Abstract:

PURPOSE: IN THE MANAGEMENT OF URINARY TRACT STONES ENDOUROLOGIC PROCEDURES HAS BEEN RECENTLY DEVELOPED EXTRAORDINARILY. WE EVALUATED THE RESULTS OF THE TREATMENT OF URINARY TRACT STONES MANAGED WITH PNEUMATIC LITHOTRIPSY.
MATERIALS & METHODS: A TOTAL OF 714 PATIENTS UNDERWENT PNEUMATIC LITHOTRIPSY, FROM JANUARY 1999 TO FEBRUARY 2002.
AGE RANGED BETWEEN 2 AND 101 YEARS.539 CASES (75.5%) WERE MALE AND 175 CASES (24.5%) WERE FEMALE. STONES WERE INCLUDED: URETHRAL STONES IN 6 CASES (10.8%), BLADDER STONES IN 64 CASES (8.9%), R. URETHRAL STONES IN 284 CASES (51.4%), L. URETHRAL STONES IN 258 CASES (46.7%) AND BOTH URETHRAL STONES IN 20 CASES (2.8%), STONES SIZE WERE BETWEEN 4 AND 9 MM IN 226 CASES (40.9%), 10 AND 15 IN 224 CASES (44.2%), 16 AND 20 MM IN 14 CASES (2.5%) AND GREATER THAN 20 MM IN 7 CASES (1.2%).
RESULTS: THE COMPLICATIONS INCLUDED: 1- TOTAL URETHRAL OBSTRUCTION IN 2 CASES (0.28%) WHICH MANAGED BY AUTO TRANSPLANTATION. 2- URETHRAL AVULSION IN 4 CASES WHICH TREATED BY URETHRAL REINPLANTATION. 3- UNSUCCESSFUL LITHOTRIPSY IN 30 PATIENTS (4.2%) 4- UNCONTROLLABLE BLEEDING IN 29 PATIENTS (4.6%) 5- RENAL COLIC IN 35 PATIENTS. (4.9%) 6- FEVER AND CHILL IN 38 PATIENTS (5.3%) 7- STONE MIGRATION TO THE KIDNEY IN 71 PATIENTS (7.2%) 8- STENTING IN 156 PATIENTS (21.8%) - SUCCESS RATES IN STONE FRAGMENTATION WERE:
1- 100% IN URETHRAL STONES 2- 100% IN LADDER STONES 3- 89.6% IN URETHRAL STONES - 71 PATIENTS (9.9%) ADMITTED IN JULY AND 49 PATIENTS (6.8%) IN NOVEMBER.
CONCLUSIONS: PNEUMATIC LITHOTRIPSY IS AN EASY, SAFE, COST, EFFECTIVE AND WITH A RELATIVELY LOW COMPLICATIONS RATE.
