Abstract: 

PROPOSE: ACCORDING TO SEVERAL REPORTS PARTIAL NEPHRECTOMY HAVE THE SIMILAR RESULTS WITH RADICAL NEPHRECTOMY IN SELECTED CASE WITH ORGAN CONFINED, SOLITARY AND LESS THAN 4 CM DIAMETER RENAL MASS. OUR PURPOSE IS PRESENTATION OF 4 CASES OF LAPAROSCOPIC PARTIAL NEPHRECTOMY WHICH HAS BEEN DONE FOR THE FIRST TIME IN IRAN.
METHODS & MATERIALS: RECORDS OF 4 PATIENTS WITH LAPAROSCOPIC PARTIAL NEPHRECTOMY FOR LESS THAN 4 CM RENAL MASS WERE REVIEWED. IN 3 CASES RETROPERITONEAL APPROACH WAS SELECTED FOR LOCATION OF MASS IN LOWER POLE AND POSTERIOR SURFACE OF KIDNEY. IN ALL PATIENTS TEMPORARY CLAMPING OF RENAL ARTERY WAS DONE DURING DISSECTION AND SUTURING WAS DONE FOR PERMANENT HEMOSTASIS.
RESULTS: MEAN OPERATING TIME WAS 3.5 H AND MEAN HOSPITALIZATION WAS 4 DAYS. POST OPERATION PAIN WAS MINIMAL IN 3 PATIENTS DUE TO RETROPERITONEAL APPROACH. PATHOLOGY WAS MALIGNANT IN TWO CASES, ONE RENAL ADENOMA AND ONE ANGIOMIOLIPOMA. RENAL ISOTOPE SCAN WAS DONE POSTOPERATIVELY THAT STABLISHED NORMAL FUNCTION IN REMANENT KIDNEY.
DISCUSSION & CONCLUSION: LAPAROSCOPIC PARTIAL NEPHRECTOMY IS A SUITABLE APPROACH FOR SELECTED RENAL MASS. IT,S ADVANTAGES ARE: SHORTER HOSPITALIZATION TIME, FASTER RESUME NORMAL ACTIVITY AND LESS POSTOPERATIVE MORBIDITY.
