Abstract:

PURPOSE: CYCTOSOPY AND URINARY CYTOLOGY ARE CURRENTLY THE BASIS OF DIAGNOSIS AND FOLLOW–UP OF BLADDER TUMOR.
RESEARCHES TO FIND A HIGHLY SENSITIVE AND SPECIFIC TUMOR MARKER FOR DIAGNOSIS OF BLADDER TUMOR ARE ACTIVELY UNDERWAY DUE TO LOW SENSITIVITY AND HIGH COST OF CYTOLOGY.
MATERIALS & METHODS: THIS CROSS–SECTIONAL STUDY WAS PERFORMED IN IMAM KHOMEINI HOSPITAL IN 5 PATIENTS. EITHER WITH ACTIVE BLADDER TUMOR IN ROUTINE FOLLOW–UP UBC LEVEL WAS DETERMINED IN ADDITION TO STANDARD CYSTOSCOPY AND CYTOLOGY FOR DIAGNOSIS AND FOLLOW–UP. UBC LEVEL WAS ALSO DETERMINED IN 65 PERSONS WHO HAD BEEN REFERRED FOR URINALYSIS TO A LABORATORY IN ONE DAY.
RESULTS: 69.4% OF THE PATIENTS HAD UBC LEVEL MORE THAN 1
MG/DL WHICH REGARDED AS POSITIVE AND 50.6% HAD UBC<1 MG/DL. MEAN UBC LEVEL IN PATIENTS WAS 3.77MG/DL WHILE IT WAS ONLY 0.508MG/DL IN CONTROLS. (P VALUE<0.0001) SENSITIVITY OF UBC WAS 53.3% AND THE SPECIFICITY OF URINARY CYTOLOGY WAS 17.3% AND 88.2% CONSECUTIVELY. THIS DIFFERENCE WAS STATISTICALLY SIGNIFICANT. (P VALUE<0.001) CONCLUSIONS: UBC IS MORE SENSITIVE THAN URINARY CYTOLOGY, ALTHOUGH CYTOLOGY STILL RETAINED ITS PRIORITY IN SPECIFICITY AND NOT FAVORABLE SENSITIVITY.
