Abstract: 

PURPOSE: RETROSPECTIVE STUDIES SUGGEST THAT IT COULD BE PREVENTED TO THE ROUTINE PLACEMENT OF URETERAL STENT AFTER UNCOMPLICATED URETEROSCOPY AND LITHOTRIPSY. THIS STUDY WAS DONE PROSPECTIVELY TO DETERMINE WHETHER IT’S NECESSARY TO THE ROUTINE PLACEMENT OF URETERAL STENT AFTER UNCOMPLICATED URETEROSCOPY AND LITHOTRIPSY.
MATERIALS AND METHODS: A TOTAL OF 100 PATIENTS, UNDERWENT UNCOMPLICATED URETEROSCOPY AND INTRACORPOREAL LITHOTRIPSY WITH PNEUMATIC. THE PROCEDURE WAS DONE UNDER GA WITH URETEROSCOPE 8-9. 8FR. AFTER STONE FRAGMENETATION PATIENTS WERE RANDOMIZED INTO TWO GROUPS, STENTED (50) AND NONSTENTED (50). IN THE STENTED GROUP URETERAL CATHETER: 5FR WAS USED FOR 48H. PATIENTS WERE FOLLOWED AFTER 1WEEK WITH K.U.B, AFTER 1 MONTH WITH ULTRASONOGRAPHY AND URINE CULTURE, AND 3 MONTH LATER WITH I.V.P.
RESULTS: THE COMPARISON BETWEEN THE LARGEST STONE SIZE AVERAGE, AND THE OPERATION TIME AVERAGE, SHOWED NO SIGNIFICANT DIFFERENCE IN THE TREATMENT GROUPS, (P=0.71, P=0.621). MORPHINE WAS USED IN 30% OF THE PATIENTS WITHOUT STENT, AND 22% OF THE PATIENTS WITH STENT, DURING HOSPITALIZATION. THE COMPARISON OF THE TWO GROUPS SHOWED NO SIGNIFICANT DIFFERENCE. (P=0.087). ALL THE PATIENTS WERE DISCHARGED WITHIN THE FIRST 24H, EXCEPT THREE PATIENTS IN NON STENTED AND TWO IN STENTED GROUP.16% OF PATIENTS WITH STENT COMPLAINED IRRITATIVE SYMPTOMS AND\OR SUPRAPUBIC PAIN. AFTER DISCHARGE FROM HOSPITAL NONE OF THE PATIENTS, IN THE NON STENTED GROUP, WERE RETURNED TO THE OPERATING ROOM FOR STENTING PLACEMENT. STONE FREE RATE WAS 100% IN THE TWO GROUPS. NO DILATION OR STRICTURE WAS SEEN IN PYELOCALICEAL SYSTEM IN I.V.P, THREE MONTHS AFTER OPERATION.
CONCLUSIONS: THERE WAS NO DIFFERENCE BETWEEN THE STENTED AND NONSTENDED GROUP IN TERMS OF STONE PASSAGE, RENAL FUNCTION RECOVERY, STONE FREE RATE, PREVENT OF URETERAL STRICTURES AND AMOUNT OF PAIN. HOWEVER THERE WAS STENT COST AND SUPRAPUBIC PAIN AND/OR IRRITATIVE SYMPTOMS IN THE STENTED GROUP. THUS, OUR OPINION IS THAT ROUTINE STENT PLACEMENT IS UNNECESSARY AFTER UNCOMPLICATED URETEROSCOPY AND PNEUMATIC LITHOTRIPSY.
