Abstract: 

INTRODUCTION: THE OBJECTIVE OF THE CURRENT STUDY WAS TO ASSESS THE POTENTIAL USE OF BLADDER ACELLULAR MATRIX FOR AUGMENTATION CYSTOPLASTY.
MATERIALS AND METHODS: WHOLE BLADDER WAS HARVESTED FROM A BRAIN-DEATH HUMAN DONOR, AFTER ALL NECESSARY BACTERIOLOGY AND VIROLOGY TESTS. THE BLADDER WAS RENDERED ACELLULAR USING A THREE-STEP PROTOCOL WITH SODUIM AZIDE, DNASE, AND SODIUM DESOXYCHOLATE. LIGHT MICROSCOPY REVEALED SOME CYTOPLASMIC REMNANTS AMONG THE COLLAGEN BUNDLES. THE ACELLUAR MATERIAL WAS HOWEVER GRAFTED TO 8 RABBIT, AND HARVESTED IN 1, 2, 4, AND 8 WEEK INTERVALS, TWO RABBITS EACH. SECTIONS WERE TAKEN AND H & E STAINED FOR LIGHT MICROSCOPY.
RESULTS: ALL GRAFTS FACED SERIOUS IMMUNE RESPONSE FROM THE HOST BLADDER TISSUE, AND NO GROWTH OF THE HOST TISSUE INTO THE MATRIX WAS OBSERVABLE. THE INSIDE SURFACE OF THE GRAFTS WAS COVER WITH A CALCIFIED MATERIAL AND THERE WAS NO GROWTH OF THE UROTHELIUM ONTO THE GRAFT. ALL THE RABBITS SURVIVED SUBTOTAL CYSTECTOMY (GRAFT HARVESTING).
CONCLUSION: REMAINED CYTOPLASMIC MATERIAL IN THE ACELLULAR MATRIX ELICITS SERIOUS IMMUNE REACTION AND PREVENTS HOST TISSUE FROM GROWTH INTO THE MATRIX. COMPLETE ELIMINATION OF THE CELLULAR COMPONENTS FROM THE MATRIX IS NECESSARY.
