Abstract: 

PROPOSE: MANY SMALLER FLEXIBLE OR RIGID INSTRUMENTS MAY PASSED WITHOUT PRIOR DILATION DEPENDING ON THE SIZE OF URETER. ALTHOUGH MOST ORIFICES ARE APPROXIMATELY 3MM IN SIZE, SOME ARE SMALLER. BECAUSE THE SIZE IS NOT PREDICTABLE, MOST AUTHORITIES ROUTINELY DILATE BEFORE A URETEROSCOPIC PROCEDURE.
METHODS & MATERIALS: CYSTOSCOPY (NO.22FR. STORZ OR NO.21FR. WOLF SHEAT) BEFORE URETEROSCOPY DONE FOR ALL PATIENTS CANDIDATES FOR TUL OR OTHER URETEROSCOPIC PROCEDURES AND SUCCESSIVELY LARGER URETERAL CATHETERS NO.4, 6, 8, 10 PASSED TO URETER. IN CHILDREN CYSTOSCOPE 12FR. WERE USED.
RESULTS: DURING A PERIOD OF 28 MONTHS 578 PATIENTS UNDERGONE URETERAL DILATION BEFORE TUL OR OTHER URETEROSCOPIC PROCEDURES.30 OF THEM WERE 2-15 YEARS OLD AGE. URETEROSCOPY AFTER URETERAL DILATION IS EASIER AND MORE SAFE SPECIALLY IN CHILDREN. IT SAVES ABOUT 200,000 RLS (25 US$) PER PATIENT. FALSE PASSAGE OCCURRED IN 48 CASES AND STONE MIGRATION IN 18 CASES.
DISCUSSION & CONCLUSION: DILATION EXPANDS THE SAFETY MARGIN AS THE URETEROSCOPES ARE PASSED, AND IT ALLOWS LARGER FRAGMENTS OF CALCULI TO BE REMOVED. SUCCESSIVELY LARGER URETERAL CATHETERS IS A SAFE AND SIMPLE AND INEXPENSIVE METHOD FOR URETERAL DILATION.
