Abstract: 

Introduction: Opioid acute poisoning is one of the most important causes of mortality in Iran. Adulterants such as volume expanders and by- product impurities induce major toxic effects in co-ingestion status. In this article, we report a case of death following ingestion of lead contaminated opium.
Case Report: The decedent was a 27 years old man .He was a worker in a tube manufacturing factory. After ingestion of opium, he was referred to hospital with developed agitation and decrease of mental status. In hospital, developed seizure and cardiopulmonary depression was occurred. CPR and intubation was performed. Then he was hospitalized in ICU ward. Because of lead poisoning history in 3 months ago, his blood lead level was analyzed.
The results showed the toxic level of lead in his blood. In spite of, initiation of chelator therapy, he was expired 3 days after admission. For analysis of lead impurities, the opium sample residue which he ingested transferred to forensic toxicology laboratory of Legal Medicine Organization of Iran. The analysis was performed by atomic absorption spectrophotometery and results showed 5.68% w/w lead impurity in opium sample.
Conclusion: We concluded that the presence of lead impurity in opium could induce lead poisoning in this case. Therefore, identification of opioid impurities is necessary for treatment of intoxicated cases.
