Abstract: 

BACKGROUND: DIFFERENT CUTOFF POINTS HAVE BEEN DEFINED FOR VARIOUS ANTHROPOMETRIC PARAMETERS TO DIAGNOSE CENTRAL OBESITY FOR DISCRIMINATING METABOLIC SYNDROME FROM NORMAL STATEMENT. HOWEVER, THESE CUT POINTS ARE MAINLY DERIVED FROM WESTERN OR SOUTHEASTERN POPULATION AND MAY NOT BE APPLICABLE IN OUR POPULATION. WE THEREFORE INVESTIGATED WHETHER BODY MASS INDEX (BMI), WAIST CIRCUMFERENCE (WC) OR WAIST TO HIP RATIO (WHR) IS A BETTER PREDICTOR FOR METABOLIC SYNDROME AND WHICH CUTOFF POINTS OF THESE SURROGATES ARE MORE APPROPRIATE FOR DISCRIMINATING THIS SYNDROME IN THE TWO GENDERS AS WELL AS IN DIFFERENT AGE SUBGROUPS. METHODS: THE PRESENT CROSS-SECTIONAL SURVEY WAS CONDUCTED ON A SAMPLE OF ISFAHAN COHORT STUDY (ICS). TOTALLY, 468 CONSECUTIVE INDIVIDUALS (194 AND 274 SUBJECTS WITH AND WITHOUT METABOLIC SYNDROME) ACCORDING TO THE NATIONAL CHOLESTEROL EDUCATION PROGRAM’S ADULT TREATMENT PANEL III (ATP-III) CRITERIA WERE RECRUITED. ANTHROPOMETRIC INDICES WERE MEASURED AND PLOTTED USING RECEIVER-OPERATING CHARACTERISTIC (ROC) CURVES. RESULTS: ACCORDING TO ROC CURVE ANALYSIS, WC AND WHR PARAMETERS WERE BETTER INDICATORS OF METABOLIC SYNDROME THAN BMI IN WOMEN, WHEREAS WHR HAD A LOWER DISCRIMINATING VALUE COMPARED TO OTHER TWO IN MEN. AMONG THESE THREE ANTHROPOMETRIC PARAMETERS, WC AND WHR HAD A HIGHER SENSITIVITY AND BMI HAD A LOWER SENSITIVITY FOR PREDICTING METABOLIC SYNDROME IN WOMEN COMPARED WITH IN MEN. THE CUT-POINTS FOR WC (90.3 VERSUS 90.0) WERE NEARLY EQUAL IN MEN AND WOMEN, RESPECTIVELY. WOMEN HAD HIGHER CUT-POINTS FOR BMI (28. 5 KG/M2) COMPARED TO MEN (26.0 KG/M2), WHILE THIS DISCREPANCY WAS ADVERSE WITH RESPECT TO WHR. THE HIGHEST SENSITIVITY AND SPECIFICITY WAS EXHIBITED BY THE WC CUT-POINTS. REGARDING OPTIMAL AGE-SPECIFIC CUT-POINTS FOR BMI, WC, AND WHR FOR PREDICTING METABOLIC SYNDROME, WC HAD THE HIGHEST DISCRIMINATING VALUE COMPARED TO OTHER INDICATORS IN THE DIFFERENT AGE SUBGROUPS. THE OPTIMAL CUT-POINTS FOR THE THREE PARAMETERS GRADUALLY INCREASED WITH AGE. CONCLUSION: ACCORDING TO OUR FINDINGS AND REGARDLESS GENDER AND AGE VARIABLES, WC CAN BE THE BEST PARAMETER FOR PREDICTING METABOLIC SYNDROME AMONG IRANIAN POPULATION.
