Abstract: 

IT IS ESTIMATED THAT THERE ARE 180,000-250,000 IDUS LIVING IN IRAN. ACCORDING TO MOH DATA MORE THAN 65% OF IDENTIFIED HIV CASES HAVE BEEN INFECTED THROUGH SHARING SYRINGES. IN A MOH STUDY IN 2008, THE PREVALENCE OF HIV AMONG IDUS IN TEN CITIES OF IRAN WAS 15. 3%. NEEDLE, SYRINGE PROGRAM (NSP) IS ONE OF HIV PREVENTION INTERVENTIONS WHICH IS RECOMMENDED TO PREVENT HIV TRANSMISSION AMONG PEOPLE WHO INJECT DRUGS. THE FIRST PILOT NSPS IN IRAN LUNCHED AT 2003 BY MINISTRY OF HEALTH AND MEDICAL EDUCATION. THE PROGRAM SCALED UP AT NATIONAL LEVEL AT 2007 BY MINISTRY OF HEALTH (MOH) AND SOCIAL WELFARE ORGANIZATION (SWO). PRISON ORGANIZATION ALSO HAS HAD LIMITED EXPERIENCES WITH NSPS IN CLOSED SETTINGS. PRIMARY NSPS WERE DELIVERED THROUGH DROP IN CENTERS AND OUTREACH TEAMS. SECONDARY NSP THROUGH PHARMACIES ALSO LEGALIZED IN IRAN SINCE 2004. HERE WE ARE GOING TO REVIEW THE ACCESSIBILITY, AVAILABILITY, COVERAGE AND EFFECTIVENESS OF NSPS IN IRAN. NSPS ARE DELIVERED IN ABOUT 150 DICS ACROSS THE COUNTRY. THEY SHOWED TO BE ACCEPTABLE FOR PEOPLE WHO INJECT DRUGS PARTICULARLY WHEN DELIVERED AS PART OF A COMPREHENSIVE PACKAGE OF A HARM REDUCTION AND TREATMENT SERVICES. THE AVAILABLE DATA SHOWS HIGH NUMBER OF DRUG USERS REFERRING TO THESE PROGRAMS, ALTHOUGH THE CURRENT REPORTING SYSTEM DOES NOT PROVIDE THE DATA NEEDED TO CALCULATE THE COVERAGE OF THE PROGRAM. THERE IS A PAUCITY OF RESEARCH ON EFFECTIVENESS OF NSPS ON HIGH RISK INJECTION AND SEXUAL BEHAVIORS OF DRUG USERS AND THERE IS NO DATA SHOWING EFFECTIVENESS OF NSP ON PREVALENCE/INCIDENCE OF HIV AND/OR OTHER BLOOD BORNE VIRUSES IN IRAN. THERE ARE TWO STUDIES WHICH ADDRESSED THE EFFECTIVENESS OF NSPS ON HIV RELATED HIGH RISK BEHAVIOR AMONG PEOPLE WHO INJECT DRUGS. THE FIRST STUDY SHOWS THAT DIC’S CLIENTS, WHO RECEIVE MORE THAN 7 SYRINGES PER WEEK IN A ROUTINE NSP, SIGNIFICANTLY REDUCE THEIR HIGH RISK INJECTING BEHAVIOR. THE SECOND STUDY RESULTS INDICATE THAT ADDING ON A BRIEF SKILL-BASED HIV PREVENTION PSYCH EDUCATION TO ROUTINE NSPS CAN CONTRIBUTE TO REDUCING HIGH RISK SEXUAL AND INJECTION BEHAVIORS. FINALLY, THE STRATEGIES TO PROMOTE NSPS IN IRAN WILL BE DISCUSSED.
