Abstract:

OBJECTIVES: IN ORDER TO ASSESS THE ACCURACY (SENSITIVITY AND SPECIFICITY) OF TRAK TEST (BLADDER TUMOR ANTIGEN LEVEL) IN GRADING AND STAGING OF BLADDER TUMOR. ALSO THE SENSIVITY AND SPECIFICITY OF BTA TRAK TEST IS EVALUATED FOR DIFFERENT STAGES AND GRADES IN RESPECT TO SIZE, SHAPE, LOCATION AND ALSO THE RELATION WITH INTERAVESICAL BCG THERAPY.
MATERIALS AND METHODS: THE 110 PATIENTS WITH SYMPTOMS OF BLADDER CANCER WERE EVALUATED. OF THIS PATIENTS, 84 PATIENTS HAD BLADDER TUMOR (CASE GROUP) AND 26 PATIENT WERE TUMOR FREE (CONTROL GROUP). THE SPECIFICITY AND SENSIVITY OF BTA TRAK TEST FOR GRADING AND STAGING OF BLADDER TUMOR WERE EVALUATED, THEN THE ACCURACY OF DIFFERENT LEVELS OF BTA FOR DETECTION OF LOW GRADE AND LOW STAGE TUMORS FROM HIGH GRADE AND HIGH GRADE TUMORS WERE OBTAINED.
RESULTS: OF PATIENTS 84 HAD BLADDER TUMOR AND THE SENSIVITY OF TEST WAS 76% AND IN 26 PATIENT WITHOUT TUMOR, 4 HAD POSITIVE TEST THEREFORE THE SPECIFICITY WAS 84%. THE SENSIVITY AND SPECIFICITY WERE HIGHER IN HIGH GRADE AND HIGH STAGE TUMORS THAN LOW GRADE AND LOW STAGE TUMOR AND THE RESULTS IN RELATION TO STAGE WERE STATISTICALLY SIGNIFICANT. THE SENSIVITY AND SPECIFICITY FOR ANY CUT OFF POINT FOR BTA TRAK FOR DETECTION OF PROGRESSIVE STAGES AND GRADES WERE OBTAINED.
CONCLUSIONS: WE RECOMMENDED OF BTA TRAK TEST AND CYSTOSCOPY FOR DETECTION OF BLADDER TUMOR.
