Abstract:

BACKGROUND: THIS IS THE GENERAL AGREEMENT THAT THE VOIDING PRESSURES IN CHILDREN ARE HIGHER THAN IN ADULTS AND THAT THERE IS A GENERAL HYPER CONTRACTILE BLADDER IN CHILDHOOD. IT IS DEBATED THAT THE BLADDER VOIDING PRESSURE MEASURED BY HEMODYNAMIC TECHNIQUES CAN REVEAL THAT THE BLADDER VOIDING PRESSURE IN CHILDREN IS NOT GENERALLY HIGHER THAN THAT IN ADULTS.
METHODS: WE SET IN THE STAGE OUR STUDY BY MEASURING HEMODYNAMIC BLADDER PRESSURE IN CHILDREN WHO ONLY HAVE PROBLEMS OTHER THAN VESICAL, VESICOURETERAL AND URETHRAL PROBLEMS FOR APPROXIMATING THE NORMAL VALUES IN CHILDREN. THESE CHILDREN HAVE ALREADY HAD SUPRAPUBIC CATHETER IN PLACE BECAUSE OF OTHER PROBLEMS. WE HAVE ALSO REVIEWED THE SACRAL RADIOGRAPHS FOR MEASURING A PARAMETER NAMED SACRAL RATIO. THIS PARAMETER HAS BEEN CONSIDERED IN RELATION TO PRESSURE CHANGES OF THE BLADDER.
RESULTS: WE HAVE EXAMINED 17 CHILDREN (5 FEMALE, 12 MALE) ACCORDING TO THESE STRICT CRITERIA. WE FOUND HIGH VESICAL PRESSURES IN CHILDREN WITH HIGHER SACRAL RATIO LEVELS. THE AGE DISTRIBUTION OF PRESSURES HAS 2 PEAKS ONE IN INFANCY (MEAN 6 MONTHS) AND THE OTHER AT 7 YEARS OF AGE. PRESSURES IN FEMALES WERE HIGHER THAN MALES BUT THE DIFFERENCE WAS NOT SIGNIFICANT.
CONCLUSION: WE HAVE CONCLUDED THAT THE NORMAL VOIDING BLADDER PRESSURE IN CHILDREN ARE NOT HIGHER THAN ADULTS EXCEPT IN CERTAIN CASES WHICH CAN BE CHARACTERIZED BY A NEURO-DEVELOPMENTAL FACTOR NAMED SACRAL RATIO.
