OTOTOXICITY OF TOPPICAL AND SYSTEMATIC CIPROFLOXACIN IN CHRONIC OTITIS MEDIA

Abstract: 

Background: Chronic otitis media is a common cause of ENT visit. The appropriate drug that has minor ototoxic side effect is of utmost importance to treat these patiellfs. The present study was conducted to determine the ototoxicity of topical alld systemic ciprofloxacin in chronic otitis media.
Materials and methods: A double blind clinical trial was performed on 72 patients. Chronic otitis media was diagnosed on clinical study basis. Our exclusion criteria were: age of <18 years, pregnancy, systemic usage of ototoxic drugs, viral infections, and sensitivity to quinolones. 32 patients have received systemic ciprofloxacin as the control group and 40 have received topical ciprofloxacin as drop and formed our case group. Drugs were prescribed for 10 days. Hearing evaluation was achieved by mans of audiometery and performed at the first and 30th day.
Results: The study population included 20 females and 12 males as the control and 24 females and 16 males as the case group with their age ranges 19-55 years. In the frequency of 250 Hz, the changes in hearing threshold of air conduction were 0.94±6.9 dB and 6.87±12.7 dB for the control and case groups, respectively (p<0.05). These figures were 1.25±10.85 and 5.5±14.19 dB for the frequency of 800 Hz, respectively (p<0.05). Furthermore, the changes in hearing threshold of air conduction was 0.93±6.77 dB and 6.25±12.49 dB for tile control and case groups, respectively in the frequency of 10000 Hz (p<0.5). Regarding the bone conduction the significant difference was observed in the frequency of 4000 Hz (4.84±12.2 dB and 2.5±5.54 dB for the control and case group, respectively (p<0.05).
Conclusion: Topical ciprofloxacin was revealed not to have any ototoxic effect, thus it can be safely used for chronic otitis media.
