Data were collected by demographic and ADKAR model questionnaires and analyzed by descriptive statistics with SPSS V. 16. Results: Most of the subjects (95%) were female. The mean age of the subjects was 31. 86 years with a mean working experience of 8. 21 years. Moreover, 90% of the samples hold a bachelor's degree and most of them served as an official employee; 67. 2% of nurses worked in university teaching hospitals and 32. 8% in general hospitals. The results showed relative readiness of the nurses (more than half of the subjects got average score above 3 in all components) and the efficiency of the ADKAR model to assess their readiness. Conclusion: According to the results, it is recommended to increase the knowledge and ability of nurses to employ nursing Kardex, strengthen supportive factors, especially in non-educational hospitals, to make the changes, and increase the participation of nurses.
