Abstract: 

PROPOSE: IS ESWL AND TUL EVERY TIME SAFE? (20-25 PERCENT ARE INFECTIVE) SO CONSIDER OF THIS POINT IN MANAGEMENT OF URINARY STONE IS IMPORTANT.
METHODS & MATERIALS: 24 PATIENTS WITH URINARY STONE SELECTED FOR SURGERY AND UNDER GENERAL ANESTHESIA AFTER PREPARING THE PATIENT FIRST IN STERILE CONDITION THE PATIENT CATHETERIZED AND URINARY SAMPLE IS TAKEN (TUBE NO 1), AFTER EXPOSING THE STONE ANOTHER SAMPLE TAKEN (TUBE NO 2) AND STONE PLACED IN FIVE ML SALIN (TUBE NO 3) AND THEN STONE PASSAGE IN ANOTHER THREE TUB WITH NORMAL SALIN IN THAT IN THIRD TUBE AFTER SAMPLE TAKEN (TUBE 4). THE STONE IS FRAGMENTIZED AND ANOTHER SAMPLE IS TAKEN (TUB N=5). THE ENTIRE SAMPLE IS CULTURED AT BLOOD AGAR AND EOSIN METHYLEN BLUE.
RESULTS: STONE CULTURE OF SEVEN CASES WERE POSITIVE (30% PATIENT) AND IN THREE PATIENT TUBE NO=1 WERE NEGATIVE BUT OTHER TUB WERE POSITIVE (IN 43% CULTURE RESULT OF URINE AND STONE NOT SAME). PROTEOUS AND POSITIVE COCCI WERE THE MOST FINDING MICROORGANISM.
DISCUSSION & CONCLUSION: 30% CASE OF URINARY HAD POSITIVE STONE CULTURE AND 43% PATIENT OF STONE POSITIVE HAD URINE NEGATIVE URINE CULTURE. THIS IS WARNING ABOUT ESWL IN PATIENT WITH HIGH RISK SO IS IT NECESSARY ANTIBIOTIC PROPHYLAXY IN THESE PATIENTS.
