THE STUDY OF 4 YEAR-MIDDLE LOBE SYNDROME IN BRONCHOSCOPY DEP. OF ZAHEDAN HOSPITAL

Abstract: 

Middle Lobe Syndrome is a condition of chronic atelectasis due to interinsic or extrinsic obstruction of the bronchus of this lobe. This bronchus is relatively long, subtends an acute angle with the intermediate bronchus and is surrounded by a collar of lymph nodes when these lymph nodes become enlarged due to infection or become scarred or even calcified secondary to chronic granulomatous disease, they may cause obstraction of the middle Lobe bronchus, leading to atelectasis with or without infection.
Because the iung disease and MLS are common in this Province and there is not any study in this regard, we decide to study the patients that refered to Brochoscopic department of khatam hospital of zahedan in the last 4 years with diagnosis of MLS. prior Bronchoscopy, questionaire were completed and then Bronchoscopy were done, BAL and biopsy (if necessary) were none. the results Showed that: from 138 patiens (62% male and 38% female) , the most probable diagnosis before FOB was tuberclosis (40.1%), the most finding FOB was erythema and edema of Bronchous . The result of Pathological report were: chronic inflamation (56.1%) , malignancy (21.2%) and tuberclusis (12.4%).
Conclusion: the most common cause of Middle Lode Syndrome in this study was chronic inflammation.
