LEFT VENTRICULAR EJECTION FRACTION IN PATIENTS WITH CONGESTIVE HEART FAILURE REFERRING TO BOOALI AND IMAM HOSSEIN HOSPITALS, 1999-2000

Abstract: 

Background: Congestive heart failure (CHF) is not always accompanied by abnormal ejection fraction, however, the presence of abnormal ejection fraction may change our therapy strategies. The present study was conducted to determine the association between left ventricular ejection fraction and congestive heart failure in group of Iranian patients referring to Booali and Imam Hossein hospitals, 1999-2000.
Materials and methods: For this descriptive study, all patients with congestive heart failure were included. Our exclusion criteria were: valvular disease, cardiomyopathy, pericardial disease, previous history of hypertension, or receiving ACE inhibitors or diuretics. CHF was diagnosed according to Framingham criteria and ejection fraction was determined by echocardiography. EF of 50 or higher was considered as normal. Blood pressure was determined in two different visit, where, systolic blood pressure of >140 mmHg or diastolic blood pressure of >90 mmHg were considered to be hypertension.
Results: During the study period 86 patients had fulfilled our inclusion criteria among which 50% had normal EF. Abnormal EF was 5.3 times more common in men (p<0.0001l). We have found no significant difference in age. Independent risk factors for normal EF in CHF patients were: a negative history of MI (OR=6.71, p<0.0004), female gender (OR=5.3, p<0.0002), and hypertension (OR=10.1, p<0.0001).
Conclusion: Congestive heart failure is not always associated with abnormal EF. There is a high possibility to find CHF with normal EF in hypertensive females who have not the previous history of MI.
