Abstract:

PURPOSE: URETHRECTAL, LIKE IMPERFORATED ANUS OR ACQUIRED DUE TO SURGICAL TRAUMA, LIKE TRANSURETHRAL RESECTION OF PROSTATE (TURP), RADICAL CYSTECTOMY OR PROSTATECTOMY, PELVIC FRACTURE, STAB WOUND AND INFLAMATORY DISORDERS.
IN THIS STUDY WE DISCUSSED ABOUT CONSERNATIVE MANAGEMENT OF THE ACQUIRED FISTUBLAS.
MATERIALS & METHODS: BETWEEN 1999 AND 2002, 5 PATIENTS 10 TO 40 YEARS OLD WITH A URETHRORECTALIS FISTULAS REFERRED TO OUR HOSPITAL. THE ETIOLOGIES OF THE FISTULAS WERE STAB WOUND IN PATIENT, GUNSHOT WOUND IN 1 PATIENT, PELVIC FRACTURE DUE TO CAR ACCIDENT IN 2 PATIENT: AND FOLLOWING SURGERY OF URETHRAL STONE IN 1 PATIENT. ALL OF THE PATIENTS MANAGED CONSERVATIVELY BY URONARY AND FECAL DIVERSIONS. IN THE OTHER HAND, ALL THE PATIENTSUNDERVENT COLOSTOMY AND CYSTOSTOMY. ONE PATIENT HAD A HISTORY OF TWO SURGICAL REPAIR OF THE FISTULAS AND THE OTHER 4 PATIENTS HAD NO HISTORY OF REPAIR. ALL OF THE PATIENTS A VALUATED WITH PERIODICCYSTOGRAPHY AND CYSTOCOPY.
RESULT: IN 4 PATIENT THE URETHRORECTALIS FISTULAS MANAGED BY CONSERVATIVE APPROACH DURING 2 TO 4 MONTHS. IN THE REMAINING PATIENT WHO HAS REPAIREDTWO TIMES THE FISTULAS DID NOT CLOSED BY CONSERVATIVE APPROACH AND THE PATIENT CANDIDTED FOR PERINEAL AND TRANSPUBIC REPAIR.
CONCLUSIONS: CONSERVATIVEMANAGMENT OF URETHRORECTAL FIATULAS IS A SUITABLE APPROACH WITHAN ACCEPTABLE RESULT. SURGICAL MANAGEMENT CAN BE RESERVED FOR PATIENTS WHO DID NOT RESPOND TO CONSERVATIVE APPROACH.
