Abstract: 

OBJECTIVE: DUE TO HIGH PREVALENCE OF ERECTILE DYSFUNCTION (ED) IN ELDERLY PATIENTS, ESPECIALLY VASCULAR ED AND BECAUSE MANY BPH PATIENTS AFTER PROSTATECTOMY COMPLAIN OF ED, IN THIS STUDY WE PERFORMED CIS AS AN OBJECTIVE TOOL TO EVALUATED ED BEFORE AND AFTER SURGERY AND TO ESTIMATE THE DEGREE OF VASCULAR INJURY DUE TO OPERATION, ALSO THE OPTIMAL DOSAGE OF DRUGS FOR INDUCTION OF ERECTION IN THESE PATIENTS WERE CALCULATED MATERIALS AND METHODS: IN THIS PROSPECTIVE STUDY 140 BPH PATIENTS DIVIDED IN TWO 70 PATIENTS GROUP. ONE (GROUP 1) UNDERWENT OPEN PROSTATECTOMY AND THE OTHER ONE (GROUP 2) TURP. THE AVERAGE AGE IN GUOUP LAND 2 WAS 67 AND 64 YEARS RESPECTIVELY. HISTORY OF ED WSA PRESENTS IN 67.1% (47) AND (61.4%) (43) OF PATIENTS BEFORE OPERATION. CIS TEST BEGAN WITH PAPAVERIN AND PHENTOLAMINE (20MG+2.5MG) AND AFTER 20 MIN FULL ERECTION WAS EVALUATED. IF IT WAS NEGATIVE.2ND , 3RD DOSES WAS REPEATED UP TO THE MAXIMUM CUMULATIVE DOSE OF 60 MG PAPAVERIN+10 MG PHENTOLAMINE. NEGATIVE CIS WERE INDICATORS OF COMPLETE ED OR INCREASE IN SEVERITY OF IT.
RESULT: IN GROUP 1 AND 2, 7.1% AND 12.8% HAD INCREASE IN SEVERITY OF ED ACCORDING TO CIS TEST RESPECTIVELY IN 41% OF CASES SELF STIMULATION CONVERTED A PARTIAL ERECTION TO A FULL ONE.
CONCUSSION: CIS TEST IS AN INEXPENSIVE AND MINIMALLY INVASIVE DIAGNOSTIC TOOL TO EVALUATE PENILE BLOOD FLOW AND BASELINE ERECTILE FUNCTION IN MEN BEFORE PROSTATECTOMY AND ALSO A SAFE ALTERNATIVE THERAPEUTIC MEAN IN AGED MEN WITH ED
