Abstract: 
Introduction: To evaluate the efficacy of the unfractionated heparin on IVF outcome in patients with repeated IVF embryo transfer failure.
Materials and Methods: A prospective randomized controlled trial was carried out in 86 patients with recurrent IVF-ET failure. Ovarian stimulation was performed with long protocol. The patients were randomly divided into two groups after embryo transfer, and one group received unfractionated heparin 5000 IU twice a day plus 100 mg progesterone and another group only received progesterone.
Results: There were no significant differences between individual characteristics of two groups. However, implantation rate and clinical pregnancy were significantly higher in patients who received unfractionated heparin. Thirty six women had at least one trombophilic mutation.
Conclusion: Using heparin in assisted reproductive cycles could prevent thrombosis in implantation site. Unfractionated heparin was recommended in ART cycle for patients with recurrent IVF-ET failure.
