Abstract:

PROSTATITIS IS DEFINED AS AN INCREASED NUMBER OF INFLAMMATORY CELLS WITHIN THE PROSTATE PARENCHYMA.
THIS DISEASE IS THE MOST COMMON UROLOGIC DIAGNOSIS IN MEN OLDER THAN 50 YEARS (AFTER BPH AND PROSTATE CANCER). PROSTATIC INFLAMMATION MAY BE NOTED IN PATIENTS WITH A DIAGNOSIS OF BPH OR PROSTATE CANCER. THE PURPOSE OF OUR STUDY WAS TO EVALUATE THE PREVALENCE OF PROSTATITIS IN THE PATICNTS WHO WERE ADMITTRD WITH DIAGNOSIS OF BPH AND TREATED WITH TURP, AND IT’S RELATIONSHIP WITH SEVERITY OF NIRRITATIVE LOWER URINARY TRACT SYMPTOMS IN THESE PATIENTS.
MATERIALS AND METHODS: DURING SIX MONTHS WE STUDY ON 18 PATIENTS (MEAN AGE 71 YEARS) ADMITTED IN SH. FAGHIHI HOSP. WITH IMPRCSSION OF BPH. THE SEVERITY OF IRRITATIVE AND OBSTRUCTIVE LUTS WAS ASSESSED BY AUA SCORE QUESTIONARE. AFTER TREATMENT WITH TURP THE PROSTATE SAMPLES WERE STUDIED BY PATHOLOGIST FOR COEXISTING OF PROSTATITIS.
RESULTS: THE MEAN IRRITATIVE SCORE, MEAN OBSTRUCTIVE SCORE AND MEAN TOTAL SCORE IN THESE PATIENTS WERE 13, 12 AND 25 RESPECTIVELY. IN PATHOLOGIC EVALUATION OF SAMPLES AFTER TURP THERE WAS CVIDENCED OF PROSTATITIS IN 14 OF 18 PATIENTS (78%). THE MEAN IRRITATIVE SCORE IN PATIENTS WITH BPH AND PROSTATITIS WAS MORE THAN PATIENTS WITH BPH ALONE. NONE OF THEM HAD PREVIOUS HISTORY OF URETHRAL CATHETERIZATION, URINARY CALCULI OR UTI.
CONCLUSION: HIGH INCIDENCE OF PROSTATITIS IN MAN WHO ARE OPERATED DUE TO BPH IS A WARNING SIGN THE HIGHER FREQUENCY OF IRRITATIVE SYMPTOMS THAN OBSTRUCTIVE SYMPTOMS IS IN FAV OR OF BPH AND PROSTATITIS SIMULTANDOUSLY. IMPORTANCE OF PROSTATITID IN MEN WITH BPH NEEDS MORE WORK UP, BUT NOW IT CAN IT CAN BE SUGGESTED THAT PROPER TREATMENT OF PROSTATITIS IN THESE PATIENTS MAY OBVIATES THE NECESSITY OF OPERATION OR AT LEAST MAY IMPROVES THE RESULT OF OPERATION IN THEM.
