CORRELATION BETWEEN ANTISPERM ANTIBODIES AND SUCCESSFUL TESTICULAR SPERM EXTRACTION IN AZOOSPERMIC MEN

Abstract: 

OBJECTIVE: IN ORDER TO PREDICT THE PROBABILITY OF RETRIEVING AT LEAST ONE SPERM FROM TESTS, WE CONDUCTED A STUDY TO DEFINE CORRELATION BETWEEN ANTISPERM ANTIBODIES (ASA) AND SUCCESS RATE TESE. SPERMS ARE ANTIGENICALLY RICH CELLS, IN PART, BECAUSE OF THEIR COMPLEX DEVELOPMENTAL AND MATURATION PROCESS.
MATERIALS & METHODS: THE SERUM AND SEMEN ANTISPERM ANTIBODIES OF 63 AZOOSPERMIC PATENTS THAT UNDERWENT TESTICULAR SPERM EXTRACTION FOR ICSI OR DIAGNOSTIC AS CYTOLOGICAL EVALUATION WERE TESTED. THESE TEST WERE DONE BY INDIRECT MAR IGG AND IGA. THE LEVEL OF 0-10 I NEGATIVE AND ALSO MORE THAN 40 INTERFERE THE FERTILITY AND 10-40 IS BORDERLINE BUT FOR THIS PURPOSE MORE THAN 10 CAN BE CONSIDERED POSITIVE. THOSE WITH THE HISTORY OF IMMUNE DISEASES OR USING THE DRUGS THAT SUPPRESS THE IMMUNE SYSTEM WERE EXCLUDED IN THIS STUDY. THE LEVEL OF ANTISPERM ANTIBODIES OF THE PATIENTS WITH FINDING AT LEAST ONE SPERM THROUGH TESE COMPARED WITH OTHERS THAT THE RESULTS OF TESE WERE NEGATIVE. ALSO OTHER PARAMETERS OF THESE PATIENTS LIKE FSH, LH, TESTOSTERONE, HISTORY OF TESTICULAR SURGERY WERE COMPARED.
RESULTS: SPERM WAS FOUND IN THE TESE OF 41 PATIENTS (65%) AND 22 (35%) WERE NEGATIVE. SERUM IGG IS HIGHER RATE THAN THE SEMEN IGA TO SHOW THIS CORRELATION DEFINITELY. SERUM IGG IN 17 PATIENTS (39%) WITH POSITIVE TESE WAS MORE THAN 10 AND ONLY 1 PATIENT ITH IGG (MORE THAN 40) WHOSE RESULT OF TESE WAS NEGATIVE. (P. VALUE=0.000) THE POSITIVE PREDICTIVE VALUE WAS 94% AND NEGATIVE PREDICTIVE VALUE WAS 46%. THE MEAN OF IGG FROM PATIENTS WITH HISTORY OF TESTICULAR SURGERY WAS 3 TIMES HIGHER THAN WITHOUT THIS HISTORY.
CONCLUSIONS: THIS STUDY SHOWS THAT THE MEAN IGG LEVEL OF PATIENTS WITH POSITIVE TESE IS HIGHER THAN THIS PARAMETER IN PATIENTS WITH NEGATIVE SPERM.
ALL PATIENTS WITH HIGH ANTIS PERM ANTIBODIES HAD SPERM. SO POSITIVE ASA AS A PREDICTIVE FACTOR FOR RETRIEVAL SPERM CAN BE VALUABLE TEST, BUT NEGATIVE ASA DOESN'T PREDICT LACK OF SPERMATOGENESIS.
