Abstract: 

PROPOSE: PREVIOUS TOLERANCE-INDUCED REGIMENS BY THYMIC TISSUE, HAVE BEEN FOUND TO BE EFFECTIVE IN RODENTS AND MINIATURE SWINE, HAS MADE IT NECESSARY TO EXAMINE LARGE ANIMAL MODELS BEFORE TESTING OF NEW APPROACHES CLINICALLY. THE GOAL OF THIS STUDY WAS TO INVESTIGATE THE BENEFICIAL EFFECT OF DONOR THYMIC TISSUE TO INDUCE TOLERANCE IN THYMOKIDNEY ALLOGRAFTS, TRANSPLANTED TO THYMECTOMIZED CROSS-BRED CANINES.
METHODS & MATERIALS: ANIMALS: 7 PAIRS OF TRANSPLANT DONORS AND RECIPIENTS WERE SELECTED FROM CROSS-BRED (MISMATCHED IN MAJOR HISTOCOMPATIBILITY COMPLEX (MHC)) CANINES AT 3-4 MONTH (MO) OF AGE. PROCEDURE: (1) CREATION OF THYMOKIDNEY IN DONOR; SEVEN JUVENILE CANINES (3-4 MO OLD) UNDERWENT PARTIAL THYMECTOMY AND RECEIVED AUTOLOGOUS THYMIC GRAFT UNDER THE RENAL CAPSULE. SUCCESSFUL ENGRAFTMENT WITH EVIDENCE OF THYMOCYTE DEVELOPMENT WAS DETERMINED BY, GROSS AND HISTOLOGIC EXAMINATION 3 MO LATER, AT THE TIME OF TRANSPLANTATION. (2) THYMOKIDNEYS (NO=7) WERE TRANSPLANTED INTO MHC-MISMATCHED CROSS BRED CANINE RECIPIENTS (NO=7), ALL OF WHOM WERE THYMECTOMIZED 3 WEEKS PRIOR TO TRANSPLANT. (3) A FEW HOURS AND 3 MO AFTER TRANSPLANTATION WE BIOPSIED THE ALLOGRAFT, FIXED IN FORMALLIN AND STUDIED HISTOLOGICALLY FOR EVIDENCES OF HYPERACUTE OR ACUTE REJECTION.
RESULTS: (1) ALL OF SEVEN JUVENILE THYMIC GRAFTS GENERATED UNDER RENAL CAPSULE WITH NORMAL THYMIC STRUCTURE BY POST OPERATIVE DAY OF 90. (2) ALSO WE DID NOT SEE HYPERACUTE REJECTION IN NEITHER SEVEN ALLOGRAFTS AND ALL OF RECIPIENTS DOING WELL TILL THE END OF FOLLOW UP, BUT ALL OF THE ALLOGRAFTS SHOWED ACUTE CELL MEDIATED REJECTION 3 MO AFTER TRANSPLANTATION.
DISCUSSION & CONCLUSION: ALTHOUGH WE COULD NOT INDUCE TOLERANCE BY VASCULARIZED DONOR THYMOKIDNEYS IN RESIDENCE FOR 3 MO IN THIS MHC-MISMATCHED CROSS-BRED LARGE ANIMAL MODEL BUT MORE INVESTIGATION INCLUDING ADDITION OF SHORT COURSE LOW DOSE IMMUNOSUPPRESANT TO THIS REGIMEN AND/OR APPLICATION OF IN-BRED MHC-MATCHED CANINES IS SUGGESTED.
