Abstract:

BACK GROUND: IN THIS STUDY WE REPORT OUR EXPERIENCE WITH SNODGRASS TECHNIQUE FOR HYPOSPADIAS REPAIR IN 33 BOYS.
METHODS: FROM APRIL 2001 TO APRIL 2003 SNODGRASS URETHROPLASTY WAS DONE ON 33 BOYS AGED BETWEEN 7 MONTHS TO 11 YEARS (MEDIUM 3 YEARS). THE POSITION OF THE MEATUS WAS DISTAL SHAFT IN 28, AT MED SHAFT IN 4 AND AT PROSIMAL SHAFT IN 1. THERE WAS ONE BOY WITH 2 FAILED PREVIOUS OPERATION WITH MEATUS AT MID SHAFT.
ALL HAD MILD CORDEE THAT WAS CORRECTED WITH PENILE DEGLOVING. IN ONE CASE WITH PROXIMAL MEATUS RESIDUAL CORDEE WAS CORRECTED WITH MIDLINE DORSAL PLICATION.
POST OPERATIVELY THE NEOURETHERA WAS STENTED FOR 7 TO 10 DAYS WITH SALASTIC STENT.
SUPRAPUBIC DIVERSION WAS DONE IN 2 PATIENTS, ONE WITH PROXIMAL HYPOSPADIAS, AND THE OTHER WITH REDO OPERATION.
RESULTS: FOLLOW UP WAS FROM 2 YEARS TO 1 MONTH. IN ALL PATIENTS THERE WAS AN SLIT LIKE MEATUS AT THE TIP OF THE GLANS. THERE WAS 5 FISTULA (14.7%), 4 IN DISTAL SHAFT PATIENTS AND ONE IN THE PATIENTS WITH PROXIMAL HYPOSPADIAS. FISTULA WAS MOSTLY IN THE BEGINNING OF STUDY AND WE THINK IT WAS BECAUSE OF MEATAL STENOSIS.
CONCLUSION: THE RESULT OF THIS STUDY SHOWS THAT SNODGRASS URETHROPLASTY (NOW CALLED T. I. P) GIVES GOOD COSMETIC AND FUNCTIONAL RESULTS. THE COMPLICATION RATE IS LOW AND THE OPERATION IS EASY TO PERFORM COMPERED TO MORE SOPHISTICATED OPERATIONS. IT COULD BE DONE FOR MOST HYPOSPADIAS PATIENTS REGARDLESS OF MEATAL POSITION.
