Abstract: 

PROPOSE: THIS STUDY WAS CONDUCTED TO EVALUATE THE SIGNIFICANCE OF SMOKING STATUS ON THE GRADE OF TRANSITIONAL CELL CARCINOMA OF THE BLADDER AND ITS PROGNOSIS.
METHODS & MATERIALS: DURING A PROSPECTIVE CASE CONTROL STUDY FROM APRIL 2000 TO FEB.2003, PATIENTS WITH TCC OF BLADDER WHO REFERRED TO OUR CLINIC WERE SELECTED AND DIVIDED IN SMOKING AND NON SMOKING GROUPS. GROUPS WERE MATCHED DUE TO OTHER KNOWN RISK FACTORS. USING CHI-SQUARE, T-TEST, LINEAR REGRESSION AND ANALYSIS OF VARIANCE, EFFECTS OF SMOKING STATUS ON SIZE, NUMBER AND PRESENTING GRADE IN EACH GROUP WERE CALCULATED. P VALUE LESS THAN 0.05 WAS CONSIDERED SIGNIFICANT.
RESULTS: AMONG 185 PATIENTS WHO WERE INCLUDED IN THE STUDY WITH MEAN AGE OF 65.1 YEARS OLD, THERE WERE 35 FEMALE AND 149 MALE WITH MALE TO FEMALE RATIO OF 4.1 TO 1. THERE WERE 83 PATIENTS WITH POSITIVE HISTORY OF SMOKING (44.9%) WITH MEAN SMOKING OF 20 PACK-YEARS (67.5%). AGE AND SEX DISTRIBUTIONS WERE NOT STATISTICALLY DIFFERENT IN THESE TWO GROUPS (P=0.2 AND P=0.4). 67.5% OF PATIENTS WITH POSITIVE HISTORY OF SMOKING HAD LOW GRADE CANCER AND 32.5% HAD HIGH GRADE TUMORS. THIS WAS 97.1% AND 2.9% IN CONTROL GROUP RESPECTIVELY, WHICH WAS SIGNIFICANTLY DIFFERENT (P<=5.8) WITH ODD RATIO=15.9 (CI=95% Z
B=5.8) THERE WAS A STATISTICALLY SIGNIFICANT CORRELATION, BETWEEN HISTORY OF SMOKING AND SIZE AND NUMBER OF TUMOR (P=0.0005, P=0.0001 RESPECTIVELY). POSITIVE HISTORY OF SMOKING WAS ALSO ASSOCIATED WITH HIGHER GRADE OF TUMOR IN BOTH MEN AND WOMEN (P=0.0002, P=0.017 RESPECTIVELY).
DISCUSSION & CONCLUSION: THIS STUDY SHOWED THAT SMOKING MAY NOT ONLY INDUCED BLADDER CANCER BUT ALSO ONCE PRODUCED; IT HAS HIGHER GRADE AND WORSE PROGNOSIS. SO STOPPING SMOKING MIGHT FAVORABLY ALTER THE COURSE OF BLADDER CANCER.
