matched according to age and sex. The basal ganglia signals were identified based on involvement areas, number of plaques, and brain atrophy in T2 and flair sequences. Data were analyzed using SPSS-16, Chi-Square, Fisher exact test and independent t-tests.Results: Among the case group, 10 patients (33.3%) demonstrated low signals in right thalamus and 14 patients (46.7%) in left thalamus, which was statistically significant compared with control group (P<0.001). The presence of plaques in right and left globus pallidus was observed in 4 (13.3%) and 7 patients (23.3%) respectively. The presence of plaques in right and left thalamus was observed in 10 patients (33.3%) and 14 patients (46.7%) respectively, which was significant compared with control group (P<0.001). The presence of brain atrophy was observed in 18 patients (60%) in case groups and in one patient (3.3%) in control group (P<0.001).Conclusion: This study indicated that reduction of T2 signals in thalamus is a valuable finding in multiple sclerosis patients and it may facilitate the diagnosis of multiple sclerosis.

Abstract: 

Background and Objective: Continuous antigenic variation of Influenza a viruses causes a major concern to develop Influenza vaccine. Conserved antigens are suitable candidates for vaccine production due to its non-requirement to match the designed strains with circulating strains. The M2 gene is conserved among Influenza a viruses and has potential to be considered as a universal vaccine. This study was designed to evaluate the effects of aqueous Echinacea purpurea extract on immunogenicity of DNA vaccine encoding M2 gene of Influenza virus.Materials and Methods: This interventional study was carried out on female BALB/c mice with 3-4 week age (250-300 gr). Plasmid DNA encoding M2 gene (pcDNA-M2) of Influenza virus A/New Caledonia/20/99 (H1N1) was transformed into E.coli top10 f' and cultured in LB broth media. Large scale plasmid preparation was done and the concentration was measured by spectrophotometric method. Mice were divided into eight groups and immunized three times with fifteen days apart. Vaccine groups received inactivated Influenza virus or pcDNA-M2, alone or in combination with Echinacea extract. Control groups were injected pcDNA, Echinacea extract, and phosphate buffer. All animals were left to bleed before immunization and at 21 days after the last vaccination and specific anti-M2 antibodies were measured by indirect ELISA. Then the mice were intranasally challenged under an aesthesia with mouse-adapted PR8 Influenza virus and monitored for 3 weeks to evaluate the vaccine regimen efficacy in reduction of mortality rate compared to control groups. Data were analyzed using SPSS-16,One-way ANOVA and Kaplan–Meier tests.Results: The highest specific immune response was obtained in mice received inactivated virus plus extract (P<0.05). Immune responses in mice inoculated with pcDNA-M2 were significantly higher compared to all control groups mice (P<0.05). In addition the specific immune responses in group inoculated with pcDNA-M2 and aqueous extract was higher compared to the group receiving only pcDNA-M2 (P<0.001). The highest survival rate was observed in mice injected with inactivated virus or pcDNA-M2 plus extract.Conclusion: This study showed that pcDNA-M2 induced specific immunity and protected mice against lethal challenge with PR8 Influenza virus. Furthermore, application of Echinacea extract with M2 gene vaccine increased vaccine efficacy.

Abstract: 

Background and Objective: Nanosilver particles are one of functional nanotechnology filed. These nanoparticles have antibacterial and antifungal characteritic. Combination therapy is one of new and specific method for therapy of diseases in medicine and pharmacology. Some plants are useful in growth inhibition of Aspergilus Niger. This study was done to evaluate the synergic effect of alcoholic eucalyptus and nanosilver on colony count of Aspergilus Niger.Materials and Methods: In this laboratory study PAN were cultured in PDA medium. Five experimental and one control group were designed for the comparison of synergic effect of alcoholic eucalyptus and nanosilver, nanosilver 50ppm, nanosilver 12.5ppm, eucalyptus ethanolic extract 100%, eucalyptus ethanolic extract 100% mixed with nanosilver 12.5ppm, eucalyptus ethanolic extract mixed with 50ppm, and double distilled water were given to group 1, 2, 3, 4, 5 and control group respectively. Morphological, diameter, and colony numbers in various culture media were compared subsequently. The repeated number of Aspergilus Niger were 24, and they were chosen randomly. Data were analyzed using SPSS-15 and ANOVA test.Results: The Aspergilus niger colony were reduced 8 days following treatment from 200 colony in control group to 90, 75, 55 and 43 in nanosilver group in 12.5 ppm, nanosilver group in 50 ppm, nanosilver group in 50 ppm mixed with eucalyptus ethanolic extract and finally eucalyptus ethanolic extract group, respectivley (P<0.05). The colony count 24 day following treatment were reduced from 200 colony in control group to 42, 14 and 2 in eucalyptus ethanolic extract group, nanosilver group in 12.5 ppm mixed with eucalyptus ethanolic extract and finally nanosilver group in 50 ppm mixed with eucalyptus ethanolic extract, respectivley (P<0.05).Conclusion: This study showed that nanosilver with 50 ppm concentration mixed with eucalyptus ethanolic extract have synergic effect on reducing the Aspergilus niger growth rate.

Abstract: 

Background and Objective: The risk of coronary heart disease (CHD) is proportional to the LDL-C lipoprotein. Due to frequent use of Friedwald formula in estimation of LDL-C in most laboratories, this study was done to compare the Friedwald formula and direct measurement to determine the serum levels of LDL-CMaterials and Methods: This descriptive study was conducted on of 598 patients 226 male and 372 female whome referred to Imam Ali hospital Andimeshk cityin Khozestan province of Iran for health check up during 2009. 5 ml of the venous blood was drown. Total cholesterol (TC) (mg/dl), Triglyceride (TG) (mg/dl), HDL-C (mg/dl) and LDL-C (mg/dl) of serum are measured with Pars azmun company kits. The Friedwald formula was used for estimation of LDL-C. The K=3, 3.5 and 5 were used to stimate the lipid by Friedwald formula. Data were analyzed using SPSS-18, Pearson correlation coefficient and ANOVA tests.Results: A total of 598 serum samples collected; 37.8% were men and 62.2% women. The mean age of participants was 38.8±10.77 years. Minimum age 21 years and maximum age was 77 years. Mean deviation for TG≤150, 201-300 and 301-400 in Friedwald formula (k=5) were -13.01±8.79,-17.11±13.17 and -18.63±18.54, respectively and with k=3 are -.39±12.04, -0.078±18.55 and 0.04±25.55 and for TG between 151-200 is -9.72±10.54 and with k=3.5 is equal to 0.82±13.70. Pearson correlation test showed that direct measurment and calculated from the equation Friedwald, for triglycerides in the area equal to or less than 150, 151-200, 201-300 and 301-400 mg/dl, with correlated to Pearson correlation coefficient were 0.982, 0.991, 0.991 and 0.975, respectively.Conclusion: This study showed that the direct measurement method is superior to the Friedwald equation, otherwise, equation Friedwald formula with K=3 is recommended.

Abstract: 

Background and Objective: Cutaneous leishmaniasis is one of the zoonosis disease which is transmitted by sand fly and has been considered as one of the most important health problems in Iran. This study was designed to assess the status of cutaneous leishmaniasis in Gonbad Kavoos, North of Iran.Materials and Methods: This descriptive-analytical retrospective study was performed on the two groups consist of: 1799 patients (995 men and 804 women) referred to health centers of Gonbad Kavoos in Golestan Province North of Iran during 2009-11. Also 278 men and 271 women were selected randomly in one of the villages for the assessment of acute and scar wounds. Patient’s characteristics such as age, sex, habitat, number and sites of ulcer(s), month and years of incidence were registered. Data were analyzed using SPSS-13 and Chi-Square test.Results: From 1799 under care patients, 995 cases (55.3%) were males and 804 cases (44.7%) were females (P<0.05). 1542 patients (85.7%) resided in rural areas, while 257 (14.3%) lived in urban areas (P<0.05). The most frequent age group was 0-9 years old (43.3%) (P<0.05). Hands were the most common sites of ulcer (42.3%) and 37.9% of the patients with one ulser. The highest prevalency of disease was observed in months of October and November (68.4%) 4% and 78.6% of subjects selected from villages were presented with acute wound and scar, respectivley.Conclusion: This study showed that Cutaneous leishmaniasis in 2010-11 was dispersed in Gonbad-Kavoos a city in North of Iran. The endemicity situation of this disease in this area is hypoendemic.

Abstract: 

Background and Objective: Nosocomial infections are one of main causative agents of mortality among hospital patients. This study was done for the determination of efficacy of commercial disinfectants such as: formaldehyde, glutaraldehyde, microten, the alcohol 70 and savlon–alcohol on isolated saprophytic fungi from hospital environments.Materials and Methods: This descriptive study was done on 33 isolated fungi from teaching hospitals of Tehran during 2009-10. The identified samples were randomly chosen. Stock fungal suspensions were supplied from each fungus with cells ranging 0.5-5×104 mg/cfu in 1ml with spectrophotometer at the wavelength of 530 nm. For evaluation of antifungal activity of commercial disinfectants formaldehyde, glutaraldehyde, microten, alcohol 70 and savlon–alcohol disinfectants, 0.25cc stock solutions were mixed with 3.75 cc disinfectants solutions and the new diluted samples held at 25c for 15, 30 and 60 min. The culture medium was checked for growth of fungi until 8 weeks.Results: Following specific period isolated fungi were including Aspergillus spp with 39.4%, Penicillium spp with 36.4%, Fusarium spp with 12.1%, Rhizopus with 6.1%, Alternaria and Circinella with 3 %. Formaldehyde 8% and glutaraldehyde 8% with activity against 63.6% and 39.3% were effective disinfectants at 15 min. Formaldehyde 8% with activity against 74.8% of fungi, was effective disinfectant at 30 min. Glutaraldehyde 8% and formaldehyde 8% with 100% prevention of growth were effective disinfectants at 60 min.Conclusion: According to this study formaldehyde 8% and glutaraldehyde 8% showed to have the highest antifungal activities. Synergetic fungicidal activity of comenercial disinfectants, dependent on time and concentration.

Abstract: 

Background and Objective: Several studies have shown that during stroke changes occur in some paraclinic parameters which may have either prognostic or diagnostic value, including electrocardiographic changes. This study was done to evaluate the prognostic value of prolonged QT segment in patients with ischemic stroke.Materials and Methods: This descriptive study with an easy convenient sampling was carried out on 175 patients (73 male and 102 female) with acute ischemic stroke whome have been diagnosed by brain imaging (computed tomography scan and magnetic resonance imaging). All patients were suffering from stroke for the first time and if they use any drug or have underlying disease except diabetes mellitus- hypertension- heart disease and hyperlipidemia were excluded. In the first 24 hours of admission, an electrocardiogram (ECG) were taken for measurement of QT segment and according to death or discharge, patients were divided into two groups, and the mean of QT segment (corrected QT) subsequently were assessed. Data were analyzed using SPSS-15, Chi-Square and independent t-tests.Results: Twenty patients died in the course of admission. The mean of QT segment (corrected QT) in dead patients was 471.15±61.70 and in discharged patients was 421.52±62.96 (P<0.05). The abundance of prolonged QT segment accompanied with death was more frequent. Hypertension, diabetes mellitus, hyperlipidemia and heart disease were significantly correlated with morbidity and mortality (P<0.05). There is no relation between mortality with age and sex.Conclusion: This study showed that mean of QT segment (corrected) and also cases of prolonged QT segment were abundant among dead patients, therefore prolongation of QT segment has prognostic value in patients with ischemic stroke.

Abstract: 

Background and Objective: The etiology of childhood leukemia as the most common childhood malignancy remains largely unknown. This study was done to ascess the risk factors in childhood lymphoblastic leukemia in Shiraz-Iran.Materials and Methods: This case-control study was done on 141 children younger than 18 years suffering from acute lymphoblastic leukemia (ALL) whome resided at Fars Province of Iran during 2009. Patients were individually matched with 141 controls in respect to age, sex and residential area. Variables included: maternal age, parental education, father occupation, child birth weight and birth order, number of siblings, history of pet ownership including cat or dog, history of child day care attendance, history of leukemia in relatives, and history of mother diagnostic radiography during pregnancy. In order to evaluate the relationships between each variable and the risk of leukemia, odds ratio (OR) and 95% confidence interval (CI) were estimated using conditional logistic regression. Data were analyzed using SPSS-16 and Chi-Square test.Results: The agricultural occupation fathers in case and control were 17% and 5.7%, respectivley (P<0.01). The association between risk of childhood lymphoblastic leukemia with birth order³3 (OR=5.939, 95% CI: 2.646-13.331, P<0.01), pet ownership (dog or cat) (OR=2.582, 95% CI: 1.265-5.269, P<0.009) and history of leukemia in first and second degree relatives (OR=3.5, 95% CI: 1.252-10.633, P<0.027) was significant. No relationship was found between birth weight, day care attendance, history of miscarriage, number of siblings and history of mother diagnostic radiology tests with risk of acute lymphoblastic leukemia.Conclusion: This study showed that father occupation, birth order, pet and history of leukemia in relation are risk factors of childhood acute lymphoblastic leukemia.

Abstract: 

Background and Objective: Irritable bowel syndrome (IBS) is the most prevalent gastrointestinal disorder and is one the common conditions seen by gastroenterologists in their daily practice. This study was done to determine the prevalence of celiac disease in patients with irritable bowel syndrome in Ardabil-Iran.Materials and Methods: This descriptive study was conducted on 105 patients with IBS whome referred to the gastroenterology unit in Ardabil-Iran during 2009-10. Serum IgA anti tTG were measured all patients with positive for antibodies against tTG were candidated for upper endoscopy and biopsy. Data were analyzed using SPSS-16, t-test, Chi-Square and Fisher’s exact tests.Results: The average age of IBS subjects were 31.4±10.14 years (range 16-63 years). Celiac disease was diagnosed in 14 subjects (13.5%). The celiac patient age were 22-55 years with mean of 34.93±9.47. Among celiac afflicted patients IBS type D and M observed among 10 and 4 patients, respectivley. Celiac affected female constituted 12 (85.7%) of all patients (P<0.05). 10 of these patients were IBS-D and 4 with IBS-M. From 14 celiac patient 4 (28.57%) were family related, but this rate among IBS patient was 3.3%, this difference was significant (P<0.05).Conclusion: This study showed that the prevalence of celiac in patients with IBS is found to be 13.5% which seem is more than ther studies in other parts of Iran.

Abstract: 

Red eye is the most common sign of ocular inflammation. Serious cases of red eye need diagnosis and proper treatment by ophthalmologist but many other from of red eye can be managed primarily by health care personals. If General practioners were able to diagnose and differentiate the causes of red eye the serious complication have not been observed and the patients were have been treated on due time. Some of the causes of red eye are: blepharitis, corneal erosion, sub conjunctival hemorrhage, glaucoma, episcleritis and scleritis.
