Abstract: 

PROPOSE: THE LOWER URINARY TRACT IS A NON-LINEAR MULTIVARIABLE DYNAMIC SYSTEM VARIANT. IN THIS STUDY, WE EVALUATE THE EFFECTS OF DIFFERENT POSITIONS OF URINATION ON UROFLOWMETRIC DATAS.
METHODS & MATERIALS: FROM JANUARY TO MARCH 2003, TEN HEALTHY MEN WITH A MEAN AGE OF 23.6 YEARS (RANGE 19-32) AND TEN MEN UNDERGOING BPH (BENIGN PROSTATIC HYPERPLASIA) WITH A MEAN AGE OF 69.6 YEARS (RANGE 58-76, MEAN AUA SCORE=21) WERE SELECTED. THE PATIENTS WERE INSTRUCTED TO VOID IN 3 POSITIONS: STANDING (ST), IRANIAN-STYLE OR DOWN-SITE (IS), EUROPEAN-STYLE (ES). IN EACH POSITION UROFLOWMETRIC DATAS (MAX FLOW RATE [MAX FR], AVERAGE FLOW RATE [AVE FR], DELAYED TIME TO VOIDING [DT], TIME TO FLOW MAX [TTFM] AND RESIDUAL URINE VOLUME OF BLADDER [RUV] ) WERE MEASURED.
RESULTS: IN HEALTHY MEN, MAXFR, AVE FR, DT, TTFM AND RUV WERE ALMOST SIMILAR IN 3 POSITIONS. ALSO, IN MEN UNDERGOING BPH, MAX FR, DT AND TTFM WERE ALMOST SIMILAR IN 3 POSITIONS. MEAN OF AVEFR WAS HIGHER IN ES POSITION THAN IS AND ST POSITIONS (3.8CC/MIN VS.2.8 AND 3.2; P=.12), BUT THIS FINDING IS INSIGNIFICANT STATISTICALLY. MEAN OF RUV WAS SIGNIFICANTLY LOWER IN ES THAN IS AND ST POSITIONS (84.5CC VS.152.2 AND146; P=0.009).
DISCUSSION & CONCLUSION: USING ES POSITION FOR VOIDING IN BPH PATIENTS MAY RESULTS IN RELATIVE IMPROVEMENT OF AVERAGE FLOW RATE OF URINE AND REMARKABLE DECREASE IN RUV OF BLADDER.
